Cette méthode présente certains avantages. La cataracte est la cause la plus fréquente de cécité dans le monde, caractérisée par une opacification du cristallin. Le seul moyen de traiter la cataracte est l’intervention chirurgicale en retirant le cristallin opaque, ce qui rétablit une haute qualité visuelle. Cependant, une réduction secondaire de la qualité visuelle, appelée opacification postérieure de la capsule (PCO), se développe chez 20 à 40 % des patients dans les 2 à 5 ans suivant la chirurgie1. Cet article présente une méthode permettant d’éliminer davantage les cellules épithéliales résiduelles du cristallin (LEC) laissées dans le sac capsulaire lors de la chirurgie de la cataracte en faisant pivoter la lentille intraoculaire (LIO) pour prévenir le PCO.
Le PCO est un processus causé par les LEC, qui sont inévitablement laissés dans le sac capsulaire après la chirurgie de la cataracte, puis commencent à proliférer et à migrer2. Au cours de la phacoémulsification, une poche capsulaire est générée par une capsulorhexis curviligne continue dans la capsule antérieure, qui comprend une partie de la capsule antérieure, la capsule équatoriale et la capsule postérieureentière 2,3. Chez la plupart des patients, une LIO est implantée dans le sac capsulaire. Un sac capsulaire transparent, en particulier la capsule postérieure, permet à la lumière de se transmettre dans les yeux, ce qui est nécessaire pour une bonne qualité visuelle postopératoire4. Une partie des LEC sont généralement encore attachés au sac capsulaire. En réaction au traumatisme chirurgical et à une réponse de corps étranger aux LIO, les cellules épithéliales résiduelles commencent à proliférer et occupent d’abord la partie restante de la capsule antérieure, puis toutes les surfaces disponibles, y compris la surface de la LIO et, surtout, la capsule postérieure précédemment acellulaire4. Par la suite, les cellules continuent de se diviser, recouvrant finalement toute la capsule postérieure et affectant l’axe visuel. Les changements suivants, y compris la fibrose et la forme régénérative5, peuvent entraîner une déficience visuelle importante6.
Le PCO qui affecte l’acuité visuelle peut être traité par capsulotomie de la capsule postérieure, généralement par un laser grenat d’yttrium et d’aluminium dopé au néodyme (Nd :YAG) et parfois par une intervention chirurgicale4. Des études récentes rapportent que l’incidence de la capsulotomie Nd :YAG pour le traitement du BCP 3 ans après la chirurgie se situe entre 5 % et 20 %7,8. Cependant, cette procédure peut rompre la morphologie capsulaire postérieure normale et plisser la capsule postérieure, affectant ainsi probablement la position des LIO, ce qui est défavorable au résultat visuel à long terme des LIO, en particulier des LIO multifocales, et des LIO toriques6. Les progrès réalisés dans les procédures chirurgicales, la conception des LIO, l’inhibition pharmacologique de la prolifération des LEC et l’induction de l’apoptose des LEC se sont avérés utiles dans la prévention de la PCO, dont la plupart ciblent les LECs9.
Les LEC sont normalement répartis sur la face interne de la capsule antérieure du cristallin sous la forme d’une seule couche1. Les LEC distribués dans la zone autour du cristallin équatorial sont le site naturel de division, connu sous le nom de zone germinative, tandis que les cellules en division sont également observées sur la capsule antérieure10,11. Il a également été démontré que les cellules équatoriales peuvent proliférer et migrer dans la capsule postérieure12. Les LEC résiduels dans le sac capsulaire sont responsables du PCO. Si les LEC dans la zone germinative sont éliminés autant que possible pendant la chirurgie de la cataracte, la possibilité de PCO survenant après l’opération diminue en conséquence. Pour autant que l’on sache, la phacoémulsification de routine n’inclut pas de procédure d’élimination des LEC équatoriaux. Dans une étude menée en Inde, l’auteur a proposé que la rotation de la LIO par un crochet de Sinskey13 dans la poche capsulaire diminue le taux de capsulotomie PCO et Nd :YAG.
