1. Procédures préopératoires
<ol><li>Stériliser les instruments chirurgicaux. Autoclave tous les instruments chirurgicaux avant la chirurgie. Si vous effectuez plusieurs interventions chirurgicales le même jour, stérilisez les instruments entre les chirurgies à l’aide d’un stérilisateur de perles sèches.</li>
	<li>Préparer thérapeutique dans 25% pluronique-127 gel (dilué dans de l’eau distillée stérile).</li>
	<li>Installez un cathéter de ballon Fogarty 2F jusqu’à l’insufflateur et placez l’extrémité ballon du cathéter dans une seringue de 1 mL remplie de solution saline.</li>
	<li>Induire l’anesthésie en plaçant le rat dans une chambre avec 5% d’isoflurane.<ol>
<li>Retirez le rat de la chambre et enregistrez le poids du rat. Utilisez des tondeuses à cheveux pour raser la fourrure sur la région ventrale du cou.</li>
		<li>Remettre le rat dans la chambre avec 5% d’isoflurane pour assurer l’induction de l’anesthésie.</li>
	</ol>
</li>
	<li>Placez la supine de rat sur une plate-forme chirurgicale, insérant le visage dans le cône de nez de sorte que le visage de rat soit vers le chirurgien.<ol>
<li>Réduire l’anesthésie par inhalation à 1,5% d’isoflurane. Vérifiez la profondeur de l’anesthésie par un réflexe de pincement des pieds sur les quatre pieds.</li>
		<li>Tapez les quatre jambes jusqu’à la plate-forme chirurgicale.</li>
	</ol>
</li>
	<li>Allumez la lampe thermique.</li>
	<li>Injecter de l’atropine (0,01 mg/kg) sous-cutanée pour réduire les sécrétions des voies respiratoires.</li>
	<li>Injecter du carprofène (5 mg/kg) sous-cutané pour la gestion de la douleur prophylactique. Si les anti-inflammatoires ne sont pas acceptables pour l’expérience se réfèrent aux étapes 3.2.2 et 3.2.3.</li>
	<li>Appliquer de l’onguent lubrifiant sur les deux yeux à l’aide d’un coton-tige stérile pour empêcher les cornées de sécher pendant la chirurgie.</li>
	<li>Écouvillonner le cou trois fois dans un mouvement circulaire alternant entre 70% d’alcool éthylique suivi de betadine du centre de la région rasée vers l’extérieur pour stériliser le site d’incision.</li>
	<li>Infiltrer la ligne d’incision .c. avec 0,25% de bupivacaine.</li>
	<li>Mettez des gants chirurgicaux stériles avant de manipuler des instruments chirurgicaux stériles et des fournitures.</li>
	<li>Déposer tous les instruments chirurgicaux autoclavés sur une feuille chirurgicale stérile.</li>
	<li>Couper trois brins indépendants de 1 pouce de suture stérile 7-0 Prolene.</li>
	<li>Déposer les cotons-tiges et la gaze sur une feuille chirurgicale.</li>
	<li>Draper le rat d’une feuille chirurgicale stérile qui n’expose que la région du cou stérilisée.</li>
	<li>Couper une petite ouverture supplémentaire en feuille qui expose une partie du cône avant. Ce sera le site pour enregistrer le cathéter ballon pendant les blessures.</li>
</ol>2. Procédures opérationnelles
<ol><li>Pendant toute la procédure chirurgicale, évaluer la profondeur de l’anesthésie en surveillant le taux respiratoire (le taux doit être constant et jugé normal) ainsi que par pincement des pieds toutes les 15 minutes. Si le taux respiratoire augmente ou s’il y a une réponse au pincement de l’orteil, alors interrompez la manipulation chirurgicale et augmentez l’isoflurane jusqu’à 2,5%.</li>
	<li>Exposer l’artère carotide commune (CCA).<ol>
<li>Faire une incision superficielle, droite et longitudinale encolure entre les os de la mâchoire du rat. Incision sera d’environ 1,5-2 cm de longueur.</li>
		<li>Faire une incision à travers le tissu conjonctif sous la peau jusqu’à ce que la couche musculaire soit exposée. Déplacez les glandes salivaires sous la peau pour accéder au tissu musculaire.</li>
		<li>Séparez brutalement le tissu conjonctif du muscle en insérant des ciseaux fermés entre la couche musculaire et le tissu conjonctif et en ouvrant doucement le ciseaux tout en tirant la peau vers le haut.</li>
	</ol></li>
	<li>Disséquer les deux muscles visibles (sternohyoid et sternomastoid) longitudinoïdement le long du côté gauche de la trachée jusqu’à ce qu’un troisième muscle (omohyoid) qui court perpendiculairement aux deux muscles superficiels soit observé.</li>
	<li>Utilisez des forceps pour créer une fenêtre séparant ce muscle perpendiculaire (omohyoid) du muscle longitudinal (sternohyoid) fonctionnant au sommet de la trachée. Effectuez doucement cette séparation pour éviter un traumatisme contondant à la trachée.</li>
	<li>Atteindre les forceps sous le muscle perpendiculaire et couper pour séparer les deux muscles longitudinals et exposer le CCA.</li>
	<li>Disséquer la CCA.<ol>
<li>Disséquer le CCA près de la bifurcation jusqu’à ce que l’artère carotide interne (ICA) et l’artère carotide externe (ECA) soient exposées.</li>
		<li>Disséquer l’artère thyroïdienne supérieure (STA), qui se ramifie de l’ECA.</li>
		<li>À l’aide des sutures prolènes pré-coupées, ligatez le STA et l’ECA près de leur bifurcation respective. Laisser la majorité de la suture d’un côté du nœud et saisir chaque suture avec un hémostat incurvé.</li>
		<li>Finissez de disséquer autour de l’ICA, atteignez les forceps en dessous et autour de l’ICA, et utilisez une pince vasculaire non concassante pour atteindre le contrôle distal. Serrez l’artère occipitale avec l’ICA.</li>
		<li>Disséquer le CCA proximal à la bifurcation, en veillant à séparer le nerf vague de la CCA.</li>
		<li>Atteindre les forceps en dessous et autour de la CCA et utiliser une pince vasculaire non concassante pour atteindre le contrôle proximal. Placer la pince à au moins 5 mm de la bifurcation.</li>
	</ol></li>
	<li>Effectuer des blessures ballon.<ol>
<li>Manœuvrez les hémostats incurvés tenant chaque branche ligatée d’artère pour exposer la bifurcation entre l’ECA et la branche supérieure.</li>
		<li>Disséquer délicatement le tissu à la bifurcation, puis faire une incision artériotomie entre l’ECA et la branche supérieure à l’aide de ciseaux de microdissection.</li>
		<li>Utilisez un coton-tige pour pousser tout le sang hors de la CCA et nettoyer le site d’artériotomie.</li>
		<li>Insérez le cathéter ballon non gonflé à travers l’artériotomie et avancez dans le CCA jusqu’à ce que l’extrémité proximale du ballon soit passée la bifurcation.</li>
		<li>Cathéter de bande au cône avant de sorte que le ballon ne glisse pas hors de l’artère pendant l’inflation.</li>
		<li>Gonflez lentement le ballon à 5 atmosphères de pression et laissez dans le CCA pendant 5 min pour induire des blessures artérielles. Assurez-vous que la pression reste constante pendant toute la durée de 5 minutes.</li>
		<li>Après 5 min, dégonfler le ballon et retirer délicatement du CCA par l’artériotomie.</li>
		<li>Rincer le CCA en pressant doucement sur la pince du CCA. N’enlevez pas la pince.</li>
		<li>Ligate l’ECA proximal à l’artériotomie, puis enlever les pinces de la CCA et ICA pour rétablir le flux sanguin à travers le CCA à l’ICA. Assurez-vous qu’il n’y a pas de saignement visible autour de l’artériotomie et que le CCA vibre.</li>
	</ol></li>
