De succesvolle inductie van invasieve overgangs-en plaveiselcelcarcinoom blaascarcinoom in menselijke MUC1.Tg muizen biedt een preklinisch model voor Immunotherapie ontwikkeling. Blaaskanker is de vierde meest voorkomende vorm van kanker en de achtste belangrijkste oorzaak van sterfgevallen door kanker in de Amerikaanse mannen. Gezien de zeer hoge herhaling ondanks de beschikbare behandelingen, is er duidelijk een behoefte aan nieuwe, meer efficiënte therapie voor blaaskanker. Uitbreiden van nieuwe immuuntherapie voor Bladder kanker is een mogelijke aanpak die belofte kan houden voor de uitbreiding van de ziektevrije overleving. Historisch gezien, is BCG is de enige effectieve immuuntherapie bij blaaskanker. Het werkingsmechanisme wordt verondersteld om de niet-specifieke inductie van een T-helper 1 (Th1) Type immuunrespons door toenemende niveaus van interleukine-2 (IL-2) en interferon gamma (IFN-γ) 4 omvatten. Cellulaire of Th1 immuniteit is essentieel bij kanker immunotherapie humorale of Th2 is immuniteit nooit aangetoond effectief tegen vaste tumoren zijn, met uitzondering van antilichamen tegen groeifactorreceptoren 7. In een poging om te verbeteren op de voordelen van BCG monotherapie werd IFN-α 2B/BCG combinatie immunotherapie geëvalueerd in een fase II klinische studie met povere resultaten 8. Een alternatieve benadering van immunotherapie voor blaaskanker kan zijn tumor-geassocieerde antigenen (TAA), de identificatie waarvan kankerimmunotherapie specifieker 7 gemaakt targeten </sup&gt;. Een dergelijke TAA 1 mucine (MUC1), een celoppervlak glycoproteïne tot overexpressie gebracht in vele epitheliale kanker zoals blaas-, borst-, long-en pancreaskanker 9,10. De expressie en modificatie van MUC1 wordt ook aanzienlijk tijdens carcinogenese gewijzigd, zodat underglycosylation bloot antigene sequenties van aminozuren genoemd variabel aantal tandem herhalingen (VNTR) de peptidekern. Terwijl MUC1 is een zelf-molecuul, worden deze immunodominante regio VNTR normaal blootgesteld door uitgebreide glycosylatie en waardoor ze worden gezien door het immuunsysteem als vreemd 11,12. Cytotoxische T-lymfocyten (CTL) die specifiek epitopen herkennen MUC1 geïsoleerd uit tumor-drainerende lymfklieren van borstkankerpatiënten 13, evenals het bloed en beenmerg van myeloom 14,15, waardoor MUC1 een potentieel doelwit voor een cellulaire immuunrespons. De immunodominante VNTRs van de underglycosylated vorm van MUC1 worden herkend door CTL&#39;s, wat resulteert in de vernietiging van tumorcellen 16-19. Inheemse cellulaire en / of humorale immuunresponsen op kanker MUC1 zijn echter niet sterk genoeg om tumoren te elimineren. Aan de reeds bestaande zwakke immuunrespons tegen MUC1 vergroten, kunnen synthetische immuundominante peptiden door vaccinatie worden ingevoerd om een CTL respons sterk genoeg te zijn van klinisch voordeel 18,20 genereren. Een liposomale MUC1 vaccin is reeds aangetoond dat overleving in longkankerpatiënten 21,22 verhogen genereren CTLs kunnen doden MUC1-positieve tumorcellen, en produceren een Th1-gepolariseerde cytokine response 23,24. Met een hoge mate van MUC1 expressie 9,11,25, blaaskanker is een logische kandidaat voor het testen van MUC1-gerichte immunotherapie 26,27. Verder MUC1 heeft potentieel als voorspellende factor bij blaaskanker 28, MUC1 expressie in TCC is significant geassocieerd met stadium en graad, en metastatische TCCis aangetoond blijven drukken MUC1 29. Om het potentiële nut van MUC1-gerichte immunotherapie bij blaaskanker evalueren we een immune intact menselijk MUC1 (hMUC1)-transgene expressie (MUC1.Tg) muismodel van blaaskanker congenic de C57BL / 6 achtergrond 30 ontwikkeld. Human MUC1 wordt uitgedrukt als een zelf-eiwit onder de controle van zijn eigen promoter, waardoor een weefsel expressie patroon verlopen waargenomen in personen 30,31. De muizen werden geïnduceerd met de bekende blaas carcinogeen N-butyl-N-(4-hydroxybutyl) nitrosamine (OH-BBN) 32, en vervolgens de resulterende tumoren werden beoordeeld op hMUC1 expressie en tumor soort en kwaliteit. Om het effect van het carcinogeen op Th1/Th2 cytokine niveaus gedurende tumorontwikkeling beoordelen, werden periodiek serummonsters verzameld voor multiplex analyse. Muizen werden vervolgens behandeld met een MUC1-gericht peptide vaccin, en serum cytokine en immuunreacties waren evaluated door multiplex fluorometrische microbead immunoassay en ELISpot.

