Både antiglobulintest og hæmolytisk assay er semikvantitative, idet den højeste serum fortynding betegnes, hvor testen er stadig positiv. Antiglobulintest og hæmolytisk assay er robuste assays, der rutinemæssigt anvendes i diagnostik. Da disse analyser er semikvantitativ og afhængig af oplevelsen af ​​den tekniker, der udfører analysen, de er ikke egnet til at studere subtile forskelle i komplement aktivering på RBC, efter behov, når at evaluere effekten af ​​komplement-hæmmere. Derfor har vi udviklet to kvantitative analyser til at bestemme komplement-aktivering af (auto-) antistoffer til RBC, som vi vil beskrive i dette dokument. Først udviklede vi en analyse for at måle nedfald af aktivering fragmenter af komplement C3 og C4 på RBC (figur 1A). I dette assay humane bromelain-behandlede type 0 RBC inkuberet med varmeinaktiveret patientserum (anti-RBC-antistof kilder), frisk AB-serum (komplement kilde) og anti-C5 monoklonalt antistof (Eculizumab). I løbet af denne incubation vil C3 og C4 aflejring, hvis patienten indeholder serum supplement aktiverende anti-RBC-antistoffer. For at forhindre RBC ved nedstrøms komplementaktivering tilsættes et blokerende anti-C5-monoklonalt antistof. Dernæst C3 og C4 aflejring på RBC påvises ved FACS under anvendelse af fluorescens-mærkede monoklonale antistoffer eller Fab-fragmenter, der reagerer med C3 og C4, hhv. Gating på enkelt RBC er vigtigt at sikre pålideligheden af ​​resultaterne. Fordelene ved denne teknik omfatter, at en lille mængde af patient-materiale er påkrævet, komplementaktivering på et tidligt stadium af kaskaden vurderes og fremgangsmåden er reproducerbare og kvantitative. En yderligere fordel ved at kigge på både C3 og C4 er, at der kan skelnes mellem den klassiske og lectin pathway aktivering (både C3 og C4 deposition) og den alternative vej aktivering (kun C3 deposition). Anvendelsen af ​​bromelain-behandlede RBC stedet for ubehandlede RBC&#39;er øger følsomheden af ​​det somsige. Det andet assay er baseret på den aktuelt anvendte hæmolytisk assay (figur 1B).