	<li>Appliquez 100 μL de véhicule thérapeutique ou de gel pluronique seul periadventitially le long de la CCA blessée. Faites-le en appliquant 50 μL sur le côté gauche du CCA, puis 50 μL sur le côté droit du CCA pour assurer un revêtement même de l’artère blessée.</li>
	<li>Fermez le site de la plaie.<ol>
<li>Couper l’excès de sutures Prolene.</li>
		<li>Fermez la plaie à l’aide de couches vicryl interrompues de 4 à 0 ou 6-0 le long du tissu conjonctif.</li>
		<li>Terminer la fermeture de la plaie à l’aide d’une suture en nylon 4-0 le long de la peau.</li>
	</ol></li>
</ol>3. Procédures postopératoires
<ol><li>Placez le rat seul dans une cage propre avec la moitié de la cage sous une lampe chauffante et surveillez jusqu’à ce que le rat reprenne suffisamment conscience pour maintenir la récumbence sternale. Gardez le rat dans une cage séparée jusqu’à ce que l’animal soit complètement alerte et mobile avant de le transférer dans sa cage d’origine.</li>
	<li>Surveillez le rat tous les jours pendant les trois prochains jours, puis trois fois par semaine jusqu’à l’euthanasie. Euthanasier à l’aide d’une surdose d’isoflurane suivie d’une thoracotomie bilatérale décrite ci-dessous.<ol>
<li>Utilisez l’échelle de grimace du Centre national pour le perfectionnement et la réduction des animaux dans la recherche (NC3Rs) pour identifier les niveaux de douleur postopératoire. Si un animal semble éprouver de la douleur ou développer un compromis neurologique, sacrifiez immédiatement.</li>
		<li>Pour les animaux qui ne reçoivent pas de carprofène, administrer de l’acétaminophène 6 mg/mL dans leur eau potable 24 h avant la chirurgie par 48 h après la chirurgie. L’acétaminophène fournit une analgésie avec un minimum d’effets anti-inflammatoires.</li>
		<li>Alternativement, d’autres stratégies d’analgésie avec des effets anti-inflammatoires minimaux peuvent être employées, telles que la buprénorphine ou la libération prolongée de buprénorphine. Consultez l’équipe vétérinaire de votre établissement.</li>
	</ol>
</li>
</ol>4. Récolte et imagerie des tissus
<ol><li>Deux semaines après la chirurgie, euthanasier le rat par surdose d’anesthésie (5% d’isoflurane). Alternativement, euthanasier les rats à un moment plus précoce pour analyser les différents aspects de la réponse des blessures artérielles.<ol>
<li>Une fois que la respiration s’arrête effectuer thoracotomy bilatérale comme une méthode secondaire de l’euthanasie.</li>
	</ol></li>
	<li>Faire une incision latérale à travers l’abdomen, puis couper vers le haut, à travers le diaphragme et les côtes, exposant la cavité thoracique.</li>
	<li>Perfuser et réparer les artères.<ol>
<li>Insérez une canule de 18 G attachée à un système gravitationnel de perfusion-fixation par le ventricule gauche. Maintenir une pression équivalente entre les rats en marquant la hauteur du système de perfusion par rapport au dessus du banc (élévation de 120 cm, équivalent à 91 ± 3 mmHg).</li>
		<li>Serrez la canule avec le ventricule à l’aide d’un hémostat incurvé.</li>
		<li>Faire une coupe dans l’atrium droit, l’ouverture du circuit vasculaire, et commencer la perfusion avec PBS suivie de 2-4% paraformaldéhyde (environ 250 mL chacun).</li>
		<li>Préparez du paraformaldéhyde dilué dans PBS le jour du sacrifice, ou tout au plus la veille du sacrifice. Si vous vous préparez le jour du sacrifice, assurez-vous que le paraformaldéhyde a refroidi à température ambiante avant de commencer la perfusion. Conserver le paraformaldéhyde à 4 °C.</li>
	</ol></li>
	<li>Après fixation, extraire les artères carotides gauche et droite et les conserver à 4 °C pendant 2 h en paraformaldéhyde de 2 à 4 %.</li>
	<li>Transférer les artères à 30% de saccharose et les conserver toute la nuit à 4 °C.</li>
	<li>Après 16-24 h, intégrer les artères dans le composé de température de coupe optimale (OCT) et congeler les blocs d’artère oct-incorporés.<ol>
<li>Conditionner les artères en OCT à température ambiante pendant 10 min. Placez l’artère parallèle au plan du cryomold rempli d’OCT, marquant le côté du cryomold auquel la bifurcation artérielle est confrontée. Snap-freeze dans l’azote liquide.</li>
		<li>Conserver les blocs congelés à long terme à -80 °C.</li>
	</ol></li>
	<li>Sectionz les blocs congelés à l’aide d’un cryostat.<ol>
<li>Recueillir six sections transversales artérielles de 5 μm d’épaisseur par diapositive, la diapositive 1 commençant à la bifurcation.</li>
		<li>Section blocs gelés jusqu’à ce que l’hyperplasie ne soit plus visible (environ 100 diapositives).</li>
	</ol></li>
	<li>Hematoxylin & eosin (H&E) diapositives de tache18<ol>
<li>Trouvez la zone de blessure en soudant une diapositive sur dix le long de toute l’artère à partir de la bifurcation (p. ex., diapositives 1, 10, 20, 30, 40, 50, 60, 70, 80, 90 et 100).</li>
		<li>Tacher d’autres toboggans autour du site de la blessure pour trouver la glissade avec occlusion maximale (p. ex., diapositives 20, 30 et 40 avaient une hyperplasie visible, donc des glissades de taches 15, 25, 35 et 45).</li>
		<li>Tacher et quantifier la diapositive à l’aide d’occlusion maximale et de diapositives équidistantes avant et après la glissade d’occlusion maximale (p. ex., occlusion maximale trouvée à la diapositive 35, puis tacher et quantifier les diapositives 25, 45, etc.) pour un total de 3 à 10 diapositives par rat.</li>
	</ol></li>
	<li>Pour l’imagerie par microscopie par fluorescence des feuilles lumineuses, conserver les artères pendant la nuit à 4 °C après fixation à l’étape 4.4.<ol>
<li>Sondez l’artère avec 1:500 dilution de l’anticorps primaire anti-CD31 de lapin dans le diluant (pH 7.4) pendant 3 jours. Puis contre-tache artère avec 1:500 dilution de l’anti-lapin Alexa Fluor 647 anticorps secondaire pendant 2 jours19.</li>
		<li>Dégagez l’artère à l’aide d’iDISCO+20.</li>
		<li>Image de l’artère à l’aide d’un microscope à fluorescence feuillede lumière 21. Rendre les images à l’aide d’un logiciel (p. ex., Imaris)19.</li>
	</ol></li>
	<li>Quantifier l’hyperplasie néointimale. Effectuez la quantification d’une manière aveuglée si possible.<ol>
<li>Utilisez le logiciel ImageJ pour tracer le périmètre de l’intima, du lamina élastique interne (IEL) et du lamina élastique externe (EEL) d’une artère sur chacune des 3 à 10 diapositives déterminées ci-dessus (étape 4.8.3).</li>
		<li>Quantifier la superficie de chaque région tracée dans ImageJ et exporter ces valeurs. La trace d’intima donne la zone de lumen, la trace IEL cède la zone IEL, et la trace EEL donne la zone EEL.</li>
		<li>Moyenne des valeurs obtenues à partir des diapositives 3-10 pour obtenir la blessure moyenne (% occlusion, intima:media (I:M) ratio, hyperplasie néointimale) par artère carotide de rat. La blessure de ballon d’artère carotide de rat est l’un des modèles animaux de restnosis les plus largement utilisés et étudiés. Le modèle original de blessure deballon 3 et la variation segmentaire modifiée de dommages segmentaires sous pression10 ont informé beaucoup d’aspects de la réponse artérielle de dommages qui se produit également chez l’homme, avec les quelques limitations étant que le thrombus fibrin-riche se développe rarement et l’inflamm...

