간세포 암종은 흔한 암입니다. 성인에서 6번째로 흔한 신생물이며 전 세계적으로 암 사망의 세 번째 주요 원인이며 발병률은 미래에 증가할 것으로 예상됩니다1. 외과적 절제술, 절제 전기화학 요법, 경동맥 화학색전술, 소라페닙과 같은 전신 요법 및 이식이 간암에 효과적인 치료 방식인 것으로 보고되었다 2,3. 이러한 옵션 중에서 간세포암(HCC)의 외과적 절제가 1차 근치적 치료법으로 간주되는데, 이는 종양을 제한적으로 제거하지 않고 완전히 제거할 수 있기 때문이다4.
복강경 수술은 개복 절제술에 비해 수술 전후 합병증이 적은 최소 침습 기법이다5, 전 세계적으로 큰 발전을 이루었으며 간 수술의 중요한 수술 방법이 되었다 6,7,8. 그러나 복강경 간 절제술에서 외과의는 직접적인 시야에서 종양 가장자리를 인식할 수 없고 복강경 지혈을 보장할 수 없다는 두려움으로 인해 대부분의 간 외과의는 이 까다로운 절차를 시도하지 못하게 되었습니다. 1960년, Lin 등은 간내 문맥 척추경 결찰술을 이용한 우측 간엽 절제술 사례를 보고했다9. 1986 년 Takasaki는 또한 Extrathecal dissection10이라는 Glisson의 척추 경 횡단면 간 절제술을 설명했습니다. 1991년 Reich 등은 양성 간 종양의 복강경 절제술을 시행하여 세계 최초의 복강경 간 절제술을 완료했다11. 그 이후로 해부학적 간 절제술은 복강경 간 절제술에 대한 기술적 지원을 제공하면서 점차 대중의 눈에 들어왔습니다. 그러나 본 연구의 경우 종양의 하단이 낭성 판에 도달하여 단순한 전통적인 해부학 적 절제술로는 R0 절제를 보장 할 수 없었지만 이러한 경우의 관리에 대해서는 상세하게보고되지 않았습니다. 1999년 Neuhaus et al. 문맥 전절제술의 원리를 제안했는데, 이는 좋은 예후 지표로 입증되어 R0 절제술 가능성을 높였습니다12. 따라서 간 해부학에 대한 새로운 이해와 함께 우리는 이 비디오 프로토콜에 묘사된 "해부학적 절제술과 결합된 블록 개념"이라는 새로운 접근 방식을 발전시켰습니다.
본 연구에서 환자는 2021년 8월 본원에 입원한 67세 여성으로 1개월간 경미한 상복부 통증을 호소하였다. 그녀의 병력은 고혈압과 당뇨병으로 유명했습니다. 복부 조영 증강 컴퓨터 단층 촬영은 247mm x 54mm x 50mm 크기의 간 세그먼트 4에 위치한 이질적인 조영 증강 종괴를 나타냈습니다. 덩어리의 하단이 낭성 판에 도달했으며 담낭 침범 가능성을 배제 할 수 없었습니다 (그림 1). Child-Pugh 간 기능13은 A 등급이었고 ICG 제거율 14,15 R15는 5.1%(<10%)였습니다. 환자는 BCLC 알고리즘16에 따라 스테이지 A 및 CNLC 알고리즘17에 따라 스테이지 IB로 분류되었다. 다학제적 회의 후, 그녀의 치료는 복강경 좌엽 간 절제술과 담낭 절제술로 결정되었습니다. 복강경 검사에서 해부학적 간 절제술과 결합된 블록 절제술의 개념은 엄청난 간 덩어리를 완전히 제거하기 위해 채택되었습니다.
