Abstract

幽门螺杆菌是一种主要的人类病原体,感染了全球约一半的人口,并且由于其抗生素耐药性的增加而正在成为严重的健康威胁。它是慢性活动性胃炎、消化性溃疡病和胃癌的病原体,已被国际癌症研究机构列为 I 组致癌物。因此,快速准确地诊断幽门螺杆菌并测定其抗生素耐药性对于有效根除该细菌病原体具有重要意义。目前,幽门螺杆菌的诊断方法主要包括尿素呼气试验(UBT)、抗原试验、血清抗体试验、胃镜检查、快速尿素酶试验(RUT)和细菌培养。其中,前三种检测方法是非侵入性的,这意味着它们很容易进行测试。然而,细菌不能通过这些技术回收;因此,不能进行耐药性测试。最后三种是侵入性检查,但它们成本高昂,需要高技能,并且有可能对患者造成损害。因此,一种无创、快速、同步的幽门螺杆菌检测和耐药性检测方法对于临床上有效根除幽门螺杆菌非常重要。该协议旨在提出一种涉及串测试与定量聚合酶链反应(qPCR)相结合的特定程序,以快速检测幽门螺杆菌感染和抗生素耐药性。与细菌培养不同,该方法可以轻松、快速、无创地诊断幽门螺杆菌感染状态和耐药性。具体来说,我们使用qPCR检测幽门螺杆菌感染的rea以及分别编码对克拉霉素和左氧氟沙星的耐药性的23S rRNA和gyrA基因突变。与常规使用的培养技术相比,该方案提供了一种无创、低成本和省时的技术来检测幽门螺杆菌感染并使用 qPCR 确定其抗生素耐药性。