Les maladies cardiovasculaires (MCV) demeurent la principale cause de décès dans lemonde 1. L’athérosclérose est la cause sous-jacente de la plupart des morbidités et mortalités liées aux MCV. L’athérosclérose est l’accumulation de plaque à l’intérieur des artères qui se traduit par un lumen rétréci, ce qui entrave la perfusion sanguine appropriée aux organes et aux tissus distals2. Les interventions cliniques pour traiter l’athérosclérose grave incluent l’angioplastie de ballon avec ou sans placement de stent. Cette intervention consiste à faire avancer un cathéter de ballon jusqu’au site de la plaque et à gonfler le ballon pour comprimer la plaque jusqu’à la paroi artérielle, élargissant ainsi la zone luminaire. Cette procédure endommage l’artère, cependant, initiant la réponse artérielle de dommages3. L’activation prolongée de cette réponse de blessure mène à la restenosis artérielle, ou re-rétrécissement, secondaire à l’hyperplasie néointimale et au remodelage de navire. Pendant l’angioplastie, la couche intimale est dénudée des cellules endothéliales menant au recrutement immédiat de plaquettes et à l’inflammation locale. La signalisation locale induit des changements phénotypiques dans les cellules musculaires lisses vasculaires (VSMC) et les fibroblastes adventitiels. Cela conduit à la migration et la prolifération de VSMC et fibroblastes vers l’intérieur vers le lumen, conduisant à l’hyperplasie néointimale4,5. Les cellules progénitrices circulants et les cellules immunitaires contribuent également au volume global de restenose6. Le cas échéant, les endosseurs d’échappement de médicaments (DES) sont la norme actuelle pour inhiber la restenose7. Des inhiber la ré-endothélialisation artérielle, cependant, créant ainsi un environnement pro-thrombotic qui peut avoir comme conséquence la thrombose tard dans-stent8. Par conséquent, les modèles animaux font partie intégrante à la fois pour comprendre la pathophysiologie de la restenose, et pour développer de meilleures stratégies thérapeutiques pour prolonger l’efficacité des procédures de revascularisation.
Plusieurs grands et petits modèlesanimaux 9 sont utilisés pour étudier cette pathologie. Il s’agit notammentde ballon-blessure 3,10 ou wire-blessure11 du côté luminal d’une artère, ainsi que la ligature partielle12 ou le placement de manchette13 autour de l’artère. Les dommages de ballon et de fil denudent la couche endothéliale de l’artère, imitant ce qui se produit médicalement après angioplastie. En particulier, les modèles de ballons-blessures utilisent des outils similaires à ceux du milieu clinique (c.-à-d. cathéter ballon). La blessure de ballon est mieux exécutée dans les modèles de rat, car les artères de rat sont une taille appropriée pour les cathéters disponibles dans le commerce de ballon. Nous décrivons ici une blessure artérielle segmentale contrôlée par pression, une version bien établie et modifiée de la blessure de ballon d’artère carotide de rat. Cette approche contrôlée par pression imite étroitement la procédure clinique d’angioplastie, et permet la formation néointimal reproductible d’hyperplasie deux semainesaprès blessure 14,15. En outre, cette blessure artérielle contrôlée par pression a comme résultat la restauration complète de couche endothéliale par 2 semaines après chirurgie16. Cela contraste directement avec le modèle original de blessure de ballon, décrit par Clowes, où la couche endothéliale ne revient jamais à la couverture complète3.
Après la chirurgie, des thérapeutiques peuvent être appliquées ou dirigées vers l’artère blessée par plusieurs approches. La méthode décrite ci-après utilise l’application périadventitiale d’une petite molécule intégrée dans une solution de gel pluronique. Plus précisément, nous appliquons une solution d’aldéhyde cimnamic de 100 μM dans le gel Pluronic-F127 de 25% à l’artère immédiatement après blessure pour inhiber la formation néointimale d’hyperplasie15. Pluronic-F127 est un gel thermoréversible non toxique capable de livrer des médicaments localement d’une manière contrôlée17. Pendant ce temps, les lésions artérielles sont locales, d’où l’administration locale permet de tester un principe actif tout en minimisant les effets hors cible. Néanmoins, la livraison efficace d’une thérapeutique utilisant cette méthode dépendra de la chimie de la petite molécule ou biologique utilisée.

