Het is algemeen bekend dat de pathogenese van opgezette buik veroorzaakt door COPD niet volledig wordt begrepen, wat de ontwikkeling van nieuwe therapieën en medicijnen belemmert. In de afgelopen jaren zijn warme acupunctuur en moxibustie op grote schaal toegepast bij COPD in combinatie met opgezette buik. Het therapeutische mechanisme van warme acupunctuur en moxibustie is voornamelijk het effect van acupunctuur, het moxibustie-verwarmende effect en het warmtegeleidende effect van het naaldlichaam31</...

Chronische obstructieve longziekte (COPD) is een veel voorkomende chronische consumptieve ziekte van de luchtwegen. Patiënten met COPD manifesteren zich voornamelijk in chronische hoest en sputum, met de progressieve ontwikkeling van de ziekte en geleidelijke achteruitgang van de longfunctie. De ziekte kan gecompliceerd worden door chronische pulmonale hartziekte, respiratoire insufficiëntie en hartfalen in de latere stadia van de ziekte, wat de kwaliteit van leven van patiënten ernstig aantast. Er wordt gemeld dat veranderingen in de darmmicrobiota geassocieerd zijn met ziekteprogressie bij COPD1. Naast longsymptomen meldde 40,31% van de patiënten met stabiele COPD opgezette buik. Opgezette buik kan een aanzienlijke invloed hebben op het verergeren van longsymptomen, het verhogen van de frequentie van acute exacerbaties van COPD en het aantasten van de kwaliteit van leven van patiënten met stabiele COPD 2,3. De oorzaken van opgezette buik bij COPD worden beschouwd als multifactorieel, waaronder het volgende: (1) dysbiose van bacteriën; (2) ascites, dat de intra-abdominale druk verhoogt; (3) elektrolytstoringen; (4) rechterhartfalen en congestie van de lever of het spijsverteringskanaal; (5) langdurige bedrust4; (6) Medicijnen5 en mechanische beademing6.
Sommige geleerden in de Chinese geneeskunde hebben voorgesteld dat het ontwikkelingsproces van COPD het proces is van vier toestanden: (1) long-qi-deficiëntie; (2) milt qi-deficiëntie; (3) Qi-deficiëntie van de nieren; (4) Yin en yang deficiëntie 7,8. Als de milt niet kan transporteren en transformeren en de maag zwak is en niet in staat is om te ontvangen, kan een opgezette buik optreden, wat op zijn beurt kan leiden tot dunner worden van de spieren en zwakte door het ontbreken van chemische bronnen9. Op dit moment zijn de geneesmiddelen die vaak worden gebruikt bij de klinische behandeling van COPD luchtwegverwijders, glucocorticoïden, antibiotica en andere geneesmiddelen10. Er zijn geen richtlijnen die gestandaardiseerde behandelprotocollen specificeren voor COPD in combinatie met opgezette buik, die vaak symptomatisch wordt behandeld.
Warme acupunctuur en moxibustie in de traditionele Chinese geneeskunde kunnen Qi begeleiden om de meridianen te baggeren, de bloedcirculatie te stimuleren, bloedstasis te verwijderen, tekort aan te vullen om yang op te warmen en kou te verdrijven. Ze kunnen antioxidanten produceren en zo een regulerende rol spelen, die de stressrespons van het immuunsysteem kan bevorderen en de bloedcirculatie kan verbeteren11,12. Warme acupunctuur en moxibustie kunnen worden gebruikt voor de behandeling van cervicale spondylose13, knieartritis 14, dysmenorroe 15, constipatie 16, prikkelbare darm syndroom 17, enzovoort. Experimentele studies hebben aangetoond dat acupunctuur opgezette buik kan verlichten door de hersen-darmaste reguleren 18. Acupunctuur in combinatie met moxibustie vermindert de opgezette buik aanzienlijk19, die kan worden uitgeoefend door de regulatie van plasmaprostaglandine E2- en gastrinespiegels in plasma- en maagslijmvliesweefsels20. Ondertussen zijn warme acupunctuur en moxibustie superieur aan prokinetiek en schijnacupunctuur in termen van het verbeteren van de symptomen van functionele dyspepsie21. Acupunctuur kan het middenrif versterken en vermoeidheid van de ademhalingsspieren verlichten, evenals het vasthouden van kooldioxide en hypoxie verbeteren en ademhalingsfalen voorkomen en verlichten22. Warme acupunctuur en moxibustie verbeteren de longfunctie, klinische symptomen en kwaliteit van leven bij patiënten met stabiele COPD23,24.
Volgens de theorie van de Chinese geneeskunde dat "de long in een inwendige en uitwendige relatie staat met de dikke darm", zijn de verwarmende acupunctuur- en moxibustiebehandeling die in deze studie wordt gebruikt, gericht op het bevorderen van het herstel van de gastro-intestinale functie en het verbeteren van de longfunctie en kwaliteit van leven. Warme acupunctuur en moxibustie zijn zeer effectieve, gemakkelijk uit te voeren en goedkope vormen van behandelingen in de traditionele Chinese geneeskunde. Concluderend is het noodzakelijk om een standaardprotocol voor te stellen over hoe het gebruik van warme acupunctuur en moxibustie op de juiste manier kan worden uitgevoerd om COPD met opgezette buik te verlichten. Dit artikel beschrijft hoe u het gebruik van warme acupunctuur en moxibustiebehandeling op de juiste manier kunt manipuleren om COPD met opgezette buik te verlichten.

De meeste patiënten met COPD hebben een combinatie van opgezette buik, waarvan is aangetoond dat deze een negatieve invloed heeft op de longsymptomen, de frequentie van acute exacerbaties en de kwaliteit van leven bij patiënten met COPD. Warme acupunctuur en moxibustie zijn effectief gebleken bij het verlichten van symptomen bij patiënten met COPD in combinatie met opgezette buik. Warme acupunctuur en moxibustie zijn zeer effectieve, gemakkelijk uit te voeren en goedkope vormen van behandelingen in de traditionele Chinese geneeskunde. De gestandaardiseerde praktijk van warme acupunctuur en moxibustie is erg belangrijk voor de behandeling van COPD in combinatie met opgezette buik. De specifieke stappen omvatten het selecteren van de juiste acupunten voor needling through syndrome differentiatiebehandeling en het selecteren van moxasticks van de juiste lengte voor moxibustie gedurende ongeveer 30 minuten na de De-qi. Het verloop van de behandeling duurt een week. De volgende indicatoren worden specifiek beoordeeld: de score van de COPD Assessment Test (CAT) en de abdominale distensie visuele analoge schaal (VAS). We stellen een gestandaardiseerde manipulatie voor van warme acupunctuur en moxibustie voor de behandeling van chronische obstructieve longziekte, of COPD, gecombineerd met opgezette buik. Deze methode kan helpen bij het beantwoorden van belangrijke vragen op het gebied van COPD met opgezette buik. De handmatige voordelen van warme acupunctuur en moxibustie zijn onder meer de hoge effectiviteit van de methode in gebruik, het gemak van uitvoering en de betaalbaarheid.Deze vormen van behandelingen in de traditionele Chinese geneeskunde kunnen veel patiënten ten goede komen. Ons laboratorium zal zich toeleggen op het bestuderen van het basismechanisme van warme acupunctuur en moxibustie om COPD met opgezette buik te verlichten.
