Terwijl de niet-specifieke gadolinium-gebaseerde contrastmiddelen geven informatie over de omvang en perfusie van de extracellulaire ruimte, macrofaag beeldvorming door USPIO-versterkte MR maakt een nauwkeurige anatomische lokalisatie en een kwalitatieve evaluatie van macrofaaginfiltratie in besmette synovium zonder de noodzaak van weefselmonsters 9. Vanwege hun hoge gevoeligheid, kunnen USPIO demonstreren zelfs subtiele cellen in de vroege fasen van infectie zit. Onder succesvolle antibiotica, zoals de ziekt...

Magnetic resonance imaging (MRI) wordt beschouwd als de beeldvormende modaliteit van keuze voor het aantonen van infectieuze synovitis vanwege de hoge ruimtelijke resolutie en contrast zacht weefsel. Veranderingen signaal waargenomen op MRI bij artritis lage T1 en T2 hoog signaal als gevolg van de verhoogde aanwezigheid van extracellulaire watergehalte, en een duidelijke verbetering na gadolinium gebaseerde-contrastmiddel administratie, in overeenstemming met de histologisch bevindingen van verhoogde vasculariteit gevolg van vaatverwijding en angiogenese 1. Niettemin MRI dikwijls niet in oplossing van infecties tonen tijdens antibioticumtherapie persistent verbetering kan worden waargenomen in de gewrichten van patiënten met klinisch en biologisch genezen septische artritis toe aan persistentie van uitgebreide extracellulaire ruimte (granulatieweefsel, fibrotische litteken) 2. Naast extracellulaire veranderingen ontstekingscellen inclusief een intense rekrutering van macrofagen massaal infiltreren ininfecteerd synovia. Macrofagen spelen een belangrijke rol in zowel de acute als de chronische fase van bacteriële infectie 3. Ze induceren ontstekingsreactie nodig pathogene micro-organismen te roeien en coördineren ontsteking resolutie, zoals macrofaag-geïnduceerde ontsteking blijft zolang necrotisch weefsel niet verwijderd, waardoor reparatie te beginnen 4. Zo kan de vermindering van de infiltratie van macrofagen in besmette synovia na efficiënte therapie een betrouwbare indicator van de resolutie van de infectie. Cellulaire beeldvorming is in staat om specifieke cellulaire infiltratie in pathologisch weefsel aan te tonen. Specifieke macrofaag MR beeldvorming met behulp van gerichte contrastmiddel is op grote schaal onderzocht en heeft de mogelijkheid om gezamenlijke ontsteking of infectie 5-7 demonstreren aangetoond. Na intraveneuze injectie, worden dergelijke gerichte contrastmiddelen zoals USPIO (ultrakleine superparamagnetische ijzeroxide) deeltjes opgenomen door fagocytische cellen zoals macrofagen en,vanwege hun magnetische eigenschappen induceren signaalveranderingen in weefsels presentatie macrofaaginfiltratie 8. De longitudinale follow-up van deze MR signaal verandert therapie zorgt voor een in vivo niet-invasieve evaluatie van macrofaag infiltratie, en een vermindering van USPIO geladen-macrofagen gerelateerde signaal veranderingen zouden therapie succes aantonen. Het doel van dit rapport is om te presenteren hoe de macrofaag MRI uit te voeren om niet-invasief monitoren septische artritis door het aantonen van de vermindering van de infiltratie van macrofagen in de synovia.

in vivo macrophage imaging using mr targeted contrast agent for longitudinal evaluation of septic arthritis

Macrofagen zijn de sleutel-cellen in de initiatie, de ontwikkeling en de regulatie van de inflammatoire respons op bacteriële infectie. Macrofagen intensief en steeds gerekruteerd septische voegen van de vroege fasen van infectie en de infiltratie wordt verondersteld eenmaal regressie efficiënte verwijdering van pathogenen verkregen. De mogelijkheid om te identificeren in vivo macrofaag activiteit in een geïnfecteerde gewricht kan daarom voorzien in twee belangrijke toepassingen: vroege opsporing van acute synovitis en controle van de behandeling. In vivo niet-invasieve detectie van macrofagen kunnen worden uitgevoerd met behulp van magnetische resonantie beeldvorming ijzer nanodeeltjes zoals ultrakleine superparamagnetische ijzeroxide (USPIO). Na intravasculaire of intra-articulaire toediening, worden USPIO specifiek phagocytized door geactiveerde macrofagen en door hun magnetische eigenschappen induceren signaalveranderingen in weefsels presenteren macrofaag infiltratie. Een quantitative evaluatie van het infiltreren is toegepast omdat het gebied met signaalverlies (aantal donkere pixels) waargenomen op gradiënt echo MR beelden na injectie deeltjes is gecorreleerd met de hoeveelheid ijzer in het weefsel en dekt dus het aantal USPIO geladen cellen. We presenteren hier een protocol om macrofaag beeldvorming met behulp USPIO-versterkte MR beeldvorming in een diermodel van septische artritis, waardoor uitvoering van een voorlopige en longitudinale in vivo niet-invasieve evaluatie van macrofagen infiltratie en een beoordeling van de therapie optreden.

Op onversterkte beelden, het synovium van geïnfecteerde knieën presenteert een diffuse zwelling van tussensignaal dat niet onderscheiden van de omringende zachte weefsel, terwijl femur en patella verschijnen vanaf signaalstructuur (figuur 1).