Les maladies cardiovasculaires demeurent la principale cause de décès et d’invalidité dans le monde, en partie à cause de l’athérosclérose. La plaque athéroclérotique rétrécit la surface luminaire dans les artères réduisant ainsi le flux sanguin adéquat vers les organes et les tissus distals. Cliniquement, les procédures de revascularisation telles que l’angioplastie de ballon avec ou sans placement de stent visent à reconstituer le flux sanguin. Bien que ces procédures rétablissent le flux sanguin en réduisant le fardeau de la plaque, elles endommagent la paroi du vaisseau, ce qui déclenche la réponse artérielle de guérison. La réponse de guérison prolongée provoque une reposnose artérielle, ou re-rétrécissement, limitant finalement le succès à long terme de ces procédures de revascularisation. Par conséquent, les modèles animaux précliniques font partie intégrante de l’analyse des mécanismes pathophysiologiques qui conduisent la restenose, et offrent la possibilité de tester de nouvelles stratégies thérapeutiques. Les modèles Murine sont moins chers et plus faciles à utiliser que les grands modèles animaux. Les blessures par ballon ou par fil sont les deux modalités de blessures couramment acceptées utilisées dans les modèles murins. Les modèles de dommages de ballon en particulier imitent la procédure clinique d’angioplastie et causent des dommages proportionnés à l’artère pour le développement de la restenosis. Ci-après, nous décrivons les détails chirurgicaux pour effectuer et analyser histologiquement le modèle modifié et contrôlé par pression de dommages de ballon d’artère de rat. En outre, ce protocole met en évidence comment l’application périadventitiale locale des thérapeutiques peut être employée pour inhiber l’hyperplasie néointimale. Enfin, nous présentons la microscopie de fluorescence de feuille de lumière comme approche nouvelle pour l’imagerie et la visualisation des dommages artériaux dans trois dimensions.