Ici, nous avons introduit une méthode consistant à faire tourner la LIO à l’aide d’un embout d’irrigation/aspiration (I/A) dans le sac capsulaire pour prévenir le PCO dans les chirurgies de la cataracte. La raison d’être de cette méthode repose sur le contact mécanique entre la LIO et le sac capsulaire, en particulier la zone équatoriale, pour éliminer les LEC résiduels. Par rapport au traitement du PCO par capsulotomie Nd :YAG, la prévention du PCO maintient l’intégrité de la capsule postérieure et la position correcte des LIO. De plus, cette méthode est rentable et ne nécessite aucun outil supplémentaire, ce qui s’applique à la phacoémulsification de la cataracte et à l’implantation de LIO. Contrairement au polissage antérieur de la capsule, qui est effectué à l’aide d’une pointe I/A dans le mode de polissage du système phaco 6,14, la rotation de la LIO est effectuée après l’implantation de la LIO et est censée éliminer davantage la matière visible du cristallin (cortex) et les cellules.

L’opacification de la capsule postérieure (PCO) est une complication postopératoire courante de la chirurgie extracapsulaire de la cataracte, qui est causée par la prolifération et la migration des cellules épithéliales du cristallin et peut affecter considérablement les résultats visuels à long terme. Le traitement le plus efficace pour le PCO est la capsulotomie au laser à l’yttrium et au grenat d’aluminium dopé au néodyme (Nd :YAG) ; cependant, ce traitement est associé à une complication du segment postérieur et peut rompre la stabilité du sac capsulaire, affectant la position et la fonction des lentilles intraoculaires (LIO) trifocales ou toriques. Les progrès réalisés dans les procédures chirurgicales, la conception de LIO et la pharmacie ont réduit le taux de PCO au cours des dernières années, en se concentrant sur l’inhibition des cellules épithéliales prolifératives du cristallin (LEC). Ce protocole visait à éliminer plus complètement les LEC lors de la phacoémulsification et de l’implantation de LIO. Les premières étapes, y compris l’incision cornéenne claire, la capsulorhexis circulaire continue, l’hydrodissection, l’hydrodélimitation et la phacoémulsification, ont été réalisées comme des procédures conventionnelles. Après avoir placé la LIO dans le sac capsulaire, une rotation de la LIO d’au moins 360° a été effectuée à l’aide d’une pointe d’irrigation/aspiration ou d’un crochet, avec une légère contrainte sur la capsule postérieure. Des résidus sont apparus dans le sac capsulaire d’origine transparent après la rotation des LIO. Ensuite, ces matériaux et le viscoélastique ont été complètement nettoyés à l’aide d’un système d’irrigation/aspiration. Une capsule postérieure transparente a été observée après l’intervention chez les patients subissant cette méthode. Cette méthode de rotation des LIO est un moyen simple, efficace et sûr de prévenir le PCO en éliminant les LEC résiduels et peut être réalisée sans outils ni compétences supplémentaires.

Cataracts are the most common cause of blindness worldwide, characterized by a clouding of the lens. The only means of treating cataracts is surgical intervention by removing the opaque lens, which restores high visual quality. However, a secondary reduction of visual quality, termed posterior capsule opacification (PCO), develops in 20%-40% of patients within 2 to 5 years after surgery1. This article introduces a method to further remove residual lens epithelial cells (LECs) left in the capsular bag in cataract surgery by rotating the intraocular lens (IOL) to prevent PCO.
