为了能够及早发现和研究骨科植入物相关感染，以避免骨髓炎和二次外科手术的并发症，我们引入了XELCI作为一种新颖的功能成像技术。它与目前可用的通过组织监测pH的技术相当。

在美国，每年约有200万个骨折固定装置入，其中5%-10%导致种植体相关感染1。由于生物膜的异质性和抗生素耐药性，这些感染在后期阶段更难用抗生素治疗2，3。如果早期诊断，可以用抗生素和手术清创治疗感染，以防止第二次手术的额外医疗费用，以更换治疗骨折部位的硬件。X线平片和其他先进的放射线照相技术应用于骨科植入物相关感染、不愈合和相关并发症的诊断。尽管这些技术经常用于获取骨科植入物周围骨骼和组织的结构信息，但它们无法在特定环境中提供生化信息。因此，我们开发了一种新颖的X射线激发发光化学成像（XELCI）技术，用于在植入部位无创地对生化信息进行高分辨率成像。骨科植入物相关感染的诊断通常通过一种或多种不同的方法进行。临床观察（疼痛、肿胀、发红、伤口分泌物等）提示感染的最初迹象。之后，进行放射学和实验室实验以确认骨愈合进展的失败并确定病原微生物4，5。核医学技术，如计算机断层扫描（CT），磁共振成像（MRI）和放射性核苷酸方法，如单光子发射计算机断层扫描（SPECT）和正电子发射断层扫描（PET）用于更好地可视化感染植入物和相关感染6，7。CT和MRI分别有利于确定骨坏死和软组织异常，但在金属植入物的近距离处会引起干扰8。不同的X射线方法（如SPECT和PET）与放射性同位素标记的分析物（如体内成像造影剂）相结合，广泛用于诊断植入物相关性骨髓炎2。当前的应用将来自CT扫描的数据和来自SPECT或PET的标记数据结合起来，以生成解剖学信息9。尽管这些成像方式中的一种或多种用于帮助感染诊断，但它们无法及早检测到与感染相关的pH变化，从而开始使用抗生素治疗以避免额外的医疗和手术费用。
利用本研究中使用的成像系统监测植入物相关感染的主要优点是它能够通过光谱参考揭示有关生物膜微环境的生化信息。虽然主要重点是成像和绘制感染部位的pH值，但可以改变这种方法以监测植入物相关感染的其他特异性生物标志物。因此，XELCI可以了解感染的病理生理学。高空间分辨率成像允许随着感染的增长绘制异质性。发生生物膜形成的表面的pH值对于理解生化变化非常重要。此外，由于细菌10，11的抗生素相关应激反应，可能会发生其他微环境变化。由于表面特异性和高空间分辨率成像，可以监测抗生素对生物膜微环境的影响。该技术还可用于研究靶向药物递送实验的生物膜环境。我们可以研究有针对性的低pH药物释放或提高pH值，使它们更容易在较高的pH下起作用。
这种成像技术的三个具体特征是X射线分辨率，植入物表面特异性和化学灵敏度（图1A）。这些特征可以与目前可用的用于成像骨科植入物相关感染的成像技术进行比较（图1B）。一旦用X射线照射，涂覆在植入物表面的荧光粉颗粒就会产生红色和近红外（NIR）光，可以穿透几厘米的组织（尽管有一些衰减）12，13。 表1 显示了所开发的成像系统与用于测量生物膜中或通过组织pH的其他方法相比的一些特征。
XELCI是一种新颖的成像技术，结合X射线激发，在植入式医疗设备附近光学获取高空间分辨率化学信息，如图 2所示。这里利用了X射线可激发荧光粉颗粒的选择性激发和光学检测。植入物涂有两层，在一层闪烁体颗粒上掺入pH敏感染料的聚合物层。一旦一系列聚焦的X射线束照射植入物，闪烁体层就会产生可见光（620 nm和700 nm）。产生的光通过pH敏感层，根据周围环境的pH调节发光光谱。低pH值通常与感染和生物膜形成有关;随着感染的进展，pH值从生理pH值（pH 7.2）变为酸性（低于pH 7），传感器中的pH染料会改变颜色，从而改变吸光度。 图2E显示了溴 甲酚绿色pH染料在pH 7和pH 4下的发光光谱的变化。收集通过组织和骨骼的透射光，光谱比决定pH值。为了生成pH图像，聚焦的X射线束一次照射闪烁体薄膜中的一个点，并逐点扫描样品上的光束。以前，该技术应用于骨科植入物14，15 表面的pH变化成像，并对其进行了测试以监测髓内管通过骨骼和组织的pH变化。
下面的图3显示了成像系统的示意图。成像系统的基本组件是带有聚毛细管光学元件的X射线激发源，连接到两个光电倍增管的一体式丙烯酸光导，x，y和z电动载物台（30 cm x 15 cm x 6 cm行程）和连接用于数据采集的计算机。X 射线源、x、y、z 载物台和收集光学元件（弯头、光导、光电倍增管 （PMT））位于 X 射线防护外壳中，而 X 射线控制器、PMT 电源、连接到数据采集 （DAQ） 板的函数发生器和计算机则保存在外部。位于外壳和门前部之间的按钮常开开关用作联锁。如果门没有完全关闭（联锁开关打开），X射线源不会打开，如果在操作过程中打开X射线源，它会自动关闭X射线源。电机可以执行连续扫描，也可以移动到任何离散位置。y轴的扫描速度通常为1-5 mm/s，而x轴上的步长通常可在150-2000μm之间选择。可以根据所需的空间分辨率选择参数。均匀的曝光时间通过整个连续扫描的一致速度来确认。
一旦聚焦的X射线束照射在X射线发光粒子上，产生的光将通过根据周围的pH调节光来穿过pH敏感膜。透射光将与组织相互作用（部分散射和吸收），而散射和吸收的光衰减将随着组织厚度的增加而增加。该系列光学元件包括一个一体式分叉丙烯酸光导，起点配有反光铝弯头（具有 90° 弯曲和抛光反光内表面）。这是为了确保光线到达光导后立即准直。这些添加显著提高了光收集效率。有关更多详细信息， 图4 显示了弯头和光导的机器图纸。90°弯头由铝加工而成，内表面抛光至镜面，光导用亚克力加工。我们还在弯头的起点安装了宽范围的长通蓝光滤光片（阻挡350-450nm的光），以确保只有红光会通过。一体式丙烯酸光导的末端分叉成两个流，导致两个不同的PMT。PMT被封装在一个与热电冷却器接触的小型光密金属盒中，以将PMT冷却到~5°C。 在其中一个PMT开始时，连接了一个窄范围长通滤光片（阻挡570-640 nm光并通过640-740 nm光）以仅测量700 nm光。因此，620 nm和700 nm光可以分别计算。PMT设置为光子计数模式，它们为每个检测到的光子生成晶体管-晶体管逻辑（TTL）脉冲。DAQ 系统 使用 USB 通信 计算 脉冲 （饱和 点 每秒 2000 万 脉冲） 计数。处理数据后生成两个单独的强度图，并通过考虑信号波长强度（620 nm）与参考波长强度（700 nm）的比值来创建最终图像。该比率解释了总光收集效率的差异，这在很大程度上取决于收集光学器件的位置、X 射线照射强度和组织厚度。此外，没有任何pH指示剂染料的空间分离参比区域解释了波长依赖性组织穿透引起的光谱失真。基于图形的编程语言用于控制成像系统，操作的基本流程图如下所示。除计算机、X 射线控制器和 DAQ 单元外，成像设置都封闭在安全的 X 射线外壳中，以最大限度地减少辐射暴露。