Implantation of a Carotid Cuff for Triggering Shear-stress Induced Atherosclerosis in Mice

It is widely accepted that alterations in vascular shear stress trigger the expression of inflammatory genes in endothelial cells and thereby induce atherosclerosis (reviewed in 1 and 2). The role of shear stress has been extensively studied in vitro investigating the influence of flow dynamics on cultured endothelial cells 1,3,4 and in vivo in larger animals and humans 1,5,6,7,8. However, highly reproducible small animal models allowing systematic investigation of the influence of shear stress on plaque development are rare. Recently, Nam et al. 9 introduced a mouse model in which the ligation of branches of the carotid artery creates a region of low and oscillatory flow. Although this model causes endothelial dysfunction and rapid formation of atherosclerotic lesions in hyperlipidemic mice, it cannot be excluded that the observed inflammatory response is, at least in part, a consequence of endothelial and/or vessel damage due to ligation.
In order to avoid such limitations, a shear stress modifying cuff has been developed based upon calculated fluid dynamics, whose cone shaped inner lumen was selected to create defined regions of low, high and oscillatory shear stress within the common carotid artery 10. By applying this model in Apolipoprotein E (ApoE) knockout mice fed a high cholesterol western type diet, vascular lesions develop upstream and downstream from the cuff. Their phenotype is correlated with the regional flow dynamics 11 as confirmed by in vivo Magnetic Resonance Imaging (MRI) 12: Low and laminar shear stress upstream of the cuff causes the formation of extensive plaques of a more vulnerable phenotype, whereas oscillatory shear stress downstream of the cuff induces stable atherosclerotic lesions 11. In those regions of high shear stress and high laminar flow within the cuff, typically no atherosclerotic plaques are observed.
 
