כריתת כבד אנטומית היא הליך שיכול להסיר בו זמנית את הנגע ואת קטעי הכבד יחד עם הוורידים המתאימים ונחשב לשיטה אידיאלית לטיפול בסרטן הכבד23,24,25,26. עם החדשנות הטכנולוגית, כריתת כבד אנטומית בטכנולוגיה לפרוסקופית התפתחה במהיר?...

קרצינומה הפטוצלולרית היא סרטן נפוץ; זהו הניאופלזמה השישית בשכיחותה במבוגרים והגורם השלישי למוות מסרטן בעולם, ושכיחותה צפויה לעלות בעתיד1. כריתה כירורגית, טיפול אלקטרוכימי אבלטיבי, כימואמבוליזציה טרנס-עורקית, טיפול סיסטמי כגון סורפניב והשתלה דווחו כשיטות טיפול יעילות לסרטן הכבד 2,3. מבין אפשרויות אלה, כריתה כירורגית של קרצינומה הפטוצלולרית (HCC) נחשבת לטיפול המרפא העיקרי מכיוון שניתן להסיר את הגידול לחלוטין ולא מוגבל4.
ניתוח לפרוסקופי, טכניקה זעיר פולשנית עם פחות סיבוכים פרי-ניתוחיים בהשוואה לכריתה פתוחה5, התקדם מאוד ברחבי העולם והפך בהדרגה לשיטה כירורגית חשובה לניתוחי כבד 6,7,8. עם זאת, בכריתת כבד לפרוסקופית, חוסר יכולתו של המנתח לזהות את שולי הגידול תחת ראייה ישירה והחשש שלא יוכל להבטיח המוסטזיס לפרוסקופי הרתיעו את רוב מנתחי הכבד מלנסות הליך תובעני זה. בשנת 1960, לין ועמיתיו דיווחו על מקרה של כריתת אונה ימנית בכבד עם קשירת ורידים פורטלית תוך כבדית9. בשנת 1986 תיאר טאקסאקי גם את כריתת הכבד של גליסון, שנקראה דיסקציה אקסטרתטית10. בשנת 1991, רייך ועמיתיו יישמו כריתה לפרוסקופית של גידולי כבד שפירים והשלימו את כריתת הכבד הלפרוסקופית הראשונה בעולם11. מאז, כריתת כבד אנטומית נכנסה בהדרגה לעיני הציבור תוך מתן תמיכה טכנית לכריתת כבד לפרוסקופית. עם זאת, במקרה במחקר הנוכחי, הקצה התחתון של הגידול הגיע לצלחת הציסטית, וכריתה אנטומית מסורתית פשוטה לא יכלה להבטיח כריתה R0, אך ניהול מקרים כאלה דווח רק לעתים רחוקות בפירוט. בשנת 1999, Neuhaus et al. הציעו את העיקרון של כריתה כוללת של ורידים פורטליים, אשר הוכיח להיות אינדיקטור פרוגנוסטי טוב, להגדיל את הסיכוי של R0 כריתה12. בהתאם לכך, עם הבנה חדשה של אנטומיה של הכבד, קידמנו גישה חדשה הנקראת "מושג en bloc בשילוב עם כריתה אנטומית", המתוארת בפרוטוקול וידאו זה.
במחקר זה, המטופלת הייתה אישה בת 67 שאושפזה בבית החולים שלנו באוגוסט 2021 עם כאבי בטן עליונה קלים למשך חודש. ההיסטוריה הרפואית שלה הייתה בולטת ביתר לחץ דם וסוכרת. טומוגרפיה ממוחשבת משופרת ניגודיות בטן חשפה מסה עם שיפור הטרוגני הממוקם בקטע 4 של הכבד, בגודל של 247 מ"מ x 54 מ"מ x 50 מ"מ. הקצה התחתון של המסה הגיע לצלחת הציסטית, ולא ניתן היה לשלול את האפשרות של פלישה לכיס המרה (איור 1). תפקודי הכבד של Child-Pugh13 היו בדרגה A, ושיעור פינוי ה-ICG 14,15 R15 היה 5.1% (<10%). החולה סווג כשלב A על פי אלגוריתם BCLC16 ושלב IB על פי אלגוריתם CNLC17. לאחר פגישה רב תחומית, הוחלט כי הטיפול בה צריך להיות כריתה לפרוסקופית של האונה השמאלית של הכבד וכריתת כיס השתן. הרעיון של כריתה en bloc בשילוב עם כריתה אנטומית של הכבד בלפרוסקופיה אומץ כדי לחסל את מסת הכבד העצומה לחלוטין.
