	<li>Prevent dehydration by wrapping the abdominal organs with warm and wet gauze. Intermittingly spread warm saline with a 50 mL syringe during the surgery.</li>
	<li>Utilizing a surgical microscope with a 10x magnification, mobilize the duodenum and proximal jejunum by blunt dissection with cotton swabs to expose the Abd. A. and IVC. Prepare the Abd. A and IVC for anastomosis and systematically implant the donor heart, in accordance with Figure 3 or previously documented methods15. 
	NOTE: Do not separate the Abd. A. and IVC.

		<li>Assuming vascular anastomosis to be placed infrarenal, prepare a sufficient portion of the aorta and IVC for clamping.</li>
		<li>Perform blunt preparation using cotton swabs or sharp-serrated forceps to remove the fats and fascia around the vessels.</li>
		<li>Place 5-0 silk ligatures to the mesenteric branches and both the cranial and caudal sides of the major vessels. Elevate the abdominal vessels and coagulate or ligate the lumbar branches with 5-0 silk sutures. Remember to spare the testicular arteries and veins and do not clamp them.</li>
		<li>Use ligatures to lift the vessels and position the micro-clamps to the mesenteric branches, caudal, and cranial sides of the major vessels to stop the blood flow at the anastomosis site. Be sure to switch off the heating pad before placing the clamps, as excess heating can exacerbate limb ischemia.</li>
		<li>Puncture the aorta using a 27 G needle and elongate the incision with micro scissors to a length equal to or slightly larger than the opening of the donor ascending aorta (Asc. A), which is approximately 5 mm.</li>
		<li>Make a longitudinal incision on the IVC in the same way as the aortotomy, but make it 3 mm closer to the caudal side compared to the aorta incision.</li>
		<li>Starting the anastomoses, placed the donor heart on the right side of the recipient&#39;s abdomen and attach the donor Asc. A to the recipient&#39;s Abd. A with one simple interrupted stitch (9-0 polypropylene) at the cranial corner of the longitudinal incision.</li>
		<li>Move the heart to the left side of the recipient abdomen and perform anastomosis of the donor&#39;s Asc. A with the recipient&#39;s Abd. A using a running 9-0 polypropylene suture.</li>

		<li>Remove the mesenteric vein clamp first after completing the anastomosis to allow the right side of the heart to fill with venous blood.</li>
		<li>Remove the air in the coronary circuit and Asc. A. by applying retrograde coronary perfusion for several seconds.</li>
		<li>Place a piece of gauze on both sides of the vessels and remove the caudal clamp and the cranial clamp.</li>
		<li>Apply gentle compression with cotton swabs for 1-2 min. After ensuring adequate hemostasis, remove the swabs and wash the anastomoses with warm saline. 
		NOTE: The heart should begin beating within the first minute of reperfusion. If the recipient rat&#39;s body temperature is below 35 &#176;C, the heart rhythm will normalize after the temperature reaches 36 &#176;C.</li>

	NOTE: Do not initiate the postoperative examinations till the body temperature reaches 37&#176;C. Maintain anesthesia at 2-2.5% isoflurane until the end of the experiments.</li>
	<li>Monitor ECG of the transplanted donor heart for 3 h. Then, excise the heart under deep anesthesia for histological studies. 
	NOTE: Confirm anesthesia depth via lack of pedal reflex before excising the heart. The surgical procedure and the ECG monitoring take less than 6 h. Diclofenac, administered perioperatively (step 4.2.3.), enables pain management for the entire duration of this procedure. The analgesia regimen can be adjusted per the institutional animal use guidelines.</li>

Til tross for en jevn økning i antall pasienter med behov for hjertetransplantasjon, er det ikke observert en proporsjonal økning i tilgjengeligheten av donororganer5. For å løse dette problemet har nye tilnærminger for å bevare donorhjerter blitt utviklet med mål om å forbedre utfordringene og øke tilgjengeligheten av givere 6,7,8,9.
Normothermic ex situ heart perfusion (NESHP) ved bruk av organpleiesystem (OCS) maskiner har dukket opp som en klinisk intervensjon 1,3. Denne teknikken har blitt ansett som et egnet alternativ til konvensjonell statisk kjølelager (SCS) metode 2,9. NESHP reduserer effektivt varigheten av kald iskemi, reduserer metabolsk etterspørsel og letter optimal næringstilførsel og oksygenering under transport av donororganer10,11. Til tross for det klare potensialet i denne metoden for å forbedre bevaring av donororganer, har dens kliniske anvendelse og videre undersøkelse blitt begrenset av høye kostnader. Derfor er prekliniske dyremodeller av NESHP avgjørende for å identifisere viktige tekniske utfordringer knyttet til denne teknikken12,13. Griser og rotter er de foretrukne dyremodellene for prekliniske studier på grunn av deres iskemiske toleranse9. Selv om svinemodellen er ideell for grunnleggende og translasjonsforskning, er den begrenset av høye kostnader og intensiv arbeidskraft som kreves for pleie og vedlikehold. Derimot er rottemodeller rimeligere og enklere å håndtere14.
I denne studien introduserer vi en forenklet rottemodell av NESHP, etterfulgt av heterotopisk hjertetransplantasjon, for å evaluere effekten av konserveringsteknikken på transplantattilstand etter implantasjon. Denne modellen er enkel, kostnadseffektiv og kan utføres av en enkelt eksperimentør. Figur 1 viser skjemaene for prosedyren.

