Staat vandaag of the art behandeling van osteosarcoom (OS), een primaire bottumor, omvat een combinatie van neo adjuvante chemotherapie en chirurgie. Fotodynamische therapie (PDT), een vrij nieuwe therapie tegen kanker, maakt gebruik van licht van een specifieke golflengte voor excitatie van een photosensiTizer (PS), dat zich ophoopt in de tumorcellen na injectie in de bloedbaan. Laserlicht excitatie van de PS genereert zuurstofradicalen in aanwezigheid van zuurstof cytotoxische reactie in tumorcellen en celdood induceren. Naast deze primaire mechanisme, twee extra PDT opgeroepen biologische processen bijdragen aan verminderde tumorgroei: PDT veroorzaakt vasoconstrictie en bloedpropvorming van de tumor microvasculature en bijgevolg lokale hypoxie en anoxie in de tumor, wat leidt tot tumor infarct. Tenslotte PDT gewond en stervende tumorcellen leiden tot een lokale immuunreactie, een vrij unieke eigenschap van PDT. Dit houdt het complementsysteem en de activering van antigeen-presenterende dendritische cellen 8. Zo worden voorwaarden geschapen voor de presentatie van tumorantigenen met daaropvolgende activatie van lymfoïde cellen, leidend tot tumorspecifieke immuniteit. Tot dusver is PDT gebruikt om verschillende soorten wekedelentumoren en hyperpl behandelenazië, zoals actinische keratose, Barrett-slokdarm, endobronchiale tumoren, blaaskanker, basaalcelcarcinoom en palliatieve behandeling van hoofd-halskanker 2. De behandeling is bekend dat lokale, grootschalige necrose met weinig bijwerkingen, en heeft dus de potentie om tumorweefsel selectief roeien. Ondanks deze voordelen, de toepassing van PDT blijft technisch veeleisender dan de toediening van chemotherapeutische geneesmiddelen. Met het oog op maximale efficiëntie, de PS concentratie, blootstelling aan licht en de totale licht energie-overdracht te realiseren moeten worden geoptimaliseerd. Dit kan in vivo experimenten, maar vanwege het relatief grote aantal parameters die moeten worden geoptimaliseerd, is het efficiënter om eerst te bepalen optimale omstandigheden in vitro. In de hieronder beschreven experimenten hebben we de in vitro werkzaamheid van PDT getest met PS 5,10,15,20-tetrakis (meta-hydroxyfenyl) chlOrin, afgekort mTHPC (Figuur 1A). mTHPC is de werkzame stof in het geneesmiddel foscan, die momenteel wordt gebruikt in de kliniek voor palliatieve behandeling van hoofd-en halskanker. De laatste jaren is het moeilijk een efficiëntere therapie tegen kanker met minder systemische bijwerkingen geweest. Daarom fotodynamische therapie is een nieuwe benadering voor een tumor selectieve behandeling. Fotodynamische therapie (PDT) die gebruik maakt van een niet-toxisch fotosensibilisator (PS), dat na activering met licht van een bepaalde golflengte in aanwezigheid van zuurstof, genereert zuurstof radicalen die een cytotoxische reactie 1 wekken. Ondanks de goedkeuring van bijna twintig jaar geleden door de FDA, is PDT tegenwoordig alleen gebruikt voor een beperkt aantal soorten kanker (huid, blaas) en nononcological aandoeningen (psoriasis, actinische keratose) 2 te behandelen. Het grote voordeel van het gebruik van PDT is de mogelijkheid om een ​​lokale behandeling, die systemische bijwerkingen voorkomt voeren. Bovendien kan de behandeling van tumoren bij gevoelige locaties (bijvoorbeeld rond zenuwen of bloedvaten). De opname van de PS mTHPC werd onderzocht met een spectrofotometer en fototoxiciteit werd uitgelokt met laserlicht excitatie van mTHPC bij 652 nm tot celdood beoordeeld met een WST-1 test en door het tellen van de overlevende cellen induceren. De gevestigde technieken stellen ons in staat om de optimale PDT instellingen voor toekomstige studies in diermodellen definiëren.
