Denne protokol præsenterer en beskrivende rapport om kirurgiske procedurer for peri-implantitis induktion ved hjælp af en ligaturmodel hos mus. Tandimplantater er i stigende grad udbredt som et ønskeligt valg til udskiftning af manglende tænder1. Forekomsten af tandimplantater i den amerikanske voksne befolkning forventes at stige op til 23% inden 20262. Baseret på en markedsanalyserapport fra Grand View Research (2022) forventedes den globale markedsstørrelse for tandimplantater at nå ca. 4,6 mia. USD i 2022. Desuden forventes det at udvise en stabil årlig vækstrate på omkring 10% indtil år 20303. Desværre kan brugen af tandimplantater føre til komplikationer, såsom peri-implantitis. Peri-implantitis er blevet defineret som en biofilm-induceret tilstand karakteriseret ved betændelse i peri-implantat slimhinden og efterfølgende progressivt tab af støtteben4.
En systematisk gennemgang viste, at den gennemsnitlige forekomst af peri-implantitis var 19,53% (95% konfidensinterval [CI], 12,87 til 26,19%) på patientniveau og 12,53% (95% CI 11,67 til 13,39%) på implantatniveau5. Periimplantitis udgør en voksende folkesundhed på grund af en stigning i implantatsvigt og følgelig betydelige behandlingsomkostninger6.
Forståelse af patogenesen af peri-implantitis er afgørende for at udvikle en systematisk tilgang til at forhindre dets begyndelse og progression og maksimere tandimplantaternes levetid med hensyn til æstetik og funktion 7,8. I denne forstand har det vist sig fordelagtigt at bruge murinmodeller i tandforskning, da mus deler mere end 95% af deres gener med mennesker 9,10, antallet af tilgængelige online genetiske databaser og evnen til at reproducere kliniske scenarier11. Alle de beskrevne fordele tillader dissektion af genetiske mekanismer i forskellige sygdomme12, tilgængelig indkvartering og styring og antistoffer, der er bredt tilgængelige som humane paneler, ud over tilgængeligheden af genetisk modifikation (f.eks. knockout og overekspression) til inflammatorisk vævsvurdering og sygdomskortlægning13. Dette skyldes blandt andet metodologiske udfordringer, herunder vanskeligheden ved at få miniimplantater eller installere dem.
For at udvikle peri-implantitis hos mus er mange protokoller blevet beskrevet, såsom ligatur-induceret peri-implantitis, bakterie-induceret peri-implantitis14, Lipopolysaccharid (LPS) -induceret peri-implantitis15, eller kombinationen LPS + ligatur-induceret peri-implantitis16. Her vil vi fokusere på ligaturmodellen, fordi det er den mest accepterede metode til at inducere parodontitis 17,18,19 og for nylig peri-implantitis 20,21. Ligaturen placeret omkring implantaterne i en submukosal position stimulerer plaqueakkumulering og følgelig vævsinflamation. Udviklingen af denne tilgang er således baseret på indikationen af en levedygtig cost-benefit-teknik til prækliniske undersøgelser af periimplantationssygdomme. Denne undersøgelse har til formål at beskrive en eksperimentel model af ligatur-induceret peri-implantitis hos mus og afgøre, om der er effektivitet i at inducere denne sygdom i betragtning af de observerede knogle- og vævsændringer.
Det overordnede mål med denne artikel er at rapportere den protokol, der anvendes til at inducere peri-implantitis hos mus ved ligatur og at observere dens effektivitet gennem vævsevaluering og knogletab omkring implantaterne.

Tandimplantater har en høj succes og overlevelsesrate. Imidlertid er komplikationer som peri-implantitis (PI) meget udfordrende at behandle. PI er karakteriseret ved betændelse i vævene omkring tandimplantater med progressivt tab af støtteben. For at optimere tandimplantaters levetid med hensyn til sundhed og funktionalitet er det afgørende at forstå peri-implantitis patofysiologi. I denne henseende har brug af musemodeller i forskning vist sig at være klare fordele ved at genskabe kliniske omstændigheder. Denne undersøgelse havde til formål at beskrive en eksperimentel model af ligatur-induceret peri-implantitis hos mus og afgøre, om der er effektivitet i at inducere denne sygdom i betragtning af de observerede knogle- og vævsændringer. Den eksperimentelle peri-implantitis induktion forstår følgende trin: tandekstraktion, implantatplacering og ligatur-inducted PI. En prøve på atten 3 uger gamle C57BL/6J hanmus blev opdelt i to grupper, ligatur (N = 9) og kontrol non-ligatur (N = 9). Evalueringen af kliniske, radiografiske og histologiske faktorer blev udført. Ligaturgruppen viste signifikant højere knogletab, øget blødt vævsødem og apikal epitelmigration end ikke-ligaturgruppen. Det blev konkluderet, at denne prækliniske model med succes kan inducere periimplantitis hos mus.

