מחלת פיירוני (PD) היא מצב פיברוטי כרוני התורם לעיוות הפין, עקמומיות, כאב, נכות מינית ואף מוביל למצוקה פסיכולוגית1. פרקינסון מייצג ככל הנראה מצב הטרוגני, כאשר גורמים אפיגנטיים תורשתיים וסביבתיים כאחד תורמים להתפתחותו והתקדמותו2. 
האפידמיולוגיה המדווחת של פרקינסון משתנה ועשויה להיות מדווחת בחסר בגלל מבוכה ותפיסות מוטעות לגבי אפשרויות הטיפול הזמינות, עם 0.4%-20.3% בקרב קבוצות אתניות וקבוצות שונות 2,3. הטיפול בפרקינסון כולל טיפול שמרני או התערבות כירורגית, בהתאם לחומרת הסימפטומים ויציבותם. טיפול שמרני הוא ההמלצה הנפוצה ביותר בשלב הפעיל, בעוד טיפולים פולשניים יותר שמורים לשלבהפסיבי 4. ניסוי אקראי ומבוקר שנערך לאחרונה להערכת היעילות של Collagenase Clostridium Histolyticum (CCH) + RestoreX מתיחת פין לעומת ניתוח + מתיחת פין RestoreX בניהול מחלת פיירוני הדגים תוצאות דומות בין שתי אסטרטגיות אלה5.
למרות שטיפול במתיחת פין וזריקות תוך נגעיות מביאים לשיפור צנוע עבור חולים רבים, התערבות כירורגית נותרה הפתרון הסופי למחלה זו, במיוחד עבור אלה עם עקמומיות חמורה ועיוותים בשעון חול או ציר1. נכון לעכשיו, יישור הפין, כגון סיבוך הפין וחתך פלאק או כריתה חלקית והשתלה, מייצג את הגישה האמינה ביותר לתיקון עקמומיות הפין או עיוות6. עם זאת, השיטות המדווחות יש שיעור גבוה של אובדן חושי, אובדן תפקוד הזקפה, או אי שביעות רצון קוסמטית 5,7,8.
מטחנה חשמלית משמשת בדרך כלל בנוירוכירורגיה ובאורתופדיה ככלי יעיל לעיצוב עצם. הוא מפגין יעילות גבוהה בטחינת רקמות קשות, כמו עצמות, תוך גרימת נזק מינימלי לרקמות רכות. מאפיין זה מעניק לו יתרון ייחודי בטיפול בפלאק קשה.
במחקר זה, פיתחנו אסטרטגיית ניהול חדשה הכוללת טחינה סיבובית חשמלית מיקרוסקופית של הפלאק הסיבי ושימוש בטוניקה וגינליס או קרום הלב של בקר כחומרי השתלה בטיפול בפרקינסון, אשר סיפקו רמות גבוהות של שביעות רצון המטופלים. פרוטוקול זה חל על מטופלים עם עקמומיות חמורה או עיוותים אחרים המגבילים את יכולתם לקיים יחסי מין חודרים משביעי רצון.

מחלת פיירוני (PD) מאופיינת קלינית על ידי התפתחות של פלאקים סיביים מקומיים, בעיקר על טוניקה albuginea, במיוחד על האזור הגבי של הפין. לוחות אלה הם מאפיין ההיכר של מצב זה, וכתוצאה מכך עקמומיות הפין, עיוות, וזקפה כואבת עבור אנשים מושפעים. למרות שקיימות אפשרויות טיפול לא ניתוחיות שונות, יעילותן הכוללת מוגבלת. כתוצאה מכך, התערבות כירורגית הפכה לבחירה האולטימטיבית עבור חולים עם עיוותים חמורים בעקמומיות הפין ותפקוד לקוי של זיקפה. צוות המחקר שלנו השתמש בהצלחה בגישה משולבת הכוללת טחינה סיבובית חשמלית מיקרוסקופית של הפלאק הסיבי ושימוש בטוניקה וגינליס או קרום הלב של בקר כחומרי השתלה לתיקון הפגמים של טוניקה אלבוגיניאה בטיפול בפרקינסון. גישה זו הניבה באופן עקבי תוצאות משביעות רצון ביותר לגבי שיקום צורת הפין, עם תוצאות קוסמטיות מצוינות ושיפור משמעותי בסיפוק המיני. 
פרוטוקול זה נועד להציג אסטרטגיית ניהול כירורגית מקיפה המשתמשת בטחינה סיבובית חשמלית של הפלאק ותיקון הפגמים של טוניקה אלבוגיניאה באמצעות טוניקה וגינליס, המייצגת אסטרטגיה כירורגית אופטימלית לטיפול בפרקינסון.

בסך הכל נכללו 21 חולים מינואר 2021 עד מאי 2023. גיל המטופל נע בין 34 ל 68 שנים, עם ממוצע של 51.5 ± 9.4 שנים. מתוך כלל החולים, 8 (38.1%) סבלו מסוכרת. בסך הכל 16 מטופלים (76.2%) הראו עקמומיות גבית פשוטה משמעותית של הפין, עם זווית עקמומיות של 60-90° ו-5 מטופלים (23.8%) הציגו עיוותים מורכבים דמויי שעון חול עם עקמומיות דורסיפ...

לאחר הנחת המטופל המרדים במצב שכיבה על מיטת הניתוח, השתמש בגומייה כחוסם עורקים כדי לקשור את בסיס הפין. להזריק מלוחים רגילים לתוך corpus cavernosum של הפין כדי לגרום לזקפה מלאכותית. בצע חתך מעגלי מתחת לחריץ העטרה.בצעו כריתה היקפית של העורלה, יחד עם הפאשיה השטחית, וחשפו את הפאשיה של הבאק. באמצעות להב עגול קטן, בצע חתך אורכי של כארבעה עד שישה סנטימטרים לאורך שני צידי פיר הפין הסמוך לשופכה בגובה הפאשיה של באק. ודא כי החתך חושף באופן מלא את אזור הפלאק בצד הגבי של הפין מבלי לפגוע בעצבים ובכלי הדם.בעזרת מיקרו-מספריים או אלקטרודת מחט עדינה, נתחו את הפאשיה של באק והפרידו אותה בזהירות מכל צד לכיוון אזור קו האמצע בצד הגבי של הפין. לאחר חשיפה מלאה של הפלאק בצד הגבי של הטוניקה אלבוגינאה, סמן את היקף הפלאק. באמצעות מקדחה טחינה חשמלית, ablate את הפלקים פיברוטיים.בהתבסס על גודל הפגם, השתמש חד צדדי או דו צדדי tunica vaginalis שתלים או קרום הלב בקר כדי לתקן את הפגמים של albuginea טוניקה. לאחר מכן, מקמו מחדש את צרורות כלי הדם העצביים ואת הפאשיה של באק.
