1. Pré-procédure de préparation <ol><li> Avant d&#39;effectuer la procédure, discutez des risques et des avantages de la procédure avec le patient, et d&#39;obtenir un formulaire de consentement signé. Ajuster la dose du patient antagonistes de la vitamine K pour atteindre un ratio international normalisé de 2,0 à 3,0. Continuer traitement antiarythmique jusqu&#39;au moment de la procédure. </li><li> Soit la veille ou le jour de l&#39;intervention, effectuer une échocardiographie transœsophagienne (ETO) pour documenter l&#39;absence de thrombus au sein de l&#39;AAL, évaluer les caractéristiques et le type de la LAA, et pour déterminer la taille appropriée WATCHMAN dispositif d&#39;implantation (voir le 2012 article JoVE par Mobius-Winkler et ses collègues pour plus d&#39;informations) 22. </li><li> Effectuez l&#39;ablation par cathéter combiné et WATCHMAN Fermeture LAA périphérique procédure de placement dans un laboratoire d&#39;électrophysiologie avec l&#39;équipement approprié de diagnostic et d&#39;imagerie pour la procédure. </li><li> Avec le patient en décubitus dorsal, administer anesthésie selon des protocoles institutionnels. </li></ol> 2. Ablation par cathéter <ol><li> Assurez-vous que le système d&#39;enregistrement électrophysiologiques (Bard Inc, Lowell, MA) utilise les paramètres de filtre de 100 à 500 kHz et une amplification du signal de 5000. </li><li> Une fois que le patient est totalement anesthésié, l&#39;administration de 20 ml de lidocaïne à 1% comme anesthésique local dans la région de l&#39;aine. Immédiatement, propre et de préparation de la région inguinale pour la ponction. </li><li> Localisez la veine fémorale, et insérer un veineuse 8 Français (8F) gaine. Introduire un cathéter quadripolaire dans le sinus coronaire à des fins de stimulation. Ensuite, insérez un fil-guide de long, et retirer la gaine. Introduire une gaine veineuse standard d&#39;accès transseptale. </li><li> Retirer le fil guide de long, et d&#39;insérer un transseptal RF Brockenbrough aiguille (Baylis Medical Company Inc, Montréal, Québec, Canada) avec une gaine orientable 12.5F-outer-diameter/9.5F-inner-diameter (Manche, Bard, Lowell, MA ) et d&#39;effectuer une transseptale ponctualitére. </li><li> Après avoir traversé le septum interauriculaire, positionner un cathéter queue de cochon dans le centre de l&#39;atrium gauche et effectuez une rotation 3D angiographie. </li><li> Le cathéter d&#39;ablation des veines pulmonaires (PVAC: Medtronic / Ablation Frontiers, Inc, Carlsbad, CA) est un 9F, over-the-wire, circulaire, cartographie et decapolar cathéter d&#39;ablation avec une matrice de 25 mm de diamètre à l&#39;extrémité distale. Le PVAC a dix électrodes sur un cadre en nitinol qui permettent la cartographie, l&#39;ablation et la vérification de l&#39;isolement des veines pulmonaires. Insérez un fil-guide 0,032 pouces dans le PVAC. </li><li> Ensuite, insérer le système dans son ensemble dans la gaine veineuse. Les informations recueillies par l&#39;acétate de polyvinyle est affichée sur un moniteur afin de générer une image 3D de coeur du patient avec l&#39;emplacement du cathéter affichée. </li><li> Administrer un bolus d&#39;héparine pour empêcher la coagulation 10.000IE à la suite de la procédure. Placez le fil dans l&#39;une des branches de la veine pulmonaire, puis avancez le PVAC dans l&#39;antre des veines pulmonaires. </li><li> Utilisez fluoroscopic orientation, électrocardiogrammes, et 3D signal électrique d&#39;imagerie (ESI) de reconstruction d&#39;évaluer la position du PVAC et de l&#39;ajuster pour obtenir le meilleur contact possible avec toutes les électrodes. </li><li> Une fois qu&#39;une position de cathéter satisfaisant est obtenu, commencer par radiofréquence (RF)-ablation en sélectionnant les paires d&#39;électrodes appropriées pour la fourniture de l&#39;énergie RF. </li><li> Fixer le générateur de régler la puissance pour atteindre une température cible de 60 ° C pour chaque électrode dans les paires sélectionnées. Si la température ne dépasse pas 50 ° C dans les 15 secondes, arrêtez l&#39;application, ajustez la position des électrodes et recommencer. </li><li> Déplacer et faire tourner le cathéter PVAC-pour