Vi beskriver en protokol til generering af humane podocytter ved episomal omprogrammering af dermale fibroblaster til menneskeskabte pluripotente stamceller. Disse celler ligner in vivo podocytter meget bedre end udødeliggjorte podocytter med hensyn til podocytfødeprocesser og ekspression af podocytspecifik markør. Podocytter er terminalt differentierede celler.
Derfor prolifererer disse celler ikke længere, og primær podocytanalyse i cellekultur er begrænset. Selvom problemet med begrænset celleantal blev overvundet ved hjælp af betinget udødeliggjorte podocytter, præsenterer disse podocytter en dedifferentieret fænotype og adfærd, hvilket sætter spørgsmålstegn ved deres anvendelse i grundforskning. Ved hjælp af inducerede pluripotente stamceller kan podocytter genereres in vitro i et næsten ubegrænset celleantal med typisk podocytmorfologi, herunder forskellige fødevareprocesser og ekspression af podocytspecifik markør.
Når de patientspecifikke stamceller bruges til at differentiere sig til podocytter, er det muligt at studere cellens sygdomsspecifikke ændringer ex vivo. I tilfælde blev den indledende hudbiopsi taget fra en patient med en genetisk podocytsygdom. Podocytter genereret med vores protokol er personlige syge celler, der bærer patientens mutation.
Således repræsenterer disse celler en forbedret ex vivo-model til undersøgelse af podocytsygdomme og potentielle terapeutiske stoffer i en individualiseret tilgang.
