Hemihepatectomies oftast utförs efter upprättat strukturer extrahepatisk hilar inom hepatoduodenal ligament, även i godartade tumörer eller levermetastaser. I olika centra 1-5 är parenkymal transection utförs under hela eller selektiva hilar vaskulär ocklusion. Största nackdelarna med dessa förfaranden är komplicerade förberedelserna av hilus med risk för skada kontralaterala strukturer och möjligheten till blödningar från portalen ven sida grenar eller nedsatt perfusion av gallgångarna. <...

Framgångsrik leverresektion krävs det en djupgående anatomiska kunskaper och delikat kirurgisk teknik. Hemihepatectomies är mestadels utförs efter upprättat strukturer extrahepatisk hilar inom hepatoduodenal ligament, även i godartade tumörer eller levermetastaser.<sup&gt; 1-5</sup&gt;. Regionala extrahepatisk lymfkörtlar är en onkologisk standard i hilar cholangiocarcinoma, intrahepatisk cholangio porösa cancer och hepatocellulär cancer, medan lymfkörtelmetastaser i levern hilus hos patienter med levermetastaser är sällan ockulta. Största nackdelarna med dessa förfaranden är komplicerade förberedelserna av hilus med risk för skada kontralaterala strukturer och möjligheten till blödningar från portalen ven sida grenar eller nedsatt perfusion av gallgångarna. Vi utvecklade en teknik för rätt hemihepatectomy eller resektion av den vänstra laterala segment med intrahepatisk transection av BLOMSTJÄLK som lämnar hepatoduodenal ligamentet helt orörd.<sup&gt; 6</sup&gt; Men om intraoperativ visualisering eller palpation av ligament är misstänkt för tumör infiltration eller lymfkörtelmetastas bör hilus undersökas och en lymfkörtlar utförs.