créer des lésions circonférentielles veine pulmonaire. </li><li> Répétez cette procédure pour toutes les veines pulmonaires. </li><li> Après crmanger lésions circonférentielles pour toutes les veines pulmonaires, rétablir le rythme sinusal par cardioversion courant continu (DCCV). </li><li> Confirmer isolement en vérifiant l&#39;absence de potentiels locaux (entrée et sortie de bloc) avec l&#39;intérieur de chaque cartographie PVAC veine associée à la stimulation des manoeuvres du sinus coronaire. </li><li> Pour des potentiels résiduels sur les paires d&#39;électrodes isolées, l&#39;ablation seule cible sur les paires adjacentes et que de faciliter l&#39;obtention température cible. </li></ol> 3. L&#39;implantation de l&#39;appareil WATCHMAN <ol><li> Une fois la procédure d&#39;ablation par cathéter est terminé, l&#39;appareil WATCHMAN est implanté. </li><li> Avancer un 0,035 &quot;rigide fil de guidage (p.ex. Amplatz super rigide 260 cm) dans la position de PVAC et le fil de guidage dans la veine pulmonaire supérieure gauche. Retirer le PVAC, tout en maintenant la position du fil de guidage. </li><li> Si l&#39;ablation n&#39;a pas été effectuée avant l&#39;implantation, l&#39;administration d&#39;héparine à 100 UI / kg de poids corporel pour obtenir un temps de coagulation activée of 200-300 sec. Vérifiez le niveau de temps de coagulation activée afin de s&#39;assurer qu&#39;il est toujours 200-250 sec. Continuer à surveiller toutes les 30 min ce que nécessaire. </li><li> Retirer le système d&#39;accès WATCHMAN et le dilatateur de l&#39;emballage dans des conditions stériles. Attentivement pour déceler tout dommage. </li><li> Remplir une seringue de 60 cc avec une solution saline et rincer l&#39;orifice latéral de la gaine d&#39;accès GARDIEN. Utilisez le sérum restant pour vider le dilatateur. Insérer le dilatateur dans la gaine d&#39;accès GARDIEN. </li><li> Avancer la gaine d&#39;accès WATCHMAN et le dilatateur sur le guide dans l&#39;oreillette gauche. Que la gaine d&#39;accès WATCHMAN se rapproche du centre de l&#39;oreillette gauche, tenir le dilatateur et la gaine d&#39;accès avancer WATCHMAN à la position initiale de l&#39;oreillette gauche seulement. </li><li> Retirer le dilatateur et le fil guide. Avant de serrer la valve hémostatique, permettent de revenir saigner à minimiser les risques d&#39;introduction d&#39;air. Rincer gaine d&#39;accès WATCHMAN avec une solution saline. </li><li> Serrer le robinet de la gaine d&#39;accès. </li><li> Pour confermes dimensions de l&#39;AAL, utilisez TEE pour mesurer les dimensions maximales AAL en 4 à 0 °, 45 °, 90 ° et 135 °. </li><li> Rincer le cathéter queue de cochon avec du sérum physiologique et le faire avancer sur le fil et à travers la gaine d&#39;accès GARDIEN. Ensuite, enlevez le fil guide et connecter une seringue avec un produit de contraste à la queue de cochon. </li><li> Tout en affichant TEE, tourner la gaine d&#39;accès WATCHMAN antihoraire pour aligner la gaine de plus antérieure et faire avancer la queue de cochon dans la position désirée dans la partie distale de l&#39;AAL. </li><li> Une fois le cathéter en queue de cochon a atteint la LAA, l&#39;angiographie à obtenir un droit antérieur oblique (RAO) de 20-30 °, 20-30 ° caudal. Effectuez ensuite TEE avec un minimum d&#39;un balayage 0-135 °. Ceci est très important lorsque la gaine est avancé près du mur ou au sommet de la LAA et tout en faisant progresser plus distalement dans toute l&#39;anatomie. </li><li> Soigneusement avancer la gaine d&#39;accès sur le cathéter queue de cochon dans le lobe supérieur plus: en angiographie c&#39;est la situat LAA-lobeed vers 2 heures à une droite oblique antérieure (RAO) de 20-30 °, 20-30 ° caudale, en ETO c&#39;est le droit le plus LAA-lobe à 135 °. </li><li> Sous guidage fluoroscopique, faire avancer la gaine d&#39;accès WATCHMAN à la profondeur désirée dans le LAA basée sur la bande de marqueur correspondant à la taille appropriée. </li><li> Une fois la queue de cochon est aussi distal que possible, injecter le produit de contraste. </li><li> Sélectionnez le format approprié WATCHMAN dispositif basé sur les dimensions maximal LAA ostium et dimensionnement 22. </li><li> Dans