대장암은 전 세계적으로 암 관련 사망의 세 번째로 흔한 원인이며1, 대장암으로 인한 혈액 전이의 가장 흔한 부위는 간입니다. 이러한 전이는 대장암 환자의 최대 50%에서 발생하며 대장암 환자의 주요 사망 원인입니다2. 간 전이가 없는 대장암 환자의 경우 외과적 절제와 수술 후 보조 화학요법, 중재적 기법으로 생존을 연장할 수 있습니다. 절제 가능한 간 전이의 경우 직경이 3cm 미만인 경우 외과적 국소 절제술, 방사선 중재술, 냉동 요법, 고주파 절제술, 마이크로파 절제술로 환자의 생존율을 높일 수 있다3. 절제 불가능한 대장암 간 전이의 경우, 기존의 화학요법, 중재적 요법 및 기타 치료 전략은 대다수 환자의 생존 혜택이 제한적입니다.
수술은 대장암으로 인한 간 전이의 황금 표준이며 5년 생존율은 40%입니다. 대장 간 전이 환자의 20%-30%만이 수술적 치료의 혜택을 받을 수 있으며, 절제 불가능한 대장 간 전이가 있는 대부분의 환자는 전통적인 보존적 치료의 혜택이 제한적이다4. 대장암 간 전이의 치료에서 중요한 방법은 마이크로파 절제술과 고주파 절제를 포함한 열 절제술입니다. 이 두 가지 기술은 국소 온열요법으로 인한 응고 괴사를 통해 세포 사멸을 유도합니다. 열 절제술의 주요 적응증은 다음과 같습니다: (i) 절제 불가능한 간 병변; (ii) 간절제술과의 병용; (iii) 중증 동반 질환 또는 수행 상태가 좋지 않은 환자(PS); (iv) 분절 절제술이 필요한 작은(<3cm) 단독 병변; (v) 환자 선호도5. 그 중 마이크로파 절제술(MWA)은 환자의 생존을 연장할 수 있는 안전하고 효과적인 치료법입니다. 그것은 광범위한 활성 가열 영역을 가지고 있으며 종양 조직의 전기 전도에 의존하지 않습니다. 이 에너지 전달은 조직 타열에 의해 제한되지 않습니다. 고주파 절제술에 비해 마이크로파 절제술은 종양 조직의 온도가 높고, 치료 시간이 짧으며,치료 범위가 더 넓다6.

MR이나 CT와 같은 수술 전 영상에서 발견하기 어려운 병변을 발견할 수 있습니다.
