I UTMD, bioaktiva molekyler, såsom negativt laddade plasmid-DNA vektorer kodar en gen, läggs till katjoniska skal av lipid mikrobubblor kontrastmedel 7-9. Hos möss dessa vektor som bär mikrobubblor kan ges intravenöst eller direkt till vänster kammare i hjärtat. I större djur de kan också infunderas genom en intrakoronar kateter. Den efterföljande leverans från cirkulationen till ett målorgan sker genom akustisk kavitation vid en resonansfrekvens av mikrobubblor. Det verkar troligt att den mekaniska energi som genereras av mikrobubblor förstörelsen resulterar i övergående porbildning i eller mellan endotelcellerna i mikrocirkulation i målregionen 10. Som en följd av detta sonoporation effekt är transfektion effektivitet i och över endotelceller förstärkas och transgen-kodning vektorer sätts in på den omgivande vävnaden. Plasmid DNA som kvarblir i cirkulationen snabbt bryts ned av nukleaser i blodet, vilket ytterligare minskar sannolikheten för leverans till icke-sonicated vävnader och leder till mycket specifika mål-organ transfektion.
