La enfermedad de Ménière (DM) es una enfermedad incapacitante caracterizada por ataques de vértigo, síntomas auditivos y pérdida de audición1. La hidropesía endolinfática en el oído interno está presente en pacientes con distrofia muscular, pero la etiología exacta de la enfermedad sigue sin estar clara. En la mayoría de los pacientes, los síntomas se resuelven con el tiempo2; A pesar de esto, la mayoría de los pacientes buscan tratamiento activo debido al patrón impredecible de ataques.
El tratamiento de la distrofia muscular tiene como objetivo reducir los ataques de vértigo. En el último siglo se han propuesto diferentes modalidades de tratamiento, tanto quirúrgico como no quirúrgico. Las intervenciones quirúrgicas destructivas, como la laberintectomía o la neurotomía vestibular, son efectivas para controlar el vértigo, pero causan sordera y pérdida de la función vestibular en el oído operado 3,4. Se han estudiado cirugías en el saco endolinfático (SE), como la descompresión y la derivación del saco, pero ninguna de las intervenciones propuestas ha demostrado ser más efectiva que la cirugía placebo4.
En 2015, Saliba et al. publicaron los resultados de un ensayo controlado aleatorizado que comparaba una nueva técnica, la obstrucción del conducto endolinfático (EDB), con la descompresión del saco endolinfático (ESD)5. El ensayo arrojó resultados prometedores, ya que el 96,5% de los pacientes del grupo EDB estaban libres de ataques de vértigo después de 2 años. La razón detrás de esta técnica es que el CE es, al menos parcialmente, responsable de la alteración de la homeostasis de la endolinfa y produce una sobrecarga de endolinfa debido al aumento de la producción. Al bloquear el conducto endolinfático (DE), el exceso de endolinfa que se genera en el saco se ve impedido de fluir hacia el resto del oído interno. Esta hipótesis es apoyada por estudios histológicos 6,7,8.
El hecho de que la EDB sea el tratamiento adecuado para un paciente individual depende de varios factores. La preferencia del paciente y la preferencia del cirujano juegan un papel importante, pero las regulaciones locales de atención médica también pueden influir en la elección del tratamiento. En nuestro centro, la BDE solo se considera en pacientes que sufren ataques de vértigo a pesar del tratamiento con inyecciones intratimpánicas (IT) con corticoides, y si se ha descartado migraña vestibular. EDB está especialmente indicado para pacientes con buena función auditiva que rechazan el tratamiento ablativo. En este artículo se describen los pasos quirúrgicos de esta nueva técnica y se discute la bibliografía disponible en la actualidad.

La obstrucción del conducto endolinfático es una opción de tratamiento relativamente nueva para la enfermedad de Ménière, cuyo objetivo es reducir los ataques de vértigo sin afectar la audición y el equilibrio. Después de una mastoidectomía regular, se identifica el canal semicircular posterior y se determina la línea de Donaldson. Se trata de una línea que atraviesa el canal semicircular horizontal, cruzando el canal semicircular posterior. El saco endolinfático se encuentra generalmente en este sitio debajo del canal semicircular posterior. El hueso del saco endolinfático y la duramadre se adelgazan hasta que el saco se esqueletiza, después de lo cual se identifica el conducto endolinfático. A continuación, el conducto se bloquea con un clip de titanio. Mediante una tomografía computarizada (TC), se confirma la posición. Las visitas de seguimiento se realizan 1 semana, 6 semanas y 1 año después de la cirugía. Hasta el día de hoy, solo se ha realizado un ensayo prospectivo que evalúe este método, comparando este nuevo método con la descompresión del saco endolinfático. Los resultados de la obstrucción del conducto son prometedores, con un 96,5% de los pacientes libres de vértigo después de 2 años. Sin embargo, se requiere más investigación.
