Het huidige werk illustreert een gemakkelijk te beheersen techniek van CSF en bloedafname bij ratten, die niet alleen nuttig kan zijn voor studies in modellen van epilepsie, maar ook van andere neurologische aandoeningen of ziekten zoals Alzheimer, Parkinson of multiple sclerose. Cerebrospinale vloeistof (CSF) en bloedafname zijn belangrijk om biomarkers van epilepsie te identificeren en te valideren, zowel in preklinisch als klinisch onderzoek 1,2. Tegenwoordig richten de diagnose van epilepsie en het meeste onderzoek naar epilepsiebiomarkers zich op EEG en neuroimaging 3,4,5. Deze benaderingen brengen echter een aantal beperkingen met zich mee. Afgezien van routinematige hoofdhuidmetingen, vereist EEG in veel gevallen invasieve technieken zoals diepte-elektroden6. Beeldvormingsmethoden van de hersenen hebben een slechte temporele en ruimtelijke resolutie en zijn relatief duur en tijdrovend 7,8. Om deze reden zou de identificatie van niet-invasieve, goedkope en op biovloeistoffen gebaseerde biomarkers een zeer aantrekkelijk alternatief bieden. Bovendien kunnen deze biofluïde biomarkers worden gecombineerd met beschikbare diagnostische benaderingen om hun voorspellend vermogen aan te scherpen.
Patiënten met de diagnose epilepsie worden routinematig onderworpen aan EEG 9,10 en bloedafname 11,12,13,14, en velen ook aan CSF-ontwenning om levensbedreigende oorzaken (d.w.z. acute infecties, auto-immuunencefalitis) uit te sluiten15. Deze bloed- en CSF-monsters kunnen worden gebruikt in klinisch onderzoek dat gericht is op het identificeren van biomarkers voor epilepsie. Hogg en collega's hebben bijvoorbeeld ontdekt dat een toename van drie plasma-tRNA-fragmenten voorafgaat aan het optreden van aanvallen bij menselijke epilepsie14. Evenzo kunnen interleukine-1bèta (IL-1β)-spiegels in menselijke CSF en serum, uitgedrukt als verhouding van IL-1β-niveaus in CSF ten opzichte van serum, de ontwikkeling van posttraumatische epilepsie na traumatisch hersenletsel voorspellen16. Deze studies benadrukken het belang van het bemonsteren van biovloeistoffen voor onderzoek naar epilepsiebiomarkers, maar ze worden geconfronteerd met meerdere beperkingen die inherent zijn aan klinische onderzoeken, bijvoorbeeld de medestichtende factor van anti-epileptica (AED's) in bloed, het frequente gebrek aan etiologische informatie, ontoereikende controles, bescheiden aantallen patiënten en andere17,18.
Preklinisch onderzoek biedt andere mogelijkheden om moleculen in biovloeistoffen te onderzoeken als potentiële biomarkers voor epilepsie. Het is zelfs mogelijk om plasma en/of CSF van dieren af te nemen tijdens het uitvoeren van EEG-opnames. Bovendien kan de bemonstering herhaaldelijk worden uitgevoerd gedurende meerdere dagen van het experiment, en kunnen een aantal leeftijds-, geslachts- en epileptische beledigingscontroles worden gebruikt om de robuustheid van het onderzoek te verbeteren. Hier wordt een flexibele techniek beschreven om CSF te verkrijgen uit cisterna magna met parallelle terugtrekking van plasma uit de staartader bij EEG-gemonitorde ratten. De gepresenteerde techniek heeft verschillende voordelen ten opzichte van alternatieve methoden. Door gebruik te maken van een vlindernaaldbenadering is het mogelijk om CSF meerdere keren te verzamelen zonder de functie van EEG-elektroden of soortgelijke hoofdimplantaten in gevaar te brengen. Dit betekent een verfijning van de procedures voor het terugtrekken van intrathecale katheters, die gepaard gaan met een relatief hoog risico op infectie. Bovendien is de gerapporteerde vrije val-druppelbenadering die wordt gebruikt voor bloedafname superieur aan andere benaderingen van bloedafname in de staartader vanwege het sterk verminderde risico op hemolyse, vanwege het feit dat bloed niet door slangen gaat en er geen vacuümdruk wordt uitgeoefend. Als het onder strikte kiemvrije omstandigheden wordt uitgevoerd, is er een bijzonder laag risico op infectie voor dieren. Bovendien kan de bemonstering meerdere keren worden herhaald door de bloedafname helemaal aan het einde van de staart van de dieren te starten. Omdat de samenstelling van lichaamsvloeistoffen veel fysiologische en pathologische dynamieken weerspiegelt, worden biologische vloeistofmonsters vaak verkregen in veel experimentele contexten om interessante moleculen te meten, zoals hormonen, groeifactoren, eiwitten of kleine niet-coderende RNA's. Een concreet voorbeeld is de bemonstering van biologische vloeistoffen in het onderzoek naar biomarkers voor epilepsie. In deze studies is het wenselijk om de niveaus van moleculen in cerebrospinale vloeistof (CSF) en in plasma te vergelijken, door CSF en plasma parallel te trekken en rekening te houden met de tijdsafstand van de bemonstering van en tot de aanvallen. De gecombineerde