Hjertetransplantasjon er den mest effektive behandlingen for hjertesvikt i sluttstadiet. Til tross for forbedringer i terapeutiske tilnærminger og intervensjoner, øker antall hjertesviktpasienter som venter på transplantasjon fortsatt. Den normoterme ex situ preserveringsteknikken er etablert som en sammenlignbar metode med den konvensjonelle statiske kjølelagringsteknikken. Den største fordelen med denne teknikken er at donorhjerter kan bevares i opptil 12 timer i fysiologisk tilstand. Videre tillater denne teknikken gjenoppliving av donorhjertene etter sirkulasjonsdød og bruker nødvendige farmakologiske inngrep for å forbedre donorfunksjonen etter implantasjon. Tallrike dyremodeller har blitt etablert for å forbedre normoterm ex situ bevaringsteknikker og eliminere bevaringsrelaterte komplikasjoner. Selv om store dyremodeller er enkle å håndtere sammenlignet med små dyremodeller, er det kostbart og utfordrende. Vi presenterer en rottemodell med normoterm ex situ donorhjertekonservering etterfulgt av heterotopisk abdominaltransplantasjon. Denne modellen er relativt billig og kan oppnås av en enkelt eksperimentør.

Figur 1 illustrerer det eksperimentelle designet som ble brukt i en smådyrmodell. Figur 2 viser det modifiserte Langendorff perfusjonsapparatet, som inkluderer en oksygenator for små dyr. Rekkefølgen av anastomose ved heterotopisk abdominalimplantasjon er presentert i figur 3.