In conclusion, the shear stress-modifying cuff procedure is a reliable surgical approach to produce phenotypically different atherosclerotic lesions in ApoE-deficient mice.

The constricting cuff presented in this article is designed to induce atherosclerosis in the murine common carotid artery. Vascular Gene Transfer from Metallic Stent Surfaces Using Adenoviral Vectors Tethered through Hydrolysable Cross-linkers

In-stent restenosis presents a major complication of stent-based revascularization procedures widely used to re-establish blood flow through critically narrowed segments of coronary and peripheral arteries. Endovascular stents capable of tunable release of genes with anti-restenotic activity may present an alternative strategy to presently used drug-eluting stents. In order to attain clinical translation, gene-eluting stents must exhibit predictable kinetics of stent-immobilized gene vector release and site-specific transduction of vasculature, while avoiding an excessive inflammatory response typically associated with the polymer coatings used for physical entrapment of the vector. This paper describes a detailed methodology for coatless tethering of adenoviral gene vectors to stents based on a reversible binding of the adenoviral particles to polyallylamine bisphosphonate (PABT)-modified stainless steel surface via hydrolysable cross-linkers (HC). A family of bifunctional (amine- and thiol-reactive) HC with an average t1/2 of the in-chain ester hydrolysis ranging between 5 and 50 days were used to link the vector with the stent. The vector immobilization procedure is typically carried out within 9 hr&#160;and consists of several steps: 1) incubation of the metal samples in an aqueous solution of PABT (4 hr); 2) deprotection of thiol groups installed in PABT with tris(2-carboxyethyl) phosphine (20 min); 3) expansion of thiol reactive capacity of the metal surface by reacting the samples with polyethyleneimine derivatized with pyridyldithio (PDT) groups (2 hr); 4) conversion of PDT groups to thiols with dithiothreitol (10 min); 5) modification of adenoviruses with HC (1 hr); 6) purification of modified adenoviral particles by size-exclusion column chromatography (15 min) and 7) immobilization of thiol-reactive adenoviral particles on the thiolated steel surface (1 hr). This technique has wide potential applicability beyond stents, by facilitating surface engineering of bioprosthetic devices to enhance their biocompatibility through the substrate-mediated gene delivery to the cells interfacing the implanted foreign material.

These studies report on reversible attachment of adenoviral gene vectors to coatless metal surfaces of stents and model mesh disks. Sustained release of transduction-competent viral particles contingent upon hydrolysis of cross-linkers used for vector immobilization results in a durable site-specific transgene expression in vascular cells and in stented arteries.

Vascular Balloon Injury and Intraluminal Administration in Rat Carotid Artery

The carotid artery balloon injury model in rats has been well established for over two decades. It remains an important method to study the molecular and cellular mechanisms involved in vascular smooth muscle dedifferentiation, neointima formation and vascular remodeling. Male Sprague-Dawley rats are the most frequently employed animals for this model. Female rats are not preferred as female hormones are protective against vascular diseases and thus introduce a variation into this procedure. The left carotid is typically injured with the right carotid serving as a negative control. Left carotid injury is caused by the inflated balloon that denudes the endothelium and distends the vessel wall. Following injury, potential therapeutic strategies such as the use of pharmacological compounds and either gene or shRNA transfer can be evaluated. Typically for gene or shRNA transfer, the injured section of the vessel lumen is locally transduced for 30 min with viral particles encoding either a protein or shRNA for delivery and expression in the injured vessel wall. Neointimal thickening representing proliferative vascular smooth muscle cells usually peaks at 2 weeks after injury. Vessels are mostly harvested at this time point for cellular and molecular analysis of cell signaling pathways as well as gene and protein expression. Vessels can also be harvested at earlier time points to determine the onset of expression and/or activation of a specific protein or pathway, depending on the experimental aims intended. Vessels can be characterized and evaluated using histological staining, immunohistochemistry, protein/mRNA assays, and activity assays. The intact right carotid artery from the same animal is an ideal internal control. Injury-induced changes in molecular and cellular parameters can be evaluated by comparing the injured artery to the internal right control artery. Likewise, therapeutic modalities can be evaluated by comparing the injured and treated artery to the control injured only artery.

This protocol uses a balloon catheter to cause an intraluminal injury on the rat carotid artery and henceforth elicit neointimal hyperplasia. This is a well-established model for studying the mechanisms of vascular remodeling in response to injury. It is also widely used to determine the validity of potential therapeutic approaches.

