不妊症は男性または女性の生殖器系の健康問題であり、世界中で約1億8600万人の生殖年齢の個人が罹患しています1。不妊症のカップルの少なくとも35%で、不妊症は女性の生殖器系の障害によって引き起こされます2。遺伝的要因、生殖管異常、内分泌機能障害、炎症性疾患、医原性治療など、女性の不妊症を引き起こす可能性のある多くの要因があります3。
遺伝的異常は、不妊症の女性の約10%に存在します4,5。すべての遺伝的異常の中で、X染色体異常は卵巣発育不全の最も一般的な原因です2。いくつかの研究では、ターナー症候群(TS)またはトリプルX症候群の女性のX染色体異常は、生殖細胞の喪失の加速または卵形成障害による早期卵巣不全に関連していることが報告されています6,7,8。
X染色体の異常は、1)X染色体の数が異なっているがX染色体が損なわれていない数値異常に分けることができます。2)X染色体が遺伝物質を獲得または失った構造異常3,9。X染色体の数値異常は、構造異常よりも一般的であり、細胞分裂中の自発的なエラーによって引き起こされることがよくあります3,9。減数分裂中にこのようなエラーが発生すると、異数性配偶子につながり、最終的にはすべての細胞に染色体異常を伴う子孫につながる可能性があります。個体発生の初期段階の有糸分裂中に発生するエラーの結果として染色体細胞に染色体欠損が生じると、モザイク現象につながる可能性があります。これらの個体では、X染色体含有量が正常な細胞とX染色体異常を有する細胞の両方が存在する。
1980年代には、中期および間期の染色体上の特定の核酸配列を視覚化して位置を特定するために、蛍光in situハイブリダイゼーション(FISH)と呼ばれる細胞遺伝学的手法が開発されました10,11。この手法では、蛍光標識されたDNAプローブを使用して染色体内の特定の配列に結合し、蛍光顕微鏡を使用して視覚化できます。
現在、FISHは臨床診断ツールとして広く使用されており、染色体異常を検出する際のゴールドスタンダードと見なされています10。生殖医療の分野では、精子に関するFISH分析を使用して、体細胞に数値的または構造的な染色体異常がある男性の精子のX染色体含有量に関する洞察を得ています12,13,14。これらの研究は、染色体異常のある男性は、正常な核型の男性と比較して、精液中に存在する異数性精子の頻度が高い可能性が高いことを示しました12,13,14。
精子とは対照的に、染色体異常を有する個体における卵巣細胞(卵母細胞、顆粒膜/テカ細胞、および間質細胞を含む)のX染色体含有量、およびこれらの細胞の異数性が生殖能力に及ぼす可能性のある結果についてはほとんど知られていない。精子と比較して卵巣細胞の核型に関する情報が不足している重要な理由は、女性が卵母細胞または卵巣皮質組織を得るために卵胞穿刺または手術などの侵襲的処置を受けなければならないという事実である。したがって、女性の配偶子は研究目的で入手することは困難です。
現在、TS15の若い女性における卵巣組織凍結保存の有効性を調査するために、オランダで観察介入研究が行われています。患者の卵巣皮質組織の1つの断片は、卵巣細胞のX染色体含量を同定するために利用可能であった16、17。研究の一環として、解離した卵巣皮質組織のFISHに基づいて、リンパ球や頬細胞などの非卵巣体細胞に染色体異常がある女性の卵巣細胞に染色体異常が存在するかどうかを判断する新しい方法が開発されました。さらに、卵胞形成に対する卵巣細胞の異数性の影響も決定されました。この目的のために、確立されたFISHプロトコルが変更され、免疫不全マウスの長期異種移植中に人為的に誘導された卵胞形成後の卵巣皮質組織の組織学的切片の分析が可能になりました。本研究では、X染色体異常を有する雌の非移植および移植卵巣皮質組織の卵巣細胞におけるX染色体含量を決定するためのFISHに基づく2つの方法を提示し、このトピックに関する基礎科学の向上を目指します。

X染色体異常を有する雌の卵巣細胞のFISH分析は、この特定のグループの卵巣細胞のX染色体含有量に関する洞察を得るためのユニークな技術です。これらの技術は、X染色体異常のある女性の生殖能力についてさらに学ぶのに役立つ可能性があります。どちらの方法も将来の研究を容易にすることを目的としており、臨床診療における診断ツールとして使用するようには設計されていないことに言及することが重要です。手順を実演するのは、私たちの不妊治療研究所の上級研究員であるロナルドピークと、私たちの病理学部門のアナリストであるミラドインテザールです。まず、凍結保存または解凍した卵巣皮質組織をメスを使用して1つずつ1ミリメートルずつ細かく切ります。組織片を4リットルの予熱したL15で摂氏37度で75分間酵素的に消化します。消化混合物を15分ごとに上下にピペットで入れます。10%ウシ胎児血清(FBS)を添加した4ミリリットルの冷たいL15を加えて、酵素消化を停止します。解離した組織を500 gの遠心分離により8ミリリットルの冷たいL15培地で1回洗浄してから、500マイクロリットルのL15培地に再懸濁します。主に個々の間質細胞および小卵胞を含む細胞懸濁液をプラスチック製のシャーレに移し、実体顕微鏡下で懸濁液を調べる。75マイクロメートルのプラスチックピペットを使用して手動で50マイクロメートル未満の卵胞をピックし、摂氏4度で10%FBSを添加したL15培地の液滴に移します。