<p class="jove_content"> De nauwkeurige kwantificering van perifere neuropathie is van belang om te definiëren op risicopatiënten, anticiperen op achteruitgang, en beoordelen van nieuwe therapieën. Conventionele methoden te bepalen neurologische tekorten en electrofysiologie en kwantitatieve sensorische tests kwantificeert functionele veranderingen op te sporen neuropathie. Echter, de eerste schade lijkt te zijn aan de kleine vezels en toch deze tests in de eerste plaats te beoordelen grote vezels dysfunctie en hebben een beperkte capaciteit om regeneratie en reparatie aan te tonen. De enige technieken die een rechtstreeks onderzoek van de niet-gemyeliniseerde zenuwvezel schade en herstel mogelijk zijn Sural zenuw biopsie met elektronenmicroscopie en huid-punch biopsie. Echter, beide zijn invasieve procedures en vereisen langdurige laboratorium procedures en aanzienlijke expertise. Hoornvlies confocale microscopie is een niet-invasieve klinische techniek die <em>in-vivo</em> beeldvorming van het hoornvlies zenuwvezels biedt. We hebben aangetoond dat vroege schade aan de zenuwen, die het verlies van intra zenuwvezels vooraf in de huid biopten samen met de gelaagdheid van neuropathische ernst en reparatie na pancreas-transplantatie bij diabetespatiënten. We hebben ook aangetoond schade aan de zenuwen in idiopathische kleine vezels neuropathie en de ziekte van Fabry. </p>