CSF- en plasmabemonstering, gekoppeld aan video-EEG-monitoring bij epileptische dieren, is een veelbelovende benadering voor de validatie van vermeende diagnostische en prognostische biomarkers. Hier wordt een procedure beschreven van gecombineerde CSF-terugtrekking uit cisterna magna en bloedafname uit de laterale staartader bij epileptische ratten die continu video-EEG worden gecontroleerd. Deze procedure biedt aanzienlijke voordelen ten opzichte van andere veelgebruikte technieken. Het maakt snelle bemonstering mogelijk met minimale pijn of invasiviteit en een kortere anesthesietijd. Bovendien kan het worden gebruikt om CSF- en plasmamonsters te verkrijgen in zowel vastgebonden als telemetrie EEG-geregistreerde ratten, en het kan herhaaldelijk worden gebruikt gedurende meerdere dagen van experiment. Door de stress als gevolg van bemonstering te minimaliseren door isofluraananesthesie te verkorten, wordt verwacht dat metingen de werkelijke niveaus van onderzochte moleculen in biovloeistoffen nauwkeuriger weergeven. Afhankelijk van de beschikbaarheid van een geschikte analytische test, kan deze techniek worden gebruikt om de niveaus van meerdere, verschillende moleculen te meten terwijl tegelijkertijd EEG-registratie wordt uitgevoerd.

Cerebrospinal fluid (CSF) and blood sampling are important to identify and validate biomarkers of epilepsy, in both preclinical and clinical research1,2. Nowadays, the diagnosis of epilepsy and most of the research on epilepsy biomarkers focus on EEG and neuroimaging3,4,5. These approaches, however, present several limitations. Apart from routine scalp measurements, in many cases, EEG requires invasive techniques like depth electrodes6. Brain imaging methods have poor temporal and spatial resolution and are relatively expensive and time-consuming7,8. For this reason, the identification of non-invasive, low-cost, and biofluid-based biomarkers would provide a very attractive alternative. In addition, these biofluid biomarkers could be combined with available diagnostic approaches to sharpen their predictiveness.
Patients diagnosed with epilepsy are routinely submitted to EEG9,10 and blood sampling11,12,13,14, and many also to CSF withdrawal to exclude life-threatening causes (i.e., acute infections, autoimmune encephalitis)15. These blood and CSF samples can be used in clinical research aiming to identify biomarkers for epilepsy. For example, Hogg and co-workers have found that an increase in three plasma tRNA fragments precedes seizure occurrence in human epilepsy14. Similarly, interleukin-1beta (IL-1&#946;) levels in human CSF and serum, expressed as ratio of IL-1&#946; levels in CSF over serum, can predict post-traumatic epilepsy development after traumatic brain injury16. These studies highlight the importance of biofluids sampling for epilepsy biomarkers research, but they face multiple limitations intrinsic to clinical trials, e.g., the cofounding factor of anti-epileptic drugs (AEDs) in blood, the frequent lack of etiology information, inadequate controls, modest numbers of patients, and others17,18.
Pre-clinical research offers other opportunities for investigating molecules in biofluids as potential biomarkers for epilepsy. In fact, it is possible to withdraw plasma and/or CSF from animals while performing EEG recordings. Moreover, sampling can be performed repeatedly across multiple days of the experiment, and a number of age, sex, and epileptic insult-matched controls can be used to improve the study&#39;s robustness. Here, a flexible technique to obtain CSF from cisterna magna with parallel withdrawal of plasma from the tail vein in EEG-monitored rats is described in detail. The presented technique has several advantages over alternative methods. By using a butterfly needle approach, it is possible to collect CSF several times without compromising the function of EEG electrodes or similar head implants. This represents a refinement of intrathecal catheter withdrawal procedures, which are associated with a relatively high risk of infection. In addition, the reported free fall dropping approach used for blood collection is superior to other approaches of tail vein blood withdrawal because of the highly reduced risk of hemolysis, due to the fact that blood does not pass through tubing and no vacuum pressure is applied. If performed under strict germ-free conditions, there is a particularly low risk of infection for animals. In addition, by starting the blood withdrawals at the very end of the animals&#39; tails, sampling can be repeated several times. Such techniques are easy to master and can be applied in many preclinical studies of central nervous system disorders.

In ons onderzoek willen we diagnostische en prognostische biomarkers voor epilepsie vinden.
