El aborto espontáneo recurrente (RM) es la pérdida de dos o más embarazos consecutivos y es una enfermedad compleja que ha llamado la atención de los médicos en los últimos años. La tasa de incidencia de RM en mujeres en edad fértil es del 1%-5% 1. Los resultados de estudios previos muestran que los factores inmunes están estrechamente asociados con la patogénesis de RM 2,3,4,5. El mantenimiento de la homeostasis inmunitaria en la interfaz materno-fetal es necesario para la implantación y el desarrollo embrionario. Las células inmunitarias endometriales desempeñan varias funciones reguladoras para mantener esta homeostasis, como promover la invasión del trofoblasto, remodelar las arterias espirales y contribuir al desarrollo de la placenta 6,7,8,9.
Se han reportado previamente células inmunitarias endometriales aberrantes en mujeres con RM. Los resultados muestran una estrecha asociación entre la alta densidad de células asesinas naturales uterinas (uNK) y la ocurrencia de RM10,11,12. Se ha descrito un aumento del número de macrófagos en el endometrio de mujeres con RM, en comparación con las que tuvieron un nacido vivo13. Las células T reguladoras (Treg) desempeñan un papel en la inmunidad materna hacia el embrión, y su nivel y función están disminuidos en la decidua de los pacientes con RM14. Los linfocitos T (CTL) y los linfocitos dendríticos (CD) también desempeñan un papel en la regulación inmunitaria del embarazo15,16. Por lo tanto, un análisis cuantitativo exhaustivo de las células inmunitarias endometriales locales durante la fase lútea media podría ayudar a comprender mejor la patogénesis de la RM. Algunos métodos actuales para el análisis cuantitativo de las células inmunitarias endometriales utilizan la citometría de flujo, que puede etiquetar con precisión las células inmunitarias con múltiples marcadores17,18. Sin embargo, la aplicación clínica de la citometría de flujo es limitada porque solo se puede realizar en tejido fresco. La obtención de tejido fresco solo es factible cuando se dispone de un gran volumen de exceso de tumor, algo poco frecuente en el endometrio. La inmunohistoquímica puede observar bien la morfología de los tejidos in situ y también puede marcar varias células inmunitarias, mientras que las técnicas inmunohistoquímicas tradicionales no pueden realizar análisis cuantitativos de las células inmunitarias.
En comparación con los experimentos de inmunohistoquímica convencionales, el análisis inmunohistoquímico cuantitativo de las células inmunitarias del endometrio tiene una importancia clínica importante. La puntuación de la intensidad de la IHQ suele clasificarse en una escala de cuatro puntos o fuerte y débil en el diagnóstico patológico y la investigación 19,20,21. Sin embargo, esta técnica semicuantitativa es subjetiva, altamente inexacta y demuestra una variabilidad significativa intraobservador e interobservador22. Una posible solución es la aplicación del aprendizaje automático, que es valioso en el análisis de imágenes digitales23,24. Al proporcionar mediciones cuantitativas, este enfoque permite una evaluación más precisa de la infiltración, distribución y densidad de las células inmunitarias dentro del tejido uterino. Esta información cuantitativa puede ayudar a dilucidar los cambios dinámicos en las poblaciones de células inmunitarias durante el ciclo menstrual y en diversas condiciones patológicas. En general, la capacidad de analizar cuantitativamente las células inmunitarias del endometrio a través de la inmunohistoquímica ofrece información valiosa sobre el microambiente inmunitario del útero.
Por lo tanto, el protocolo tenía como objetivo desarrollar y validar una plataforma digital de análisis de imágenes inmunohistoquímicas para analizar cuantitativamente las células inmunitarias endometriales, incluidas las células uNK, Tregs, macrófagos, CD y células T citotóxicas durante la fase lútea media en pacientes con RM.

platform for quantitative detection of endometrial immune cells based on immunohistochemistry and digital image analysis