PCO is a process caused by LECs, which are inevitably left in the capsular bag following cataract surgery and then begin to proliferate and migrate2. During phacoemulsification, a capsular bag is generated by continuous curvilinear capsulorhexis in the anterior capsule, which comprises a part of the anterior capsule, the equatorial capsule, and the entire posterior capsule2,3. In most patients, an IOL is implanted into the capsular bag. A transparent capsular bag, especially the posterior capsule, permits the light to transmit into the eyes, which is necessary for good postoperative visual quality4. A proportion of LECs are usually still attached to the capsular bag. As a reaction to the surgical trauma and a foreign body response toward IOLs, the residual epithelial cells start to proliferate and occupy first the remaining part of the anterior capsule, and then all available surfaces, including the surface of IOL and, most importantly, the previously acellular posterior capsule4. Subsequently, cells continue to divide, ultimately covering the entire posterior capsule and affecting the visual axis. The following changes, including fibrosis and regenerative form5, can cause significant visual impairment6. 
PCO that affects visual acuity can be treated with capsulotomy of the posterior capsule, usually by a neodymium-doped yttrium aluminum garnet (Nd:YAG) laser and sometimes a surgical procedure4. Recent studies report that the incidence of Nd:YAG capsulotomy for treating PCO 3 years after surgery is between 5% and 20%7,8. However, this procedure can break the normal posterior capsular morphology and wrinkle the posterior capsule, thus likely affecting the position of IOLs, which is unfavorable to the long-term visual outcome of IOLs, especially multifocal IOLs, and toric IOLs6. Advances in surgical procedures, IOL design, the pharmacological inhibition of LEC proliferation, and the induction of LEC apoptosis have been confirmed useful in preventing PCO, most of which target the LECs9. 
LECs are normally distributed over the inner side of the anterior lens capsule in single-layer form1. LECs distributed in the area around the equatorial lens are the natural site of division, which is known as the germinative zone, while the dividing cells are also observed on the anterior capsule10,11. It has also been shown that equatorial cells can proliferate and migrate in the posterior capsule12. Residual LECs in the capsular bag are responsible for PCO. If LECs in the germinative zone are cleared as much as possible during cataract surgery, the possibility of PCO occurring postoperatively decreases as a consequence. As far as it is known, routine phacoemulsification does not include a procedure to remove equatorial LECs. In a study in India, the author proposed that rotation of the IOL by a Sinskey hook13 in the capsular bag decreases PCO and Nd:YAG capsulotomy rate.
Here, we introduced a method by rotating the IOL using an irrigation/aspiration (I/A) tip in the capsular bag to prevent PCO in cataract surgeries. The rationale of this method relies on the mechanical contact between the IOL and the capsular bag, especially the equatorial area, to remove residual LECs. Compared with treating PCO using Nd:YAG capsulotomy, the prevention of PCO maintains the integrity of the posterior capsule and the correct position of IOLs. Additionally, this method is cost-effective and requires no extra tools, which applies to cataract phacoemulsification and IOL implantation. Different from anterior capsule polishing, which is conducted using an I/A tip in the polishing mode of the phaco system6,14, the rotation of the IOL is conducted after the IOL implantation and is supposed to further remove visible lens matter (cortex) and cells.

Author Spotlight: Enhancing Visual Outcomes in Cataract Surgery: A Novel Technique to Prevent Posterior Capsular Opacification Through IOL Rotation

This research describe a novel technique to enhance the removal of residual lens epithelial cells that may remain in the capsular bag. Foreign cataract surgery by rotating the intraocular lens to prevent the posterior capsular opacification. The primary treatment for PCO is capsulotome.However, this procedure can cause changes to the shape of the posterior capsule, which may impact the placement of IOLs. This could potentially lead to suboptimal, long-term visual outcomes for patients with multifocal or toric IOLs. We have developed a technique for preventing PCO in cataract surgery by rotating the IOL inside the capsular bag, using Envision and aspiration tip.This method for preventing PCO is simple and cost effective. It only requires one additional step during cataract surgery, reducing the likelihood of PCO and the need for future treatments. We plan to perform extensive research on various IOL designs and PCO with a significant number of participants included in both rotating and non-rotating designs.Additionally, we'll conduct in vitro experiments on animal eyes to determine the effects of IOL rotation on equatorial cells.