Vi har utviklet en modell for å redusere komplikasjoner forbundet med normoterm ex situ perfusjon og øke sikker bevaringstid for donorhjerter. Sammenlignet med de alternative dyremodellene er denne modellen enkel og kostnadseffektiv, og kan utføres av en enkelt eksperimentør. Denne modellen kan være nyttig for å teste den langsiktige effekten av farmakologiske og genetiske inngrep på donorhjertene etter transplantasjoner.Den kirurgiske prosedyren krever mikrokirurgisk ferdighet. Nøkkelfaktoren for å oppnå suksess i denne modellen er å øve og fokusere på de kritiske trinnene som er skissert i manuskriptet. For å begynne, monter Langendorff-apparatet, inkludert oksygenatoren, pumpen og perfusjonslinjene.Fyll perfusjonskretsen med 20 ml saltoppløsning, og sirkuler den til den er primet med autologt blod. På den bedøvede donorrotten, gjør en 7-centimeter midtlinje abdominal snitt, og deretter bilaterale snitt som måler 3 centimeter fra xiphoidprosessen til midtkragebenet. Åpne brysthulen fra membranen, og kutt den dårligere vena cava nær membranen for å forhindre ventrikulær distensjon.Skjær ribbeina bilateralt langs brystryggen opp til thoraxinnløpet. Reflekter den mobiliserte ventrale brystveggen overlegent med myggtang. Etter administrering av all kardioplegisk løsning, isoler aortabuen fra det omkringliggende vevet, og dissekere den forsiktig like under venstre subklaviske arterie.Transekter brakiocephalic og forlot vanlige carotisarterier i en fjern stilling, slik at de lengre stubbene i aortabuen for enkel håndtering under aorta kanylering. Deretter transekterer hovedpulsåren så nært bifurkasjonen som mulig. Forsiktig ligere den overlegne vena cava og dårligere vena cava med 5-0 silke suturer, og forhindre hindring av høyre atrium og koronar sinus.Dekk venstre kant av thorax med vått gasbind, legg hjertet på det, og trekk forsiktig de overlegne vena cava og dårligere vena cava ligaturene for å avsløre hilum. Ligate lunge og azygos årer sammen med en 5-0 silke sutur. Kutt vevet dorsalt til ligaturen og trekk ut hjertet.Vei hjertet etter å ha undersøkt det for eventuelle skader. Bytt ut saltvannsprimet krets med blodpriming, og start perfusjonen med en strømningshastighet på to til tre milliliter per minutt, slik at perfusatet kan lekke fra kanyleringsstedet for å fjerne eventuelle luftbobler. Etter å ha plassert den permanente ligaturen, sørg for at hjertet begynner å trekke seg sammen innen få sekunder, og når en normal rytme på 60 sekunder.Et gjennomsnittlig perfusjonstrykk på 55 til 65 millimeter kvikksølv med en koronar strømningshastighet på tre til fire milliliter ved 37 grader Celsius indikerer tilstrekkelig perfusjon. Samle 0,15 ml blod fra reservoaret, og kontroller blodgassanalysen i begynnelsen av perfusjonen og hvert 20. minutt etterpå. Etter 120 minutters perfusjon, administrer tre milliliter Custodiol gjennom sprøytepumpen med en hastighet på 250 milliliter per time for å arrestere hjertet.Utfør mid-line laparotomy og sett inn en retractor for å utvide bukhulen. Mobiliser bukorganene til venstre side av mottakeren ved hjelp av bomullspinner for å gi plass til prosedyren. Ved hjelp av et kirurgisk mikroskop med 10x forstørrelse, mobiliser tolvfingertarmen og proksimal jejunum ved stump disseksjon med bomullspinner.å eksponere abdominal aorta og dårligere vena cava. Plasser 5-0 silke ligaturer på mesenteriske grener av både kraniale og kaudale sider av de store fartøyene. Løft bukkarene og koaguler eller liger lumbale grener med 5-0 silke suturer.Husk å skåne testikkelårene og venene, og ikke klemme dem. Bruk ligaturer til å løfte karene og plassere mikroklemmene til mesenteriske grener, caudale og kraniale sider av de store karene for å stoppe blodstrømmen på anastomosestedet. Punkter aorta ved hjelp av en 27-gauge nål, og forleng snittet med mikrosaks til en lengde som er lik eller litt større enn åpningen av donorens stigende aorta.Lag et langsgående snitt av den dårligere vena cava på samme måte som aortotomien, men gjør den tre millimeter nærmere den kaudale siden sammenlignet med aortasnittet. Start anastomosene, plasser donorhjertet på høyre side av mottakerens mage, og fest donorens stigende aorta til mottakerens abdominale aorta med ett enkelt avbrutt sting i kranialhjørnet av langsgående snitt. Flytt hjertet til venstre side av mottakerens mage, og utfør anastomose av donorens stigende aorta med mottakerens abdominal aorta, ved hjelp av en løpende 9-0 polypropylen sutur.Fikser donor lungearterien til den nedre vena cava med to avbrutte suturer i kaudale og kraniale hjørner av langsgående snitt. Etter ferdigstillelse av anastomosen, fjern kaudale klemmen og kranialklemmen. Forsikre tilstrekkelig hemostase, erstatt bukorganene på en meanderlignende måte, og lukk lagene i buksnittet ved bruk av kontinuerlige 5-0 polypropylensuturer.Parametrene som brukes til å vurdere hjertets levedyktighet under ex situ perfusjon, som laktat, kalium og gjennomsnittlig aortatrykk, er vist i denne figuren. Her reduserte bruken av normoterm ex situ-bevaring den totale iskemiske tiden for seks vellykkede tilfeller til 46,2 pluss-minus 4,7 minutter, mens den totale ut-av-kroppen-tiden var 166,2 pluss-minus 4,7 minutter. Behovet for ekstraksjon av hjertet fra donor og forberedelse til ex situ perfusjon og heterotopisk transplantasjon var 5,8 pluss-minus 1,3 minutter.Den totale suksessraten for operasjonen var 70 % og gjennomsnittlig anastomosetid for de seks vellykkede tilfellene var 38,4 pluss-minus 3,4 minutter. Den elektrofysiologiske ytelsen til donorhjertet før anskaffelse og etter implantasjon er presentert i denne figuren. I alle forsøkene sank hjertefrekvensen betydelig umiddelbart etter implantasjon, men den kom seg til slutt over tid.Det makroskopiske og mikroskopiske utseendet til donorhjertet før normotterm ex situ konservering, etter normotterm ex situ bevaring, og etter to timer med heterotopisk implantasjon er vist her. Når du gjør denne prosedyren, er det viktig å ta nok blod til perfusjon, kutte aorta forsiktig uten å skade den stigende delen, og følg nøye med på de fysiologiske parametrene under konservering. Etter denne prosedyren kan vi teste effektiviteten av nye konserveringsløsninger og mottakerens immunrespons på nylig introduserte konserveringsmetoder.Med kombinasjonen av ex situ perfusjon og heterotopisk transplantasjon er vi i stand til å undersøke nye teknikker gjennom forlenget bevaringstid, og observere den langsiktige effekten på mottakerens immunrespons.
