Anatomische hepatectomie is een procedure die tegelijkertijd de laesie en de leversegmenten samen met de bijbehorende aderen kan verwijderen en wordt beschouwd als een ideale methode voor de behandeling van leverkanker23,24,25,26. Met technologische innovatie heeft anatomische leverresectie met laparoscopische technologie zich snel ontwikkeld als alternatief voor conventionele open leverresecti...

Hepatocellulair carcinoom is een veel voorkomende kanker; Het is het zesde meest voorkomende neoplasma bij volwassenen en de derde belangrijkste doodsoorzaak van kanker wereldwijd, en de incidentie ervan zal naar verwachting in de toekomst stijgen1. Chirurgische resectie, ablatieve elektrochemische therapie, transarteriële chemo-embolisatie, systemische therapie zoals sorafenib en transplantatie zijn naar verluidt effectieve behandelingsmodaliteiten voor leverkanker 2,3. Van deze opties wordt chirurgische resectie van hepatocellulair carcinoom (HCC) beschouwd als de primaire curatieve behandeling, omdat de tumor volledig kan worden verwijderd in plaats van beperkt4.
Laparoscopische chirurgie, een minimaal invasieve techniek met minder perioperatieve complicaties in vergelijking met open resectie5, heeft wereldwijd grote vooruitgang geboekt en is gestaag een belangrijke chirurgische methode geworden voor leverchirurgie 6,7,8. Bij laparoscopische leverresectie hebben het onvermogen van de chirurg om de tumormarges onder direct zicht te herkennen en de angst om laparoscopische hemostase niet te kunnen garanderen, de meeste leverchirurgen echter ontmoedigd om deze veeleisende procedure te proberen. In 1960 rapporteerden Lin et al. een geval van rechter leverlobectomie met intrahepatische poortader pedikelligatie9. In 1986 beschreef Takasaki ook Glisson's pedikel transect hepatectomie, genaamd extrathecale dissectie10. In 1991 pasten Reich et al. laparoscopische resectie van goedaardige levertumoren toe en voltooiden 's werelds eerste laparoscopische hepatectomie11. Sindsdien is anatomische hepatectomie geleidelijk in de publieke opinie gekomen en biedt het technische ondersteuning voor laparoscopische hepatectomie. In het geval van deze studie bereikte het onderste uiteinde van de tumor echter de cystische plaat en eenvoudige traditionele anatomische resectie kon geen R0-resectie garanderen, maar de behandeling van dergelijke gevallen is zelden in detail gerapporteerd. In 1999 stelden Neuhaus et al. het principe van totale poortaderresectie voor, wat een goede prognostische indicator bleek te zijn, waardoor de kans op R0-resectie12 werd vergroot. Dienovereenkomstig, met een nieuw begrip van de anatomie van de lever, hebben we een nieuwe benadering ontwikkeld genaamd "en bloc concept gecombineerd met anatomische resectie", die wordt weergegeven in dit videoprotocol.
In dit onderzoek was patiënte een 67-jarige vrouw die in augustus 2021 in ons ziekenhuis werd opgenomen met milde bovenbuikpijn gedurende 1 maand. Haar medische geschiedenis was opmerkelijk voor hypertensie en diabetes. Abdominale contrastversterkte computertomografie toonde een massa met heterogene versterking in segment 4 van de lever, met een grootte van 247 mm x 54 mm x 50 mm. Het onderste uiteinde van de massa had de cystische plaat bereikt en de mogelijkheid van een galblaasinvasie kon niet worden uitgesloten (figuur 1). De Child-Pugh leverfunctie13 was graad A en de ICG-klaring 14,15 R15 was 5,1% (<10%). De patiënt werd geclassificeerd als stadium A volgens het BCLC-algoritme16 en stadium IB volgens het CNLC-algoritme17. Na een multidisciplinair overleg werd besloten dat haar behandeling laparoscopische linkerkwabresectie van de lever en cholecystectomie moest zijn. Het concept van en bloc resectie in combinatie met anatomische leverresectie bij laparoscopie werd aangenomen om de enorme levermassa volledig te elimineren.

Laparoscopische hepatectomie is in veel onderzoeken gemeld en het is de reguliere methode voor leverresectie. In sommige specifieke gevallen, zoals wanneer er tumoren naast het cystische bed liggen, kunnen chirurgen de chirurgische marges niet palperen via de laparoscopische benadering, wat leidt tot onzekerheid over R0-resectie. Conventioneel wordt de galblaas eerst gereseceerd en de leverkwabben of segmenten worden vervolgens gereseceerd. In de bovenstaande gevallen kunnen tumorweefsels echter worden verspreid. Om dit probleem aan te pakken, gebaseerd op de herkenning van de porta hepatis en intrahepatische anatomie, stellen we een unieke benadering van hepatectomie voor in combinatie met galblaasresectie door en bloc anatomische resectie in situ. Ten eerste, na het ontleden van de cystische ductus, zonder de galblaas primair te snijden, wordt de porta hepatis vooraf afgesloten door de ureter met één lumen; ten tweede wordt de linker hepatische steel vrijgemaakt door de opening van het Laennec-membraan en de Hilar-plaat; Ten derde wordt de assistent gevraagd om de fundus van de galblaas te slepen en wordt het leverparenchymweefsel gereseceerd met behulp van een harmonisch scalpel langs de ischemielijn op het leveroppervlak en intraoperatieve echografie. De hele middelste leverader (MHV) en zijn zijrivieren lijken volledig; ten slotte wordt de linker leverader (LHV) losgekoppeld en wordt het monster uit de buikholte gehaald. De tumor, galblaas en andere omliggende weefsels worden en bloc gereseceerd, wat voldoet aan het tumorvrije criterium, en een brede incisusmarge en R0-resectie worden bereikt. Daarom is de laparoscopische hepatectomie met de combinatie van het en bloc concept en anatomische resectie een veilige, effectieve en radicale methode met een laag postoperatief recidief en metastase.

