Proteiner er den raskest voksende klassen av terapeutiske midler 6, og den raske utvidelsen av bioterapeutiske rørledninger har fokusert økende oppmerksomhet på utfordringer knyttet til utvikling og bruk av protein narkotika. En unik faktor stammer fra det faktum at, i en sunn og fungerende immunsystem blir alle ekstracellulære proteiner samplet av antigen-presenterende celler (APCer). Når internalisert av APC, er et protein spaltes i små peptid fragmenter, og antatte immunogene segmenter er lastet inn i sporet i klasse II store histocompatibility komplekse proteiner (MHC II). De peptid-MHC II-kompleksene blir deretter vist på APC overflate, og sann immunogene peptider, betegnet T-celle-epitoper, danner ternære II MHC-peptid-T-celle-reseptor-komplekser med beslektede CD4 T-celleoverflatereseptorer 7.
