Halvautomatiserad planimetri möjliggör en snabb, exakt och objektiv kvantifiering av områden täckta med tandplack i kliniska prövningar. Jämfört med traditionell planimetri, där placktäckta områden definieras manuellt av operatören, är halvautomatiserad planimetri mer objektiv och det minskar behandlingstiden kraftigt. Börja med att montera den skräddarsydda distansen på fluorescenskameran, anslut sedan intraoralkameran till en dator och öppna kameraprogramvaran.
Klicka på Patient och sedan på Ny patient för att skapa och fylla i patientinformationen i systemet. Klicka sedan på Patient och Spara för att spara relevanta data. Klicka på Video och den intraorala kameran är nu redo att användas.
Applicera ett rött avslöjande färgämne, som är 5% erytrosin, med en bomullspellet på tandytorna av intresse för att avslöja placket. Instruera patienten att skölja med vatten i 10 sekunder för att avlägsna överflödigt färgämne. Ta bort eventuell tandköttsfläck med en bomullspellet och lufttorka varje tand i tre sekunder.
Placera den intraorala kameran i ett horisontellt läge framför tanden av intresse med distansen som rör vid tandköttet eller de intilliggande tänderna. Se till att hela tandytan av intresse är i fokus och fångad i bilden utan att inkludera antagonist eller kontralaterala tandytor. Hämta fluorescensbilden genom att trycka på kameraknappen.
Utför färgning och bildförvärv för alla tänder av intresse. Markera sedan alla bilder i kameraprogramvaran och klicka på Spara bilder och videor i menyn, se till att bilderna sparas i plackläge och inte i kariesläge. Symbolen P eller C i menyn indikerar aktuellt läge.
För att exportera bilderna, gå till Viewer och välj de bilder som ska exporteras. Klicka på Arkiv och exportera, Spara som och sedan på alla bilder av patienten för att exportera bilderna. I fönstret Exportera väljer du inställningen Lägesstandard och klickar på Exportera sökväg.
Välj önskad mapp och under Bildtypsval, markera den vänstra rutan och välj Bildtillstånd som Originaldata från rullgardinsmenyn. Expandera exportfönstret för att visa fler alternativ och välj Filnamn innehåller för att välja Kortnummer eller Userinput eller Patientnamn. Välj sedan Formatera som TIF och klicka på OK för att exportera bilderna.
Du kan också ställa in en automatisk filexport före avbildning genom att klicka sekventiellt på Alternativ, Visa konfiguration, Moduler, Visningsprogram, Exportera eller E-post, Exportalternativ och sedan välja Autoexportläge. Klicka på Exportera sökväg och välj önskad mapp och välj Bildläge som Originaldata. Välj Filnamn innehåller för att välja Kortnummer eller Userinput eller Patientnamn.
Välj sedan Formatera som TIF och klicka på OK för att ställa in standardexportinställningarna. Den digitala bildanalysen kan utföras när som helst efter bildinsamlingen och upp till 1000 fluorescensbilder kan bearbetas parallellt. Byt namn på alla bilder med sekventiella indexnummer.
Importera fluorescensbildserien i en dedikerad bildanalysprogramvara i rött, grönt, blått läge genom att klicka på Arkiv, Importera bilder och Importera som färg. Utför en tröskelbassegmentering av bildserien genom att klicka på Segment, Automatisk segmentering och sedan Anpassad tröskel. Ställ in det låga tröskelvärdet över intensiteten hos de orala mjukvävnaderna och lämna det höga tröskelvärdet vid 255.
Således känner programvaran bara igen tänderna med rena och placktäckta områden som föremål. Klicka på Använd, OK och Segment för att initiera segmenteringen. Öppna visualiseraren genom att dubbelklicka på namnet på bildserien och ange objektredigeraren.
Utför en visuell kvalitetskontroll av segmenterade områden och ta bort artefakter genom att avvisa och ta bort sådana objekt. Sammanfoga de återstående objekten i alla bilder genom att klicka i alla bilder och sammanfoga markerade objekt. Nu finns det bara ett objekt per bild.