De duur van de operatie was 255 min, er werden geen complicaties waargenomen tijdens de operatie en het geschatte bloedverlies was minder dan 20 ml. De operatie werd niet omgezet in een open operatie en er werden geen postoperatieve complicaties gezien. Talrijke studies hebben de voordelen van anatomische resectie aangetoond. Niettemin blijft de vraag of anatomische resectie de R0-resectiesnelheden kan verhogen, controversieel. Bijgevolg beschrijft deze studie een innovatieve procedure met het en bloc concept in combinatie met anatomische resectie in laparoscopische hepatectomie, die postoperatief recidief en metastase kan verminderen.

Met deze minimaal invasieve methode is het verkrijgen van een breed scala aan negatieve marges gemakkelijker en wordt de verspreiding van tumorweefsel vermeden. Deze methode helpt bij het maximaliseren van R0-resectie en het voorkomen van perioperatieve complicaties. Deze techniek kan worden gebruikt bij laparoscopische leverresectieoperaties met resectie van vier tot vijf segmenten.Omdat deze minimaal invasieve chirurgische methode ervaring met open operaties vereist, wordt een geschikte open chirurgietraining aanbevolen. Nadat u de patiënt op de procedure hebt voorbereid, trekt u de fundus van de galblaas omhoog en gebruikt u een harmonisch scalpel voor Calot's Triangle Dissociation. Zorg ervoor dat u het cystische kanaal en de slagader met middelgrote hemostatische clips ligaat.Vermijd het snijden van de galblaas en laat de galblaas voornamelijk in situ. Scheid zorgvuldig de linker coronaire en driehoekige ligamenten, vermijd letsel aan de aangrenzende phrenische adertakken. Bereid je vervolgens voor op hepatische instroomocclusie door van links naar rechts achter het hepatoduodenale ligament te gaan via een mitella verwijderd van de aangehechte 14F enkele menselijke katheter.Na het liggeren van de linker leverslagader met middelgrote vasculaire clips ervoor zorgen dat de eerste porta hepatis is geblokkeerd met de katheter met één lumen om onverwachte bloedingen tijdens de mobilisatie van de linker leverpedikel te voorkomen. Til voorzichtig de inferieure rand van de lever op, bevrijd de linker glissonese steeltjes door het Laennec-membraan en de hilaire plaatspleet en bereid vervolgens een tourniquetsysteem voor om de linker leverinstroom te blokkeren door een 8F enkele menselijke katheter door de linker glissonese pedikel in te brengen. Na het loslaten van de eerste porta hepatis, blokkeert u de linker hepatische pedikel door de katheter met één lumen vast te klemmen met een hemostatische clip die na parenchymtranssectie zal worden geniet.Volg de scheidslijn om de projectieposities van de middelste leveraders te zoeken en te markeren met laparoscopische echografie. Besteed aandacht aan het in kaart brengen van de locatie en trajecten van het vitale intrahepatische vat, vooral die op het verwachte transversale vlak van het leverparenchym. Sleep de fundus van de galblaas met behulp van een ultrasoon scalpel met de hulp van een assistent en transect het parenchym van de voet naar de hoofdzijde langs de middelste leverader.Activeer het ultrasone scalpel vroeg om leverparenchymale bloedingen effectief te verminderen en klem de scalpelpunt niet vast om vaatschade te voorkomen. Beveilig de grote intrahepatische vaten en galwegen die tijdens de parenchymale resectie worden aangetroffen met 2-O-hechtingen indien nodig. Leg nu de vasculaire pedikels bloot die instromen in de segmenten 4A, 4B en de linker leverader in de snijlijn.Leg vervolgens de hele middelste leverader en zijn zijrivieren volledig bloot en ontleed later de wortels van de linker- en middelste leverader. Gebruik monopolaire elektrocoagulatie voor hemostase wanneer bloedingsvlekken op het snijoppervlak worden gevonden. Wikkel de gereseceerde linker leverkwab in een plastic zak.Haal het eruit via een vier centimeter lange incisie in de onderbuik en plaats twee drainagebuizen. Het gereseceerde leverweefselmonster met een aangehechte galblaas wordt getoond. De paraffinepathologie van de tumor bevestigde hepatocellulair carcinoom dat 2,2 centimeter verwijderd was van de negatieve leverresectiemarge.Er werd geen abnormale heterogene versterking waargenomen in het resterende leverparenchym na zes dagen en één maand na de operatie. Het is belangrijk om te voorkomen dat u de galblaas doorsnijdt en de galblaas in de eerste plaats in hechting laat. Maak ook de linker Glissonese steeltjes vrij door de opening tussen het Laennec-membraan en de hilaire plaat.Deze methode kan ook worden gebruikt voor hepatische poortblokkade met hechtingen die de hilar van de lever blokkeren. De blokkeringstijd is echter moeilijk te controleren.
