evaluation of respiratory muscle activation using respiratory motor control assessment (rmca) in individuals with chronic spinal cord injury

במהלך נשימה, הפעלה של שרירי נשימה מתואמת על ידי הזנה משולבת מהמוח, גזע המוח, וחוט השדרה. כאשר התיאום הזה מופר על ידי פגיעה בחוט השדרה (SCI), שליטה על שרירי נשימה מעוצבבים מתחת לרמת הפגיעה נפגעת 1,2 מוביל לתפקוד לקוי של שרירים נשימה וסיבוכים ריאתי. תנאים אלה הם בין הגורמים המובילים למוות בחולים עם SCI 3. בדיקות תפקוד ריאות רגילות שהערכת תפקוד מוטורי נשימה כוללות תוצאות spirometrical ומקסימום נשימה לחץ: קיבולת חיונית בכפייה (FVC), נפח נשיפה מאולץ בשנייה אחת (1 FEV), לחץ שאיפה מקסימלי (PI מקסימום) ולחץ מקסימאלי נשיפה (PE מקסימום) 4,5. ערכים אלה מספקים מדידות עקיפות של ביצועי שריר הנשימה 6. בפרקטיקה ומחקר קליניים, electromyography פני השטח (sEMG) נרשם משרירי הנשימהניתן להשתמש בו כדי להעריך את התפקוד מוטורי נשימה ולעזור לאבחן התוקפת פתולוגיה. עם זאת, שוני במשרעת sEMG מעכב את המאמצים לפתח מדדים אובייקטיביים וישירים של תפקוד מוטורי 6 דרכי הנשימה. בהתבסס על גישת sEMG רב שריר לאפיין שליטה מוטורית של שרירי גפיים 7, המכונים מדד התגובה מרצון (VRI) 8, פיתחנו כלי אנליטי לאפיון בקרת נשימה מנוע ישירות מsEMG הנתונים שנרשמו מרירי נשימה מרובים במהלך ההתנדבות משימות בדרכי הנשימה. יש לנו מכונים בקרת מנוע הערכת נשימה זו (RMCA) 9. שיטת ניתוח וקטור זה מכמת את הסכום וההפצה של פעילות על פני שרירים ומציגה אותו בצורה של מדד שמתייחסת באיזו מידה דומה לפלט sEMG תוך בדיקת נושא שמקבוצה של נבדקים בריאים (לא פצועים). ערך המדד וכתוצאה הוכח יש תוקף פנים גבוה, רגישותוסגוליות 9-11. אנחנו הראו בעבר כי תוצאות 9 RMCA לתאם באופן משמעותי עם רמות של SCI מידות ותפקוד ריאות. אנו מביאים כאן את השיטה כדי להשוות דפוסים שלאחר חוט השדרה פגיעה בדרכי הנשימה מרובות שרירים הפעלת כמותית לאלו של אנשים בריאים.
