Wijzigingen en probleemoplossing: IFN-γ ELISPOT-tests werden gebruikt om celgemedieerde immuunresponsen tegen een verscheidenheid van microbiële pathogenen, waaronder humaan immunodeficiëntievirus type 1 (HIV-1) 24,25, hepatitis C virus (HCV) 26 onderzocht, 27, en Mycobacterium tuberculosis 28,29, om er maar een paar te noemen. Hier beschrijven we de ontwikkeling van een IFN-γ ELISpot test om cellulaire immuniteit tegen te meten, met de hoop van het definiëren van correlate...

Eerste uitgevoerd in 1989, wordt UCBT steeds meer gebruikt als onderdeel van de behandeling van verschillende neoplastische en nonneoplastic bloed bij kinderen 1. VZV is een cytopatisch menselijk alfaherpesvirus die twee verschillende ziekten, varicella (na primaire infectie) en herpes zoster (na reactivering) veroorzaakt. Na primaire infectie, VZV blijft gedurende de gehele levensduur van de gastheer beschut binnen sensorische zenuwen van de dorsale wortel ganglia. Een van de meest bedreigende infectieuze complicaties na UCBT geassocieerd met VZV 2-4. In ons klinisch centrum, in afwezigheid van VZV profylaxe, de cumulatieve incidentie van VZV ziekte VZV ziekte op 3 jaar postUCBT was 46% 2. Bij deze patiënten is de novo infectie met of reactivering van VZV vaak geassocieerd met viscerale verspreiding onder het centrale zenuwstelsel, de longen en de lever 5-7. Dientengevolge, acyclovir, valacyclovir of famciclovir profylaxe gewoonlijk toegediend aan UBCT ontvangers 8,9. Echter, deze behandelingsstrategie geen rekening gehouden met de beschermende potentieel van VZV specifieke T-lymfocyten en de kinetica van reconstitutie van VZV specifieke T cel responsen. Mogelijke problemen in verband met het toenemende gebruik van lange termijn antiherpetic profylaxe onder a) patiënt overbehandeling; b) de ontwikkeling van resistentie tegen antivirale middelen 10,11; en c) bijzondere waardevermindering van VZV specifieke immuunreconstitutie 12,13. Omdat detectie van functionele VZV-specifieke T-lymfocyten correleert met de aanwezigheid van lange termijn bescherming tegen VZV infectie en betere klinische uitkomst 4,14,15, monitoring cel-gemedieerde immuunrespons gericht tegen VZV in de periode na transplantatie kan leiden tot een rationeler gebruik van antivirale behandeling doordat artsen aan patiënten die baat hebben bij VZV onderscheiden profylaxe van degenen bij wie het immuunsysteem is in staat de controle VZV replicatie 4,13. De IFN-γ ELISPOT wordt veel gebruikt voor het bewaken celgemedieerde immuunresponsen in verschillende experimentele systemen en klinische voorwaarden. Spots gegenereerd na de splitsing van een chromogeen substraat, het genereren van een zichtbare en stabiel precipitaat op de plaats van de reactie. Elke individuele vlek daardoor vertegenwoordigt de voetafdruk van een individuele cytokine producerende cellen. IFN-γ ELISPOT meet niet alleen het vermogen van afzonderlijke cellen ex vivo IFN-γ produceren in reactie op in vitro stimulatie met verwant antigeen, maar ook een schatting van de frequentie van reagerende cellen in een bepaalde celpopulatie 16,17. Naast de hoge gevoeligheid, IFN-γ ELISPOT is eenvoudig uit te voeren, waarbij de toepassing mogelijk in het kader van gepersonaliseerde klinische protocollen die als leidraad initiatie of beëindiging van antivirale behandeling. De onderstaande procedure describ beschrevenes een ELISPOT-assay die specifiek is ontworpen voor het opsporen en meten van de productie van IFN-γ door perifere mononucleaire cellen na in vitro stimulatie met VZV afgeleide antigenen.

Varicella zoster-virus (VZV) is een belangrijke oorzaak van morbiditeit en mortaliteit na transplantatie van navelstrengbloed (UCBT). Daarom is antiherpetic profylaxe systematisch toegediend aan pediatrische UCBT ontvangers complicaties met VZV-infectie te voorkomen, maar er is geen sterk bewijs gebaseerde consensus dat de optimale duur bepaalt. Omdat T-cel gemedieerde immuniteit is verantwoordelijk voor de controle van VZV-infectie, de beoordeling van de reconstructie van VZV-specifieke T-cel responsen na UCBT kan aanwijzingen verschaffen over de vraag of profylaxe moet worden gehandhaafd of kan worden gestaakt. Hiervoor werd een specifiek VZV gamma interferon (IFN-γ) enzym-gekoppelde immunospot (ELISPOT) assay ontwikkeld IFN-γ productie karakteriseren door T-lymfocyten als reactie op in vitro stimulatie met bestraalde levend verzwakt VZV-vaccin. Deze test geeft een snelle, reproduceerbare en gevoelige meting van VZV specifieke cell gemedieerde immuniteit geschikt voor het bewaken van de reconstructie van VZV specifieke immuniteit in een klinische setting en beoordelen immuunrespons te VZV antigenen.

De IFN-γ ELISPOT protocol hierboven beschreven werd ontwikkeld en geoptimaliseerd in ons laboratorium om de omvang en de kwaliteit van celgemedieerde immuunresponsen tegen VZV 4 meten. Diverse bronnen van VZV-antigeen kan worden gebruikt ter bevordering stap. Deze omvatten: a) de handel verkrijgbaar detergent geïnactiveerd uittreksels uit VZV geïnfecteerde Vero-cellen 18; b) pools van overlappende synthetische peptiden uit specifieke VZV gecodeerde eiwitten, waaronder IE63 15 en ORF4 ...