des conditions stériles, retirez le périphérique WATCHMAN de l&#39;emballage pour déceler tout dommage. Pour confirmer le périphérique est connecté au fil d&#39;âme, ouvrir la valve hémostatique et rétracter les périphériques environ 1 cm. </li><li> Aligner l&#39;extrémité distale du dispositif de la bande de marquage. Le dispositif WATCHMAN ne doivent pas dépasser. </li><li> Fixez une grosse seringue de 60 cc contenant une solution saline. Rincer le système plusieurs fois pour enlever l&#39;air. Ensuite, immerger la pointe du cathéter d&#39;administration danssaline et appuyez dessus pour enlever les bulles. </li><li> Revérifier la position de la gaine d&#39;accès par l&#39;angiographie. Desserrer la soupape de gaine d&#39;accès et retirer lentement le cathéter pigtail de la gaine. Permettre à la gaine de retour saigner. </li><li> Afin d&#39;éviter l&#39;introduction d&#39;air, injecter une solution saline à travers le port de rinçage de sorte qu&#39;il s&#39;écoule à partir du cathéter de délivrance, puis, introduire le système de dispositif de gardien. </li><li> Sous contrôle scopique, lentement avancer le cathéter de délivrance dans la gaine d&#39;accès jusqu&#39;à ce que la bande de marquage la plus distale sur les lignes d&#39;accès de gaine avec la bande de marquage sur le cathéter de délivrance et de stabiliser le cathéter de délivrance. </li><li> Rétracter la gaine d&#39;accès et enclenchez-le sur le cathéter de pose. En utilisant la fluoroscopie, réaffirmer la position de l&#39;extrémité du cathéter de livraison. Ne pas faire avancer la gaine d&#39;accès une fois le système de livraison a été cassé en elle. </li><li> Si un repositionnement est nécessaire, retirez le système de livraison, réinsérez le cathéter queue de cochon et de faire avancer la gaine d&#39;accès dans le propéra position. </li><li> Une fois que le système de distribution est positionné, desserrer la vanne sur le cathéter GARDIEN. Observez l&#39;extrémité distale du dispositif afin de s&#39;assurer qu&#39;aucun progrès vers l&#39;avant ou par rapport à un repositionnement ostium se produit. </li><li> Puis, pour déployer le dispositif, maintenir le bouton de déploiement stationnaire et lentement rétracter l&#39;ensemble de cathéter d&#39;accès gaine / envoi d&#39;un mouvement lent stable sur une période de 3-5 sec. </li><li> Retirer l&#39;ensemble cathéter d&#39;accès gaine / livraison à quelques centimètres de l&#39;appareil afin de l&#39;aligner avec la LAA, en laissant le fil d&#39;âme attachée. </li><li> Une fois que le dispositif a été déployé, injecter contraste. Puis, en utilisant la fluoroscopie et l&#39;ETO, confirmer la position, Ancre, Taille et Seal (PASS) les critères de libération de périphériques ont été réunis comme suit: </li><li> Poste: Confirmation du dispositif est correctement positionné en veillant à ce que le plan de diamètre maximal du dispositif s&#39;étend sur l&#39;orifice LAA et est au niveau ou juste en aval de l&#39;orifice de la LAA. </li> &lt;li&gt; Anchor: Confirmez le dispositif est ancré en place, par le retrait de l&#39;ensemble de cathéter gaine d&#39;accès / de livraison: 1 - 2 cm de la surface de l&#39;appareil, puis doucement rétracter et en relâchant le bouton de déploiement. La LAA et le dispositif doit se déplacer à l&#39;unisson. </li><li> Taille: vérifier que le dispositif est dimensionné de manière appropriée en mesurant le plan du plus grand diamètre à l&#39;aide du dispositif de T dans les 4 types de vues 0 °, 45 °, 90 °, et 135 °, assurant la douille filetée est visible. L&#39;appareil doit être 80-92% de la taille originale. </li><li> Joint: Doppler couleur, de sorte que tous les lobes distaux de dispositif sont scellées. Doppler couleur ne doit pas être détectés à proximité de l&#39;appareil. Si il ya un fossé ou un jet autour de l&#39;appareil qui est supérieure à 5 mm, l&#39;appareil doit être repositionné ou entièrement repris et remplacés. </li><li> Si tous les critères de libération sont remplies, déplacez la gaine d&#39;accès / de cathéter jusqu&#39;à la surface de l&#39;appareil et tournerle bouton dans le sens antihoraire 3-5 déploiement tour de rôle pour le libérer. </li><li> Après sa