induction of invasive transitional cell bladder carcinoma in immune intact human muc1 transgenic mice:  a model for immunotherapy development

Een preklinisch model van invasieve blaaskanker werd in humane mucine 1 (MUC1) transgene (MUC1.Tg) muizen voor de evaluatie immunotherapie en / of cytotoxische chemotherapie. Blaaskanker, C57BL / 6 muizen (MUC1.Tg en wild type) werden oraal behandeld met de kankerverwekkende N-butyl-N-(4-hydroxybutyl) nitrosamine (OH-BBN) en 3,0 mg / dag, 5 dagen / week induceren gedurende 12 weken. Om de effecten van OH-BBN op serum cytokine profiel gedurende tumorontwikkeling te beoordelen, werd volbloed verzameld via submandibulaire bloedt voorafgaand aan de behandeling en om de vier weken. Daarnaast werden een MUC1-gericht peptide vaccin en placebo toegediend aan groepen muizen per week gedurende acht weken. Multiplex fluorometrische microbeads immunoanalyses van serumcytokinen gedurende tumorontwikkeling en na vaccinatie werden uitgevoerd. Bij beëindiging interferon gamma (IFN-γ) / interleukine-4 (IL-4) ELISPOT-analyse voor MUC1 specifieke T-cel-immuunrespons en histopathologische evaluatie van tumortypeen rang werden uitgevoerd. De resultaten toonden aan dat: (1) de incidentie van blaaskanker zowel MUC1.Tg en wild type muizen was 67%, (2) overgangsepitheelcarcinoom carcinomen (TCC) ontwikkeld in een 2:1 verhouding vergeleken met plaveiselcelcarcinoom (SCC) , (3) inflammatoire cytokines toe met de tijd gedurende tumorontwikkeling en (4) de toediening van het peptide vaccin induceert een Th1-gepolariseerde serum cytokine profiel en een MUC1 specifieke T-cel respons. Alle tumoren in MUC1.Tg muizen waren positief voor MUC1 expressie, en de helft van alle tumoren in MUC1.Tg en wild-type muizen waren invasief. Tot slot, met behulp van een team aanpak door de coördinatie van de inspanningen van farmacologen, immunologen, pathologen en moleculair biologen, we hebben een immuunsysteem intact transgeen muismodel van blaaskanker die hMUC1 uitdrukt ontwikkeld.

De preklinische beoordeling van de gevolgen van nieuwe immuuntherapie en combinaties in blaaskanker vereist de ontwikkeling van een geschikt diermodel. In onze transgeen muismodel inductie met het chemische carcinogeen-BBN OH resulteerde in een hoge incidentie van blaaskanker voornamelijk TCC met enkele SCC, die vergelijkbaar is met blaaskanker bij mensen.
