La fibroélastose endocardique (EFE), définie par l’accumulation de collagène et de fibres élastiques dans le tissu sous-endocardique, se présente sous la forme d’un endocarde épaissi nacré ou opaque ; La croissance de l’EFE est la plus active pendant la période fœtale et la petite enfance1. Dans une étude d’autopsie, 70 % des cas de syndrome hypoplasique du cœur gauche (HLHS) étaient associés à la présence d’EFE2.
Les cellules exprimant des marqueurs pour les fibroblastes constituent la principale population cellulaire de l’EFE, mais ces cellules expriment également de manière concomitante des marqueurs endothéliaux endocardiques, ce qui est une indication de l’origine de ces cellules EFE. Notre groupe a précédemment établi que le mécanisme sous-jacent de la formation d’EFE implique un changement phénotypique des cellules endothéliales endocardiques en fibroblastes par la transition endothéliale-mésenchymateuse (EndMT)3. EndMT peut être détecté à l’aide d’une double coloration immunohistochimique pour les marqueurs endothéliaux tels que le cluster de différenciation (CD) 31 ou l’endothélium vasculaire (VE)-cadhérine (CD144) et les marqueurs fibroblastiques (par exemple, l’actine du muscle lisse alpha, α-SMA). De plus, nous avons également précédemment établi le rôle régulateur de la voie TGF-ß dans ce processus avec l’activation des facteurs de transcription SLUG, SNAIL et TWIST3.
EndMT est un processus physiologique qui se produit au cours du développement cardiaque embryonnaire et conduit à la formation des septa et des valves à partir des coussinets endocardiques4, mais il provoque également une fibrose d’organe dans l’insuffisance cardiaque, la fibrose rénale ou le cancer et joue un rôle clé dans l’athérosclérose vasculaire 5,6,7,8. EndMT dans la fibrose cardiaque est principalement régulée par la voie TGF-β, comme nous l’avons rapportéavec d’autres 3,9. Divers stimuli ont été décrits pour induire EndMT : inflammation 10, hypoxie 11, altérations mécaniques 12 et troubles de l’écoulement, y compris des altérations du flux sanguin intracavitaire 13, et EndMT peut également être une conséquence d’une maladie génétique 14.
Ce modèle animal a été développé en utilisant les composants clés du développement de l’EFE cardiaque, qui sont l’immaturité et les altérations du flux sanguin intracavitaire, en particulier la stagnation du flux. L’immaturité a été comblée par l’utilisation de cœurs de rats nouveau-nés comme donneurs, car les rats nouveau-nés sont connus pour être immatures sur le plan du développement immédiatement après la naissance. La transplantation cardiaque hétérotopique offrait la possibilité de restreindre le débit intracavitaire15.
D’un point de vue clinique, ce modèle animal permet de mieux étudier l’impact d’EndMT sur le ventricule gauche (VG) en croissance. La restriction de croissance imposée au cœur fœtal et néonatal par la formation d’EFE induite par EndMT16 empêche les patients présentant des obstructions des voies d’écoulement ventriculaire gauche (LVOTO) telles que la sténose aortique critique congénitale et le syndrome hypoplasique du cœur gauche (HLHS) de la réparation chirurgicale biventriculaire anatomique curative17. Ce modèle animal facilite l’étude des mécanismes cellulaires et de la régulation de la formation tissulaire grâce à EndMT et permet de tester des options de traitement pharmacologique 3,18.
L’échocardiographie transabdominale est utilisée pour surveiller la viabilité du greffon, la contractilité et la perméabilité des anastomoses. Après l’euthanasie, la formation d’EFE peut être observée macroscopiquement dans les 3 jours suivant la transplantation. Le tissu EFE présente les mêmes caractéristiques histopathologiques que le tissu EFE humain de patients atteints de LVOTO.
Par conséquent, ce modèle animal, bien que développé pour une utilisation pédiatrique dans le spectre de HLHS, peut être appliqué lors de l’étude de diverses maladies basées sur le mécanisme moléculaire d’EndMT.