次に、精製した卵胞を0.06%トリプシン、1ミリリットルEDTAあたり1ミリグラム、および1ミリリットルグルコースあたり1ミリグラムの溶液に移し、摂氏37度で20分間インキュベートします。インキュベーション後、75マイクロメートルのプラスチックピペットを使用して皮質細胞懸濁液から卵巣間質細胞を得る。蛍光In situハイブリダイゼーション、または個々の卵巣細胞のFISH分析の場合、トリプシン/EDTA/グルコースまたは間質細胞で処理した卵巣卵胞を、5マイクロリットルの0.15ミリモル塩化カリウムと15マイクロリットルのDPBSの液滴にスライド上に移し、摂氏37度で20分間インキュベートします。スライドを300マイクロリットルの0.05ミリモル塩化カリウム、7.5%酢酸、および22.5%メタノールで2分間乾燥させ、事前に固定します。スライドを3対1のメタノール酢酸で2分間覆い、固定を確定します。固定後、調製したてのSSCでサンプルを摂氏73度で2回洗浄します。100マイクロリットルの2%プロテイナーゼKで覆い、カバースリップで密封します。ハイブリダイゼーションステーションで摂氏37度で10分間スライドをインキュベートします。インキュベーション後、カバースリップを取り外し、振とうしながら室温でDPBSでスライドを5分間洗浄します。サンプルを1%ホルムアルデヒドで5分間固定します。次に、スライドをDPBSで洗浄し、エタノール濃度を上げながらそれぞれ2分間連続脱水します。サンプルを風乾してから、蛍光標識プローブとハイブリダイズします。次に、1マイクロリットルのCEPX、1マイクロリットルのCEP18、および18マイクロリットルのハイブリダイゼーションバッファーをサンプルに加え、スライドに接着されたカバースリップで密封します。スライドをハイブリダイゼーションステーションに移して摂氏73度で3分間変性させた後、摂氏37度で一晩インキュベーションしながらハイブリダイゼーションを行います。ハイブリダイゼーション後、カバースリップと残っている接着剤をスライドから取り除きます。スライドを0.4X SSC、0.3%トゥイーン-20、摂氏72度で2分間洗浄した後、2X SSCおよび0.1%トゥイーン-20で1分間インキュベートします。示されているようにスライドを脱水し、暗所で風乾してから、DAPIを含む封入剤で覆います。スライドをマイナス20°Cで10分間置いてから、蛍光顕微鏡で分析します。パラフィン切片をハイブリダイズするには、パラフィンリボンから卵胞を含む切片の前または後に新しい切片を選択します。スライドガラスに1つのセクションを取り付け、摂氏56度のストーブで45分間乾燥させます。切片をキシレンで10分間脱パラフィンし、次にスライドを99.5%エタノールに浸し、水道水で5分間すすぎます。スライドをターゲット検索溶液で摂氏96度で10分間前処理します。冷却後、スライドを蒸留水ですすいでください。スライドを0.01モルの塩酸で5分間処理した後、摂氏37度で15分間ペプシン消化します。スライドを塩酸で再度すすぎ、続いてPBSですすいでください。スライドを1%ホルムアルデヒドPBSで5分間固定します。次に、スライドをPBSで簡単にすすぎ、再び脱塩水で洗い流してから、99.5%エタノールで脱水します。前処理されたスライドに5マイクロリットルのプローブCEP18とCEPXを適用します。次に、カバースリップを適用し、写真のりで領域をシールします。スライドをハイブリダイザーに入れて、摂氏80度で10分間変性し、摂氏37度で一晩ハイブリダイゼーションを行います。翌日、スライドを摂氏42度のSSCでそれぞれ5分間2回すすぎ、続いて摂氏73度の0.3%ノニデットP-40で2分と1分間すすぎます。再度、スライドを2X SSCですすぎ、蒸留水ですすぎ、99.5%エタノールで脱水します。スライドを風乾し、DAPI含有封入剤に取り付けます。患者Aの凍結保存された卵巣皮質組織は、リンパ球の50%が45、X核型、50%が46、患者B XX.In リンパ球の38%が45、X、28%が46、XX、34%が47、XXであった。周囲の間質細胞もX染色体含量について分析した。卵巣皮質組織の酵素消化は、大部分が解離した細胞の懸濁液をもたらしたが、顆粒細胞の単層に囲まれた無傷の卵母細胞からなる原始卵胞を残した。小さな卵胞は、顆粒症細胞の凝集のために、FISH後のシグナルの解釈を困難にしました。FISHの前にトリプシンで卵胞をさらに消化すると、個々の顆粒症細胞が卵胞の表面で球形の形態に収縮しました。ヘマトキシリンエオジン染色は、異種移植の5ヶ月後に卵巣皮質組織において形態学的に正常な二次卵胞および胞状卵胞を示した。これらの卵胞の顆粒化細胞のX染色体含量をFISH分析により測定した。移植された卵巣皮質組織をブアン溶液に固定すると、緑色のもやのために濾胞細胞の蛍光シグナルがぼやけ、ほとんど不明瞭になりました。対照的に、ホルムアルデヒドで固定された卵巣皮質組織は優れた蛍光シグナルを提供しました。このプロトコルの主な課題は、卵巣細胞の単離と組織の酵素消化にあります。このプロトコルから逸脱すると、プロセス中に卵巣細胞が大幅に失われる可能性があり、これはすでに卵巣予備能が低い女性では特に避けるべきです。追加の遺伝子発現プロファイリングは、卵胞形成に対する異数性卵巣細胞の影響、ひいてはX染色体異常を有する女性における早期卵巣不全のプロセスについてさらに学ぶのに役立つ可能性がある。