Til denne metode blev atten 3 uger gamle C57BL/6J hanmus brugt og gennemgik tandekstraktioner, implantatplacering og peri-implantitis induktion. Der var ni dyr pr. gruppe, hvilket var statistisk signifikant, idet lineært knogletab opnåede 80% effekt, 15% standardafvigelse (σ) og 95% konfidensinterval (α = 0,05). Mus blev fodret med en blød kost ad libitum under eksperimentet. Ni mus fik en ligatur (ligatur-induceret periimplantitis-eksperimentel gruppe), og ni mus modtog ikke ligatur (kontrolgruppe).

Det overordnede mål med denne artikel er at rapportere den protokol, der anvendes til at inducere peri-implantitis hos mus ved ligatur og at observere dens effektivitet gennem vævsevaluering og knogletab omkring implantaterne. Følgende procedurer blev udført i et kirurgisk rum, der overholdt alle biosikkerheds- og beskyttelsesstandarder. Alle procedurer blev udført under 10X mikroskopisk forstørrelse og udført af uddannede og kalibrerede operatører.Alle operationer blev udført under inhalationsanæstesi med isofluran og ilt. En assisterende operatør var nødvendig for at stabilisere dyret og opretholde mundåbningen. For at forhindre irritation i øjnene blev oftalmisk smøremiddel brugt, inden ekstraktionen blev startet.Til denne metode blev 18 tre uger gamle C57BL/6J hanmus brugt og gennemgik tandekstraktioner, implantatplacering og peri-implantitis induktion. Umiddelbart efter ekstraktion modtog alle dyr smertestillende medicin, administreret gennem subkutan injektion. Derudover blev regelmæssig mad i fire uger efter ekstraktioner erstattet af en blød kost, og antibiotika blev administreret oralt, idet medicinen blev inkorporeret i drikkevand.Implantat placering. Ved hjælp af et 15c blad blev der lavet et mesiodistalt snit gennem det keratiniserede væv i området svarende til de tidligere tilstedeværende tænder. Osteotomien blev udført ved hjælp af en mikrohåndbor med en diameter på 0,3 millimeter hårdmetal fastgjort til en stiftskruestik og aktiveret gennem rotation med uret. Osteotomistederne var cirka en millimeter dybt ind i de helede ekstraktionsstik.Derefter blev titaniumimplantater, et pr. Dyr, selvtappet i området for den første og anden maksillære venstre molarer ved hjælp af en skruebevægelse med uret. Implantaterne havde en fire ugers helingsperiode, hvor musene fik antibiotika og fodres som tidligere beskrevet. Ligaturer blev kontrolleret hver anden dag for at sikre, at de stadig var til stede.Hvis der manglede, blev en ny ligatur placeret. Efter denne periode blev alle dyr ofret. Maxillae blev høstet, fotograferet ved hjælp af et optisk mikroskop, fastgjort i 10% formalin i 24 timer og derefter opbevaret i 70% ethanol.Klinisk evaluering blev udført ved at tage billeder umiddelbart efter ofringer ved hjælp af optisk mikroskopi. Sammenlignet med kontrolgruppen blev inflammation, lommedannelse og øget bløddelsødem observeret omkring implantatet i peri-implantitisgruppen. Der blev ikke observeret tegn på alvorlige kliniske fænotypekomplikationer.To uger efter ligaturplacering, når man sammenlignede ikke-ligatur- og ligaturgrupper, var der signifikante forskelle i knoglehøjde observeret via lineær analyse og knogletabsvolumen observeret via volumetrisk analyse. For at bestemme cellulære ændringer blev afkalkede prøver snittet og farvet med hæmatoxylin og eosin. Som et resultat blev der observeret mere apikal epitelmigration og knogletab i peri-implantitisprøver sammenlignet med kontrolgruppen.Efter at have set denne video skal du have en god forståelse af, hvordan du inducerer periimplantitis i en musemodel, samt identificere de kliniske, mikrotomografiske og histologiske forskelle, der findes. Selv om denne metode ikke repræsenterer alle peri-implantitis-aspekterne, er den uundværlig for at etablere et årsagssammenhæng, der giver information om vævsheling og understøtter undersøgelser vedrørende knogletab og periimplantatsygdom.