La fibroélastose endocardique (EFE), définie par l’accumulation de tissu sous-endocardique, a des impacts majeurs sur le développement du ventricule gauche (VG) et empêche les patients atteints de sténose aortique critique congénitale et de syndrome hypoplasique du cœur gauche (HLHS) d’une réparation chirurgicale anatomique biventriculaire curative. La résection chirurgicale est actuellement la seule option thérapeutique disponible, mais l’EFE récidive souvent, parfois avec un schéma de croissance encore plus infiltrant dans le myocarde adjacent.
Afin de mieux comprendre les mécanismes sous-jacents de l’EFE et d’explorer des stratégies thérapeutiques, un modèle animal adapté aux essais précliniques a été développé. Le modèle animal tient compte du fait que l’EFE est une maladie du cœur immature et qu’elle est associée à des troubles de l’écoulement, comme le confirment les observations cliniques. Ainsi, la transplantation cardiaque hétérotopique de cœurs de donneurs de rats nouveau-nés est à la base de ce modèle.
Un cœur de rat nouveau-né est transplanté dans l’abdomen d’un rat adolescent et relié à l’aorte infrarouge et à la veine cave inférieure du receveur. Alors que la perfusion des artères coronaires préserve la viabilité du cœur du donneur, la stagnation du flux dans le VG induit la croissance de l’EFE dans le cœur très immature. Le mécanisme sous-jacent de la formation d’EFE est la transition des cellules endothéliales endocardiques vers les cellules mésenchymateuses (EndMT), qui est un mécanisme bien décrit du développement embryonnaire précoce des valves et des septa, mais aussi la principale cause de fibrose dans l’insuffisance cardiaque. La formation d’EFE peut être observée macroscopiquement dans les jours qui suivent la transplantation. L’échocardiographie transabdominale est utilisée pour surveiller la viabilité du greffon, la contractilité et la perméabilité des anastomoses. Après l’euthanasie, le tissu EFE est prélevé et il présente les mêmes caractéristiques histopathologiques que le tissu EFE humain des patients HLHS.
Ce modèle in vivo permet d’étudier les mécanismes de développement de l’EFE dans le cœur et de tester des options de traitement pour prévenir cette formation de tissu pathologique et offre la possibilité d’un examen plus généralisé de la fibrose induite par EndMT.

In an autopsy study, 70% of cases with hypoplastic left heart syndrome (HLHS) were associated with the presence of EFE2.
Cells expressing markers for fibroblasts are the main cell population in EFE, but these cells also concomitantly express endocardial endothelial markers, which is an indication of the origin of these EFE cells. Our group previously established that the underlying mechanism of EFE formation involves a phenotypical change of endocardial endothelial cells to fibroblasts through endothelial-to-mesenchymal transition (EndMT)3. EndMT can be detected using immunohistochemical double-staining for endothelial markers such as cluster of differentiation (CD) 31 or vascular endothelial (VE)-cadherin (CD144) and fibroblast markers (e.g., alpha-smooth muscle actin, &#945;-SMA). Furthermore, we also previously established the regulatory role of the TGF-&#223; pathway in this process with activation of the transcription factors SLUG, SNAIL, and TWIST3.
EndMT is a physiological process that occurs during embryonic cardiac development and leads to the formation of the septa and valves from endocardial cushions4, but it also causes organ fibrosis in heart failure, kidney fibrosis, or cancer and plays a key role in vascular atherosclerosis5,6,7,8. EndMT in cardiac fibrosis is mainly regulated through the TGF-&#946; pathway, as we and others have reported3,9. Various stimuli have been described to induce EndMT: inflammation10, hypoxia11, mechanical alterations12, and flow disturbances, including alterations of the intracavitary blood flow13, and EndMT may also be a consequence of a genetic disease14.
This animal model was developed using the key components of cardiac EFE development, which are immaturity and alterations of the intracavitary blood flow, specifically flow stagnation. Immaturity was fulfilled by using neonatal rat hearts as donors, since neonatal rats are known to be developmentally immature immediately after birth. Heterotopic heart transplantation offered the provision of intracavitary flow restriction15.
From a clinical point of view, this animal model allows for better investigating the impact of EndMT on the growing left ventricle (LV). The growth restriction imposed on the fetal and neonatal heart through EndMT-induced EFE formation16 precludes patients with left ventricular outflow tract obstructions (LVOTO) such as congenital critical aortic stenosis and hypoplastic left heart syndrome (HLHS) from curative anatomical biventricular surgical repair17. This animal model facilitates the study of the cellular mechanisms and regulation of tissue formation through EndMT and allows for the testing of pharmacological treatment options3,18.
Transabdominal echocardiography is used to monitor the graft viability, contractility, and the patency of the anastomoses. Following euthanasia, EFE formation can be macroscopically observed within 3 days after transplantation. EFE tissue shows the same histopathological characteristics as human EFE tissue from patients with LVOTO.
Hence, this animal model, though developed for pediatric use in the spectrum of HLHS, can be applied when studying various diseases based on the molecular mechanism of EndMT.

Endocardial fibro elastosis hinders left ventricular growth and prevents biventricular surgical repair for patients with congenital critical aortic stenosis and hypoplastic left heart syndrome. While surgical resection is currently the only available treatment option, EFE frequently recurs. Therefore, we aimed to investigate the mechanisms behind EFE and develop new therapeutic strategies using an animal model.The main developments in the field of EndMT research are restricted to cell culture models which have inherent limitations, and research has been hampered due to a lack of in vivo models. Our group previously established that the underlying mechanism of EFE formation involves a phenotypical change of endocardial endothelial cells to fibroblasts, a process called endothelial to mesenchymal transition. This animal model allows for a more generalized examination of EndMT induced fibrosis in the heart, whereas other models are designed to induce EndMT through genetic modifications, hypertension, or dietary restrictions.As EFE often recurs after primary resection, sometimes with an even more infiltrative growth pattern, we are especially interested in the molecular interaction with the underlying myocardium.