Quantification of Neurovascular Protection Following Repetitive Hypoxic Preconditioning and Transient Middle Cerebral Artery Occlusion in Mice

Experimental animal models of stroke are invaluable tools for understanding stroke pathology and developing more effective treatment strategies. A 2 week protocol for repetitive hypoxic preconditioning (RHP) induces long-term protection against central nervous system (CNS) injury in a mouse model of focal ischemic stroke. RHP consists of 9 stochastic exposures to hypoxia that vary in both duration (2 or 4 hr) and intensity (8% and 11% O2). RHP reduces infarct volumes, blood-brain barrier (BBB) disruption, and the post-stroke inflammatory response for weeks following the last exposure to hypoxia, suggesting a long-term induction of an endogenous CNS-protective phenotype. The methodology for the dual quantification of infarct volume and BBB disruption is effective in assessing neurovascular protection in mice with RHP or other putative neuroprotectants. Adult male Swiss Webster mice were preconditioned by RHP or duration-equivalent exposures to 21% O2 (i.e. room air). A 60 min transient middle cerebral artery occlusion (tMCAo) was induced 2 weeks following the last hypoxic exposure. Both the occlusion and reperfusion were confirmed by transcranial laser Doppler flowmetry. Twenty-two hr after reperfusion, Evans Blue (EB) was intravenously administered through a tail vein injection. 2 hr later, animals were sacrificed by isoflurane overdose and brain sections were stained with 2,3,5- triphenyltetrazolium chloride (TTC). Infarcts volumes were then quantified. Next, EB was extracted from the tissue over 48 hr to determine BBB disruption after tMCAo. In summary, RHP is a simple protocol that can be replicated, with minimal cost, to induce long-term endogenous neurovascular protection from stroke injury in mice, with the translational potential for other CNS-based and systemic pro-inflammatory disease states.

A Rat Carotid Balloon Injury Model to Test Anti-vascular Remodeling Therapeutics

The rat carotid balloon injury is a well-established surgical model that has been used to study arterial remodeling and vascular cell proliferation. It is also a valuable model system to test, and to evaluate therapeutics and drugs that negate maladaptive remodeling in the vessel. The injury, or barotrauma, in the vessel lumen caused by an inflated balloon via an inserted catheter induces subsequent neointimal growth, often leading to hyperplasia or thickening of the vessel wall that narrows, or obstructs the lumen. The method described here is sufficiently sensitive, and the results can be obtained in relatively short time (2 weeks after the surgery). The efficacy of the drug or therapeutic against the induced-remodeling can be evaluated either by the post-mortem pathological and histomorphological analysis, or by ultrasound sonography in live animals. In addition, this model system has also been used to determine the therapeutic window or the time course of the administered drug. These studies can leadto the development of a better administrative strategy and a better therapeutic outcome. The procedure described here provides a tool for translational studies that bring drug and therapeutic candidates from bench research to clinical applications.

Reproducible Arterial Denudation Injury by Infrarenal Abdominal Aortic Clamping in a Murine Model

Percutaneous vascular interventions uniformly result in arterial denudation injuries that subsequently lead to thrombosis and restenosis. These complications can be attributed to impairments in re-endothelialization within the wound margins. Yet, the cellular and molecular mechanisms of re-endothelialization remain to be defined. While several animal models to study re-endothelialization after arterial denudation are available, few are performed in the mouse because of surgical limitations. This undermines the opportunity to exploit transgenic mouse lines and investigate the contribution of specific genes to the process of re-endothelialization. Here, we present a step-by-step protocol for creating a highly reproducible murine model of arterial denudation injury in the infrarenal abdominal aorta using external vascular clamping. Immunocytochemical staining of injured aortas for fibrinogen and &#946;-catenin demonstrate the exposure of a pro-thrombotic surface and the border of intact endothelium, respectively. The method presented here has the advantages of speed, excellent overall survival rate, and relative technical ease, creating a uniquely practical tool for imposing arterial denudation injury in transgenic mouse models. Using this method, investigators may elucidate the mechanisms of re-endothelialization under normal or pathological conditions.

The Rabbit Model of Accelerated Atherosclerosis: A Methodological Perspective of the Iliac Artery Balloon Injury

Acute coronary syndrome resulting from coronary occlusion following atherosclerotic plaque development and rupture is the leading cause of death in the industrialized world. New Zealand White (NZW) rabbits are widely used as an animal model for the study of atherosclerosis. They develop spontaneous lesions when fed with atherogenic diet; however, this requires long time of 4 - 8 months. To further enhance and accelerate atherogenesis, a combination of atherogenic diet and mechanical endothelial injury is often employed. The presented procedure for inducing atherosclerotic plaques in rabbits uses a balloon catheter to disrupt the endothelium in the left iliac artery of NZW rabbits fed with atherogenic diet. Such mechanical damage caused by the balloon catheter induces a chain of inflammatory reactions initiating neointimal lipid accumulation in a time dependent fashion. Atherosclerotic plaque following balloon injury show neointimal thickening with extensive lipid infiltration, high smooth muscle cell content and presence of macrophage derived foam cells. This technique is simple, reproducible and produces plaque of controlled length within the iliac artery. The whole procedure is completed within 20 - 30 min. The procedure is safe with low mortality and also offers high success in obtaining substantial intimal lesions. The procedure of balloon catheter induced arterial injury results in atherosclerosis within two weeks. This model can be used for investigating the disease pathology, diagnostic imaging and to evaluate new therapeutic strategies.

