이 프로토콜의 목표는 병상 제공자가 현장 진료 초음파를 사용하여 비침습적 방식으로 두개내 고혈압 환자를 신속하게 평가할 수 있도록 하는 것입니다. 최근 몇 년 동안 병상 옆 의사 결정 및 치료는 현장 진료 초음파(POCUS)의 출현으로 강화되었습니다. POCUS는 환자의 주치의가 진단 또는 절차적 지도를 위해 초음파를 사용하는 것을 포함합니다. 이 기사는 시신경 초의 진단 POCUS에 초점을 맞춥니다.

그러나, 임상적 중요성에도 불구하고, ONSD POCUS를 사용하는 의사의 숙련도는 변수 2,3이며, 이는 이 양식의 적절한 사용을 제한한다4. 이 연구는 진단 ONSD POCUS를 위한 시간 효율적이면서도 철저한 이미지 획득 프로토콜을 설명하고 임상 실습에서 일반적으로 발견되는 비정상적인 소견을 설명하는 것을 목표로 합니다. 문헌에는 여러 이미징 프로토콜이 설명되어 있으며, 이는 구조적 해석, 초음파 설정, 마커 배치 및 스캔 기술에 변형을 나타낸다5. 마커 포인트에 따라 ICP의 변화에 대한 민감도가 다르며, 배치는 ICP가 정상인 환자와 높은 ICP를 가진 환자를 구별하는 능력에 영향을 미칩니다. 이러한 이유로 본 논문에서는 시신경초(optic nerve sheath) 이미지를 획득하고 일관되게 측정하기 위한 표준 기법을 간략하게 설명합니다.
측정 위치 및 기술의 차이로 인해 비정상적인 시신경 초(5,6)로 간주되는 것의 임계값이 크게 달라졌다. 최근의 메타 분석에서, 두개내 고혈압이 없는 환자의 평균 ONSD는 4.1mm였고, ICP가 증가한 환자의 평균 ONSD는 5.6mm였다. 확장된 ONSD에 대해 일반적으로 허용되는 임계값은 5.5mm이지만, 기준선을 사용할 수 있는 경우 정상 기준선에서 ONSD의 변화가 훨씬 더 예측적입니다. 중증 TBI의 경우, 양쪽 눈의 ONSD가 함께 변하는 것으로 보인다7. 한쪽에서 ONSD가 증가하는 개별 사례가 있지만 이는 드문 경우8.
중증 외상성 뇌 손상(TBI) 인구에서 ONSD를 측정하는 데에는 추가적인 문제가 있습니다. 중증 TBI(글래스고 혼수 점수 <8로 정의됨)가 있는 환자는 명령을 따를 만큼 충분히 주의력이 없습니다. 이는 신경염에 대한 선택적 방식으로 안과 사무실에서 평가되는 환자와 비교하여 이 집단의 신경을 측정하기 위해 다른 접근 방식이 필요하다는 것을 의미합니다. A-스캔은 오랜 시간 동안 눈을 가만히 둘 수 있는 협조적인 환자의 시신경을 측정하는 데 사용할 수 있지만 TBI 환자에게는 유용한 기술이 아니므로 B-스캔이 표준 기술입니다(아래 2.7단계 참조).
