bronchial thermoplasty:  a novel therapeutic approach to severe asthma

기관지 thermoplasty 과도한기도 평활근을 줄이기 위해 정확하게 제어 방식으로기도 벽에 열에너지를 제공 심각한 영구적인 천식을위한 비 약물 절차입니다. 기도 평활근을 줄이면함으로써 천식 발작의 빈도를 감소, 수축에 대한기도의 능력을 감소시킵니다. 기관지 thermoplasty는 Alair 시스템에 의해 제공됩니다 세 외래 수술 방문, 약 삼주 간격 예정된 각 수행됩니다. 첫 번째 절차는 오른쪽 하단 엽의 항공을 취급, 두 번째는 왼쪽 하단 엽의기도를 취급하고 세 번째이자 마지막 절차는 모두 상위 엽 (叶)의기도를 취급합니다. 세 절차가 수행됩니다 후에는 기관지 thermoplasty 처리가 완료되었습니다. 기관지 thermoplasty는 적당한 약물로 안정을 환자와 함께 bronchoscopy 동안 수행됩니다. mainstem 기관지 오른쪽 중간 엽의 예외로 지름 3 사이에 10mm로 말초 액세스 가능한 모든 항공은 bronchoscopic 시각화에 따라 처리됩니다. 장치의 연속이 아닌 중복 정품 인증은 앞에서 설명한 것처럼기도의 길이를 따라 근위하고, 체계적으로기도부터 말초에서기도로 이동, 사용됩니다. 개념 간단하지만, 기관지 thermoplasty의 실제 실행은 매우 복잡하고 단일 엽의 치료를위한 절차적 기간은 종종 상당히 더 이상 일상 bronchoscopy 동안 발생보다. 따라서, 기관지 thermoplasty는 복잡한 interventional bronchoscopy 간주되어야하며 경험 bronchoscopist위한 것입니다. 최적의 환자 관리는 이러한 복잡하고 긴 기간 bronchoscopic 절차에서 중요합니다. 이 문서는주의 환자 선택의 중요성, 환자 준비, 그 기관지 thermoplasty 안전하게 수행하기 위해 환자 관리, 절차 기간, 수술 후 관리 및 후속에 대해 설명합니다. 기관지 thermoplasty는 지속적인 천식 관리를 제공하고 심한 천식 환자의 삶의 천식 관련 품질을 개선하여 천식 관리 약품을 보완 예정입니다. 또한, 기관지 thermoplasty는 직장, 학교 및 천식으로 인해 다른 일상 생활에서 심각한 손실 exacerbations (천식 발작) 호흡기 증상에 대한 응급실 방문, 시간을 줄이기 위해 증명되었습니다.
