Hoog-risico humaan papillomavirus (HR-HPV) is goed voor ongeveer 5% van alle kanker wereldwijd 1. De incidentie van HPV-geassocieerde orofarynxcarcinoom is toegenomen in de afgelopen decennia, vooral bij mannen. rnascope for in situ detection of transcriptionally active human papillomavirus in head and neck squamous cell carcinoma

De &#39;gouden standaard&#39; voor oncogene HPV-detectie is de demonstratie van transcriptionally actief hoog-risico HPV in tumorweefsel. Echter, detectie van E6/E7 mRNA door middel van kwantitatieve omgekeerde transcriptie polymerase-kettingreactie (qRT-PCR) vereist RNA-extractie die vernietigt het tumorweefsel context van cruciaal belang voor morfologische correlatie en is moeilijk in de dagelijkse klinische praktijk worden aangenomen. Het recent ontwikkelde RNA in situ hybridisatie technologie, RNAscope, maakt directe visualisatie van RNA in formaline gefixeerde, in paraffine ingebedde (FFPE) weefsel molecuul gevoeligheid en enkele cel resolutie, die zeer gevoelig en specifiek in situ analyse van elke RNA biomarker kunnen in routine klinische monsters. De RNAscope HPV-test werd ontworpen om de E6/E7 mRNA zeven hoog risico HPV-genotypen (HPV16, 18, 31, 33, 35, 52 en 58) met een pool van genotype-specifieke probes detecteren. Het heeft een uitstekende gevoeligheid aangetoonden specificiteit tegen de huidige &#39;gouden standaard&#39; methode voor het opsporen E6/E7 mRNA door qRT-PCR. HPV-status bepaald door RNAscope is sterk voorspellende van klinische resultaten bij orofaryngeale kankerpatiënten.

RNAscope HPV test workflow De RNAscope test heeft een sterk gestroomlijnde werkstroom die vergelijkbaar IHC (figuur 1). Het bestaat uit vier belangrijke stappen: voorbehandelingen, hybridisatie signaal versterkingen, en detectie. Het maakt gebruik van een unieke probe-ontwerp strategie die high fidelity signaalversterking 5 zorgt. In de RNAscope HPV test worden de zeven HR-HPV probe sets samengevoegd, alle erkende door dezelfde signaalamplificatiesysteem gekoppeld aan mierikswortel peroxidase (HRP). Specifieke hybridisatie signalen worden gedetecteerd als bruine precipitaten gevormd door HRP gekatalyseerde chromogene reactie met DAB als substraat, dat gemakkelijk kan worden gevisualiseerd door standaard helderveld microscopie. Vertegenwoordiger kleuring voor HPV-detectie Figuur 2 toont bijvoorbeeld beelden van gebrandschilderd FFPE delen van hoofd en nek plaveiselcelcarcinoom. Bij de HPV-positieve geval (Figure 2A), de HR-HPV-probes gedetecteerd sterke puntvormige signalen specifiek de tumorcellen. De UBC probe gedetecteerd talrijke punctata cytoplasmatische signalen in zowel tumorcellen en stromale cellen (Figuur 2B). Het bacterieel gen dapB probe toonde een schone achtergrond (figuur 2C). Bij de HPV-negatieve geval zowel de HR-HPV-sonde en de dapB sonde gedetecteerd geen signalen (figuren 2D en 2F), terwijl sterke signalen werden gedetecteerd met de UBC sonde (figuur 2E). In deze assay, UBC dient als positieve controle weefsel RNA kwaliteit en dapB beoordeelt negatieve controle voor achtergrond signalen. De score voor de HPV-status bepaling impliceert onderzoek van alle drie dia&#39;s voor elk geval.