libération, effectuer une angiographie avec produit de contraste afin de documenter que le dispositif est toujours en place. Puis, en utilisant TEE, vérifiez la taille et le joint. Retirer l&#39;ensemble gaine de l&#39;oreillette gauche. </li><li> Si l&#39;appareil est trop distale ou proximale de la LAA ou les critères de périphériques ne sont pas remplies, il peut être nécessaire de récupérer partiellement ou totalement l&#39;appareil (pour plus de détails s&#39;il vous plaît voir l&#39;article 2012 JoVE par Möbius-Winkler et ses collègues) 22. </li></ol> 4. Post-opératoire <ol><li> Une fois mise en place correcte de l&#39;appareil a été confirmée, retirer la gaine. La veine fémorale doit ensuite être comprimé pendant quelques heures et le patient doit être surveillé attentivement pendant au moins 6 heures avec la pression artérielle et la fréquence cardiaque. </li><li> Vérifiez hématome et / ou saignement à intervalles réguliers, conformément aux directives institutionnelles. Le patient doit être administré une prophylaxie antibiotique par ee American Heart Association lignes directrices. REMARQUE: l&#39;héparine procédure post n&#39;est pas recommandée. </li><li> Une fois que le patient est éveillé, effectuer un examen neurologique. </li><li> Le patient doit rester sur la warfarine et l&#39;aspirine 81 mg pour un minimum de 45 jours après la procédure d&#39;implantation (interne Normalized Ratio = 2.0-3.0). </li><li> Le patient doit être hospitalisé pendant la nuit et libéré le lendemain. Une échocardiographie transthoracique (ETT) ou une radiographie pulmonaire peut être effectuée pour confirmer l&#39;absence d&#39;épanchement péricardique. </li><li> A 45 jours, évaluer l&#39;emplacement des dispositifs WATCHMAN par ETO. Cessation de la warfarine est à la discrétion du médecin, si la LAA est complètement fermée et thrombus de l&#39;appareil a été écartée. Si le débit est noté autour de l&#39;appareil supérieure à 5 mm, il faudrait envisager de garder le patient sous warfarine jusqu&#39;à ce qu&#39;il ait diminué à moins de 5 mm. </li><li> Les patients qui cessent warfarine devrait commencer 75 mg par jour pendant 6 mois post-implantation et continuez à prendreSpirin quotidienne indéfiniment. </li><li> Prescrire la prophylaxie endocardite approprié pendant 6 mois après l&#39;implantation. Poursuivre la prophylaxie endocardite delà de 6 mois est à la discrétion du médecin. </li></ol>

catheter ablation in combination with left atrial appendage closure for atrial fibrillation

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia, affecting millions of individuals worldwide 1-3. The rapid, irregular, and disordered electrical activity in the atria gives rise to palpitations, fatigue, dyspnea, chest pain and dizziness with or without syncope 4, 5. Patients with AF have a five-fold higher risk of stroke 6.
Oral anticoagulation (OAC) with warfarin is commonly used for stroke prevention in patients with AF and has been shown to reduce the risk of stroke by 64% 7. Warfarin therapy has several major disadvantages, however, including bleeding, non-tolerance, interactions with other medications and foods, non-compliance and a narrow therapeutic range 8-11. These issues, together with poor appreciation of the risk-benefit ratio, unawareness of guidelines, or absence of an OAC monitoring outpatient clinic may explain why only 30-60% of patients with AF are prescribed this drug 8.
The problems associated with warfarin, combined with the limited efficacy and/or serious side effects associated with other medications used for AF 12,13, highlight the need for effective non-pharmacological approaches to treatment. One such approach is catheter ablation (CA), a procedure in which a radiofrequency electrical current is applied to regions of the heart to create small ablation lesions that electrically isolate potential AF triggers 4. CA is a well-established treatment for AF symptoms 14, 15, that may also decrease the risk of stroke. Recent data showed a significant decrease in the relative risk of stroke and transient ischemic attack events among patients who underwent ablation compared with those undergoing antiarrhythmic drug therapy 16.
