Este artículo describe una batería de pruebas utilizadas para evaluar clínicamente el rendimiento auditivo de usuarios experimentados de implantes cocleares actualizados a una nueva estrategia de codificación de estructura fina. Las pruebas proporcionan orientación tanto para los usuarios de IC como para los candidatos y médicos para elegir diferentes tecnologías, así como para dirigir su rehabilitación clínica. Prepare a los participantes para el experimento colocándolos a un metro del altavoz en un ángulo de 45 grados con respecto al lado del implante coclear en la cabina de sonido.
Quítese los audífonos del oído contralateral y asegúrese de que el tapón para los oídos o la mascarilla de la manguita sean efectivos para los participantes con audición residual. Informe a los participantes que se llevarán a cabo sesiones de prueba de práctica seguidas de pruebas formales. Lleva a los participantes y a sus guardianes a la sala de mapeo.
Siente a los participantes en la sala. Ingrese la contraseña para desbloquear el software de mapeo. Retire el procesador de voz y conéctelo a la caja max con un cable de programación.
Elija el nombre del participante en el software y luego haga clic en la opción de impedancia. Pruebe la impedancia del electrodo para asegurarse de que esté dentro del rango normal de 2,2-12 kiloohmios. Asegúrese de que la estrategia de codificación sea FS4 y de que se utilice una frecuencia de pulso estándar de 12 pulsos por segundo.
Ajuste la estimulación de un solo electrodo a dinámica con tres barridos. Deje que los participantes distingan el volumen de cada electrodo señalando la imagen apropiada en una escala pictórica de confort fuerte. Utilizando métodos de prueba ascendentes y descendentes, realice los mismos resultados que el resultado final de la estimulación eléctrica.
Para establecer el nivel máximo de comodidad, active el mapa pulsando el botón En vivo. Active la función de cambio e inclinación para devolver a los participantes al modo de ajuste. Presione Estrategia para establecer la tasa de estimulación en 1, 274 pulsos por segundo y la secuencia de muestreo específica del canal en 4.
Establezca el espacio de la interfaz de pulso bifásico en 2,1 microsegundos. Finalmente, establezca la relación de compresión en 3:1 y la sensibilidad en 75%Verifique que el valor umbral para cada canal sea el rango de frecuencia de 70-8, 500 hercios. Pruebe la percepción del habla en este orden.
Una vez que los participantes estén sentados, como se demostró anteriormente, inicie el software de voz. Pida a los participantes que repitan claramente el contenido que escuchan. Encienda el audiómetro y seleccione las opciones de audición.
Ajuste el volumen del sonido a un nivel de audición de 70 decibelios por encima del umbral puro promedio de 500, 1, 000, 2, 000 y 4, 000 hercios. Para las pruebas formales, presente a los participantes pruebas de práctica. Pida a los participantes que repitan las palabras y oraciones de cada prueba.
Mantener el orden de los contenidos aleatorio y reproducirlo una vez. Para probar el reconocimiento de oraciones en condiciones ruidosas, realice una prueba de tono con el software configurado a 60 decibelios y ajuste la relación señal/ruido a 10 decibelios utilizando el balbuceo de cuatro hablantes. Luego, en la misma cabina, interprete los resultados cuidadosamente.
Después de confirmar que todos los participantes están familiarizados con el vocabulario de la prueba, presente las pruebas de práctica simultáneamente con la prueba formal. Pida a los participantes que digan lo que han escuchado una vez. Presione Test seguido de ToneID para elegir el tono en el que los participantes repiten el contenido, manteniendo el orden aleatorio.
Para realizar la prueba de música, haga clic en el software de música y elija la selección de tono en la misma cabina. Después de presentar tanto las pruebas de práctica como las formales, instruya a los participantes para que escuchen los estímulos presentados secuencialmente y determinen cuál de los dos tiene un contorno de tono descendente o ascendente. Registre las respuestas de los participantes y repita.
El reconocimiento de espondee en condiciones silenciosas mejoró significativamente desde antes de la actualización hasta tres meses después de la actualización. La mejora fue insignificante desde antes de la actualización hasta seis semanas después de la actualización o desde antes de la actualización hasta inmediatamente después de la actualización. Se observó una mejora significativa en el reconocimiento de monosílabos en condiciones tranquilas desde los sujetos previos a la actualización hasta los inmediatamente posteriores a la actualización.
Se observó una mejora del 59,6 % antes de la actualización al 71,4 % a las seis semanas posteriores a la actualización, mientras que se observó un aumento al 82,1 % a los tres meses después de la actualización. El reconocimiento de frases en condiciones silenciosas mejoró significativamente desde antes de la actualización hasta tres meses después de la actualización. Los otros grupos no mostraron mejoras significativas.
En condiciones ruidosas, el reconocimiento de sentencias aumentó de antes de la actualización a tres meses después de la actualización. Se observó una mejora significativa en el reconocimiento de tonos desde antes de la actualización hasta seis semanas después de la actualización y tres meses después de la actualización, mientras que no se observó ninguna mejora significativa inmediatamente después de la actualización. No se observó una mejora significativa en la percepción del tono musical de 16,5 limen antes de la actualización a 23,7 limen.
Este procedimiento proporciona métodos de evaluación efectivos para la rehabilitación clínica y se utilizará en la próxima etapa de la investigación relacionada para evaluar los efectos clínicos en pacientes con IC.