Para evaluar el microambiente inmune endometrial de pacientes con aborto espontáneo recurrente (RM), se desarrolló y validó una plataforma de análisis de imágenes inmunohistoquímicas digitales para analizar cuantitativamente las células inmunes endometriales durante la fase lútea media. Todas las muestras de endometrio se recogieron durante la fase lútea media del ciclo menstrual. Los tejidos endometriales incluidos en parafina se seccionaron en portaobjetos de 4 μm de espesor y se llevó a cabo una tinción inmunohistoquímica (IHQ) para detectar células inmunitarias endometriales, incluidas las células uNK CD56+, las Tregs Foxp3+, los macrófagos CD163+ M2, las CD CD1a+ y las células T CD8+. Las diapositivas panorámicas se escanearon con un escáner digital de diapositivas y se utilizó un sistema comercial de análisis de imágenes para el análisis cuantitativo. El porcentaje de células inmunitarias endometriales se calculó dividiendo el número de células inmunitarias en el total de células endometriales. Recurrent miscarriage (RM) is the loss of two or more consecutive pregnancies and is a complex disease drawing attention from clinicians in recent years.The incidence rate of RM in women of childbearing age is 1%-5% 1. Results of previous studies show that immune factors are closely associated with the pathogenesis of RM2,3,4,5. Maintaining immune homeostasis at the maternal-fetal interface is required for embryo implantation and development. Endometrial immune cells perform several regulatory roles to maintain this homeostasis, such as promoting trophoblast invasion, remodeling spiral arteries, and contributing to placenta development6,7,8,9.
Aberrant endometrial immune cells in women with RM have previously been reported. Results show a close association between the high density of uterine natural killer cells (uNKs) and the occurrence of RM10,11,12. An increased number of macrophages has been reported in the endometrium of women with RM, compared with those who had a live birth13. Regulatory T cells (Treg) play a role in maternal immune tolerance toward the embryo, and their level and function are decreased in the decidua of RM patients14. Cytotoxicity T cells (CTL) and dendritic cells (DCs) also play a role in the immune regulation of pregnancy15,16. Therefore, a comprehensive quantitative analysis of local endometrial immune cells during the mid-luteal phase could help to better understand the pathogenesis of RM. Some current methods for quantitative analysis of endometrial immune cells use flow cytometry which can accurately label immune cells with multiple markers17,18. However, clinical application of flow cytometry is limited because it can only be performed on fresh tissue. Obtaining fresh tissue is only feasible when a large volume of excess tumor is available, a rare occurrence for endometrium. Immunohistochemistry can observe tissue morphology well in situ and can also label various immune cells, while traditional immunohistochemical techniques cannot perform quantitative analysis of immune cells.
Compared to conventional immunohistochemistry experiments, quantitative immunohistochemical analysis of immune cells in the endometrium has important clinical significance. IHC intensity scoring is usually ranked on a four-point scale or strong and weak in pathological diagnostics and research19,20,21. However, this semi-quantitative technique is subjective, highly inaccurate, and demonstrates significant intra-observer and inter-observer variability22. One possible solution is the application of machine learning, which is valuable indigital image analysis23,24. By providing quantitative measurements, this approach enables a more precise assessment of immune cell infiltration, distribution, and density within the uterine tissue. This quantitative information can help to elucidate the dynamic changes in immune cell populations during the menstrual cycle and in various pathological conditions. Overall, the ability to quantitatively analyze immune cells in the endometrium through immunohistochemistry offers valuable insights into the immune microenvironment of the uterus.
Therefore, the protocol aimed to developed and validated a digital immunohistochemistry image analysis platform to quantitatively analyze endometrial immune cells including uNK cells, Tregs, macrophages, DCs and cytotoxic T cells during the mid-luteal phase in RM patients.

Currently, it is challenging to stabilize the analysis within a certain range. It is also difficult to maintain the quality control of the experiment. Our research can help the patients with abnormal endometrial sensitivity in immune to find the cause of reproductive failure.The local proportion of immune cells in the endometrial can be stably and repeatedly detected. Our findings may promote the research progress of the endometrial sensitivity evaluation in patients with repeated implantation failure.