Since the left atrial appendage (LAA) is the source of thrombi in more than 90% of patients with non-valvular atrial fibrillation 17, another approach to stroke prevention is to physically block clots from exiting the LAA. One method for occluding the LAA is via percutaneous placement of the WATCHMAN LAA closure device. The WATCHMAN device resembles a small parachute. It consists of a nitinol frame covered by fabric polyethyl terephthalate that prevents emboli, but not blood, from exiting during the healing process. Fixation anchors around the perimeter secure the device in the LAA (Figure 1). To date, the WATCHMAN is the only implanted percutaneous device for which a randomized clinical trial has been reported. In this study, implantation of the WATCHMAN was found to be at least as effective as warfarin in preventing stroke (all-causes) and death (all-causes) 18. This device received the Conformit&eacute; Europ&eacute;enne (CE) mark for use in the European Union for warfarin eligible patients and in those who have a contraindication to anticoagulation therapy 19.
Given the proven effectiveness of CA to alleviate AF symptoms and the promising data with regard to reduction of thromboembolic events with both CA and WATCHMAN implantation, combining the two procedures is hoped to further reduce the incidence of stroke in high-risk patients while simultaneously relieving symptoms. The combined procedure may eventually enable patients to undergo implantation of the WATCHMAN device without subsequent warfarin treatment, since the CA procedure itself reduces thromboembolic events. This would present an avenue of treatment previously unavailable to patients ineligible for warfarin treatment because of recurrent bleeding 20 or other warfarin-associated problems.
The combined procedure is performed under general anesthesia with biplane fluoroscopy and TEE guidance. Catheter ablation is followed by implantation of the WATCHMAN LAA closure device. Data from a non-randomized trial with 10 patients demonstrates that this procedure can be safely performed in patients with a CHADS2 score of greater than 1 21. Further studies to examine the effectiveness of the combined procedure in reducing symptoms from AF and associated stroke are therefore warranted.

In a preliminary study of WATCHMAN implantation in 10 patients that had a high risk of stroke and bleeding, 9 of the patients received the device after a catheter ablation. The data from this study is published in the Netherlands Heart Journal, and is presented here with permission 21. All patients included in the study were fully informed about the procedure and signed a consent form. The study was approved by the hospital's ethics committee and all procedures were performed in accordance with the hospital's ethics standards and the Helsinki Declaration of 1975 (revised in 2008).
The characteristics of these patients are summarized in Table 1. All patients had documented AF with a high stroke risk (CHADS2 &gt;1) and all but one of the patients had history of stroke. LAA closure was indicated as an alternative to OAC for the following reasons: stroke during OAC therapy (3/10), severe bleeding during OAC therapy (2/10) and patient preference (5/10). Prior to the procedure all patients were taking a vitamin K antagonist. The median CHADS2 score for this group was 3, with a median CHA2DS2-VASc score of 3.5 and a median HAS-BLED score of 1.5. Prior to the procedure, all patients underwent TEE to exclude LAA thrombus and to rule out structural cardiac abnormalities. Among patients, the average maximum LAA diameter was 20.2&plusmn;2.2 mm with an average depth of 27.9&plusmn;5.4 mm. 40% of the patients had a multi-lobular atrial appendage.
Table 2 outlines procedural characteristics. The WATCHMAN LAA closure device was successfully placed in all 10 patients. Based on patient anatomy determined during preliminary TEE, three different device sizes were used. The median total procedure time was 104 min (range 75-167 min), including a median 56 min (range 38-137 min) for LAA occlusion. During the procedure, an asymptomatic catheter thrombus occurred in one patient. The thrombus was aspirated and the patient received an extra bolus of heparin. Prolonged femoral vein bleeding in this patient necessitated a blood transfusion and extended hospitalization. All other patients were discharged from the hospital on the day following the procedure (Table 2). OAC was continued at least until TEE was performed at 45 days, at which point the device position and closure rate was assessed.
The findings at the 45-day patient follow-up are summarized Table 3. Successful device implantation was defined as long-term sealing of the LAA as measured by TEE 45 days after the procedure without major adverse events. Residual flow (&lt; 5 mm) around the device was allowed after the procedure (according to the PROTECT-AF study). Importantly, no thromboembolic cerebral events occurred. Minimal residual flow was observed in 3 patients. TEE revealed asymptomatic embolization of the device in one patient, and the device was percutaneously retrieved via the femoral vein. Thrombus was not seen on the device in any cases. One patient required a redo-ablation procedure 3 months after the initial procedure, in which pulmonary vein isolation was successful. The implanted LAA device was not affected by and did not interfere with the procedure. Three of the 10 patients were able to discontinue warfarin treatment at 45 days. In the PROTECT-AF18 and Continues Access Registry29 device embolization occurred in 3 patients out of the 1,002 patients implanted.
Figure 2 shows a TEE screen capture of the LAA in one example patient in which no residual flow is observed 45 days post-procedure. In contrast, flow around the device, as evidenced by color Doppler, is seen for another patient (Figure 3).