Carotid cavernous fistula (CCF) is a rare disease caused by abnormal communications between the internal carotid artery (direct fistula) or meningeal branches of the external carotid artery (indirect fistula) and the cavernous sinus (CS). Trauma is the most common cause of CCF. The clinical presentation of CCF is closely related to the venous drainage pattern. Orbital and neuro-ophthalmological symptoms are the most common clinical presentation of CCF with drainage through the superior ophthalmic vein (SOV). Endovascular embolization by arterial or venous approaches is the most common management of CCF. Transvenous embolization using detachable coils and ethylene-vinyl alcohol copolymer (EVOH) is an alternative method for the treatment of CCF. Endovascular embolization offers different options to treat CCF by minimally invasive approach decreasing morbidity and residual fistulas. The purpose of this article is to report our treatment experiences via the inferior petrosal sinus (IPS), and immediate-term outcomes of endovascular embolization of CCF by using detachable coils and EVOH.

Complete and Partial Resuscitative Endovascular Balloon Occlusion of the Aorta for Hemorrhagic Shock

Resuscitative endovascular balloon occlusion of the aorta (REBOA) devices grew out of a military-civilian partnership to develop new capabilities for hemorrhage control. With the advent of purpose-built devices, REBOA has become increasingly common in civilian trauma and acute care settings. Currently available REBOA catheters were designed as complete aortic occlusion devices. However, the therapeutic window for complete aortic occlusion is time-limited due to ischemia-reperfusion injury. The partial procedure allows blood flow past the level of occlusion while maintaining targeted proximal pressure, which has been shown to reduce distal ischemia and adjunctive resuscitation requirements in preclinical studies with prolonged occlusion times as compared to traditional complete occlusion.
pREBOA-PRO is the first catheter designed to enable partial and complete aortic occlusion and is currently in limited market release at seven Level I trauma centers in North America. This paper will focus on procedural considerations for REBOA, including patient selection criteria and a comparison of complete and partial aortic occlusion in a simulator, along with highlighting critical steps to improve clinical outcomes. Additionally, this paper reviews a contrast-enhanced CT scan from a trauma patient that shows distal perfusion after 2 h of partial aortic occlusion using this newly designed catheter and discusses representative results from the limited market release to highlight the profound effect of technological innovation on outcomes in vascular emergencies.

Real-Time Monitoring and Modulation of Blood Pressure in a Rabbit Model of Ischemic Stroke

Control of blood pressure, in terms of both absolute values and its variability, affects outcomes in ischemic stroke patients. However, it remains challenging to identify the mechanisms that lead to poor outcomes or evaluate measures by which these effects can be mitigated because of the prohibitive limitations inherent to human data. In such cases, animal models can be utilized to conduct rigorous and reproducible evaluations of diseases. Here we report refinement of a previously described model of ischemic stroke in rabbits that is augmented with continuous blood pressure recording to assess the impacts of modulation on blood pressure. Under general anesthesia, femoral arteries are exposed through surgical cutdowns to place arterial sheaths bilaterally. Under fluoroscopic visualization and roadmap guidance, a microcatheter is advanced into an artery of the posterior circulation of the brain. An angiogram is performed by injecting the contralateral vertebral artery to confirm occlusion of the target artery. With the occlusive catheter remaining in position for a fixed duration, blood pressure is continuously recorded to allow for tight titration of blood pressure manipulations, whether through mechanical or pharmacological means. At the completion of the occlusion interval, the microcatheter is removed, and the animal is maintained under general anesthesia for a prescribed length of reperfusion. For acute studies, the animal is then euthanized and decapitated. The brain is harvested and processed to measure the infarct volume under light microscopy and further assessed with various histopathological stains or spatial transcriptomic analysis. This protocol provides a reproducible model that can be utilized for more thorough preclinical studies on the effects of blood pressure parameters during ischemic stroke. It also facilitates effective preclinical evaluation of novel neuroprotective interventions that might improve care for ischemic stroke patients.

Continuous arterial blood pressure recording allows the investigation of impacts of various hemodynamic parameters.
