Vi beskriver ett protokoll för att generera humana podocyter genom episomal omprogrammering av dermala fibroblaster till humaninducerade pluripotenta stamceller. Dessa celler liknar in vivo podocyter mycket bättre än odödliga podocyter när det gäller podocytmatprocesser och uttryck av podocytspecifik markör. Podocyter är terminalt differentierade celler.
Därför prolifererar dessa celler inte längre och primär podocytanalys i cellodling är begränsad. Även om problemet med begränsat cellantal övervanns genom att använda villkorligt odödliga podocyter, presenterar dessa podocyter en dedifferentierad fenotyp och beteende, vilket ifrågasätter deras användning i grundforskning. Med hjälp av inducerade pluripotenta stamceller kan podocyter genereras in vitro i ett nästan obegränsat cellantal med typisk podocytmorfologi inklusive distinkta livsmedelsprocesser och uttryck av podocytspecifik markör.
När man använder de patientspecifika stamcellerna för att differentiera till podocyter är det möjligt att studera cellens sjukdomsspecifika förändringar ex vivo. I fallet togs den initiala hudbiopsin från en patient med en genetisk podocytsjukdom. Podocyter som genereras med vårt protokoll är personliga sjuka celler som bär patientens mutation.
Således representerar dessa celler en förbättrad ex vivo-modell för att studera podocytsjukdomar och potentiella terapeutiska substanser i ett individualiserat tillvägagångssätt.