Kvantifiera den totala tandytan i varje bild genom att klicka sekventiellt på Analys, Mät objekt, Rensa alla och sedan Pixlar. Slutligen exportera data. Importera fluorescensbildserien igen till programvaran, den här gången med delade röda, gröna och blå färgkanaler genom att klicka på Arkiv, Importera bilder och Importera som grå.
Stäng de blå kanalbilderna och överför objektlagret från de röda, gröna, blå bilderna till de röda kanalbilderna genom att klicka på Segmentera och överför objektlager. Ta bort pixlar som inte är objekt i de röda kanalbilderna med objektredigeraren i alla bilder och ta bort pixlar som inte är objekt. Mjuka vävnader tas nu bort från bilderna.
Multiplicera den röda kanalbildserien med en faktor 2 genom att sekventiellt klicka på Redigera, Bildkalkylator, Multiplikation, Parametrar och ange en faktor på 2,00. Klicka sedan på Apply och OK för att förbättra kontrasten mellan placktäckta och rena tandområden. Om du vill ta bort rena tandområden från bilderna subtraherar du bildserien med grön kanal från bildserien med den förbättrade röda kanalen genom att klicka på Redigera, Bildkalkylator, Andra operandbilder, planimetri grön, Subtraktion, Använd och OK.To sedan identifiera de placktäckta områdena på tänderna, utföra en tröskelbassegmentering och sammanfoga de återstående objekten som visats tidigare.

Plackavlagringar visualiseras av erytrosin, medan rena tandområden och den förvärvade pellikeln lämnas ofärgade. Bilder som förvärvats med en fluorescenskamera förbättrar avsevärt kontrasten mellan de rena tandområdena, placktäckta områden och omgivande mjuka problem. Jämfört med de rena tandområdena verkar de placktäckta områdena något ljusare i den röda kanalen.
I den gröna kanalen maskeras tandens autofluorescens avsevärt i de placktäckta områdena. Denna maskeringseffekt utnyttjas för att subtrahera de gröna kanalbilderna från den röda kanalen. Den starka kontrasten mellan de rena och placktäckta områdena i de resulterande bilderna möjliggör en intensitetströskelbaserad halvautomatisk bestämning av PPI.
Den beskrivna metoden kan användas för planimetriska inspelningar av supergingivalplack och kalkyl på både ansikts- och muntandytor. Olika tandfärgade material fluorescerar i det gröna spektrumet med varierande intensitet. Därför kan PPI vanligtvis bestämmas med en standard bildanalysalgoritm på tänder med glasjonomercement och kompositrestaureringar.
Däremot avger amalgam- och gjutrestaureringar vanligtvis svagt i både de röda och de gröna kanalerna, och det är därför inte möjligt att bestämma placktäckningen på sådana ytor. Detsamma gäller för metalliska ortodontiska fästen, men eftersom konsolytan vanligtvis utesluts från PPI-inspelningar är halvautomatiserad planimetri lämplig för ortodontiska patienter. Vid halvautomatisk identifiering av placktäckta områden, om för mycket omgivande ljus kommer in är det svårt att skilja mellan tänderna och mjuka vävnader.
Otillräcklig munöppning hindrar den halvautomatiska behandlingen. När planimetri utförs på pre-molarer eller molarer är korrekt kameravinkling avgörande för att undvika att avbilda delar av ocklusalytan. Det är mycket viktigt att torka tänderna och dämpa ljuset i rummet innan du tar en bild.
Se till att inte fånga antagonisttänder eller delar av bettytan.
Denna studie presenterar en halvautomatiserad digital bildanalysprocedur för planimetrisk kvantifiering av avslöjad tandplack baserat på bilder som förvärvats med en intraoral fluorescenskamera. Metoden möjliggör snabb och tillförlitlig kvantifiering av tandplack i forskningsmiljön.
