Die hier vorgestellte Methode ist ein Standard-Sequenzierung Methode angewendet, um Tropismus Tests. Die klinische Anwendung von HIV-Envelope-V3-Loop-Sequenzierung der viralen Tropismus vorherzusagen hat bis spät in Grenzen. Diese Methode hat sich gezeigt, in retrospektiven Analysen der Maraviroc (ViiV Healthcare) klinische Studien, um mindestens ebenso in der Lage, virale Tropismus im Vergleich zu anderen validierten Tests klinisch verwendeten vorherzusagen. Es gibt viele Vorteile für die Durchführung von genotypischen Analyse der V3-Schleife für Tropismus Vorhersage. In erster Linie können diese Prozedur an jedem Standort mit operativen Sequenzierung Ausrüstung durchgeführt werden, eine starke Erhöhung der Erreichbarkeit und der Verringerung der Durchlaufzeit von Tropismus Tests. Im Vergleich dazu ist die phänotypische Verbesserte Empfindlichkeit Trofile Assay (ESTA) (Monogram Biosciences), die als Goldstandard für Tropismus Tests gedient hat, in einem Zentrum in Südkalifornien durchgeführt. Weitere Vorteile sind die Forderung nach weniger Ausgangsmaterial und eine mindestens erforderliche Viruslast im Blut von 500copies/mL. Wie gut sind die operativen Kosten für den Betrieb der genotypische Test relativ niedrig im Vergleich zu denen eines phänotypischen Assays. Die Nutzung der Software ReCall insbesondere entfällt die Notwendigkeit für manuelle Sequenz zu überprüfen, die zu einer arbeitsintensiven Teil des Genotyps analysiert werden neigt. Die hier vorgestellte Methode ist recht einfach, ohne besondere Schritt überwiegt eine andere Bedeutung. Wir empfehlen läuft PCR und Sequenzierung in dreifacher Ausfertigung an die Chancen für den Fang von Arten Minderheit innerhalb der viralen Population einer Stichprobe zu erhöhen. Repliziert mehr als drei durchgeführt werden kann, dreifach aber wir haben festgestellt, dass sowohl effizient und zuverlässig. Wir empfehlen die Verwendung der One-Step Reverse-Transkriptase-PCR Kit (Qiagen), die beide RT-PCR und ersten Runde PCR führt in die gleiche Reaktion bei gleichzeitig hoher Sensitivität und Spezifität.

Hintergrund: Vor dem Empfang eines Medikaments aus CCR5-Antagonisten-Klasse in der HIV-Therapie, muss sich ein Patient eine HIV-Tropismus-Test, um zu bestätigen, dass seine virale Population der CCR5 Korezeptor verwendet für die zelluläre Eintrag, und nicht eine Alternative Korezeptor. Ein Ansatz zur Tropismus Tests ist es, die Reihenfolge der V3-Region des HIV-Envelope, die mit dem Korezeptor interagiert untersuchen. Methoden: Viral RNA wird aus Blutplasma gewonnen. Die V3-Region ist in dreifacher Ausfertigung mit verschachtelten Reverse-Transkriptase-PCR amplifiziert. Die Verstärkungen werden dann sequenziert und analysiert die Verwendung der Software RE_Call. Die Sequenzen werden dann zu einer bioinformatischen Algorithmen wie geno2pheno auf virale Tropismus aus der V3-Region schließen eingereicht. Die Sequenzen werden geschlossen, um nicht-R5 werden, wenn ihre geno2pheno False Positive-Rate unterschritten 5,75%. Wenn einer der drei Sequenzen aus einer Probe wird entnommen, um Nicht-R5 werden, wird der Patient wahrscheinlich nicht zu einer CCR5-Antagonisten reagieren.
