Array CGH zal niet evenwichtig herschikkingen of ploidie afwijkingen zoals triploïdie detecteren. Bovendien kunnen lage niveau mozaïek onevenwichtigheden niet worden gedetecteerd. Het is vele jaren bekend dat deleties of extra exemplaren van chromosomale segmenten veroorzaken verstandelijke beperking, dysmorfie en congentical misvormingen, en in sommige gevallen leiden tot genetische syndromen 1. Echter, de enige technologie beschikbaar tot het midden van de jaren 2000 voor het genoom-brede detectie van deze veranderingen was G-gestreepte chromosoom analyse, die een resolutie van rond 5-10Mb heeft, afhankelijk van de regio en de aard van de onbalans, en dat kan niet detecteren abnormale chromosomen waar materiaal is vervangen door een gebied van een ander chromosoom met hetzelfde bandenpatroon. Bijkomende cytogenetische technieken zoals fluorescentie in situ hybridisatie en multiplex ligatie-specifieke probe versterking zijn beschikbaar voor de ondervraging van specifieke loci geweest, in de gevallen van vermoedelijke specifieke syndromatische onbalans, maar het was niet tot de invoering van de array-comparatieve genomische hybridisatie (array CGH) in routine klinische diagnostische service 2-5 dat genoom-brede detectie van copy number varianten (CNV) en werd het mogelijk op een sterk verhoogde resolutie (meestal rond de 120kb). Klinische servicewerkzaamheden naast studies hebben aangetoond dat CNV sommige gebieden zijn wijdverspreid in de normale populatie 6-7, terwijl andere CNV eerder detecteerbaar zijn verbonden neurodisabilities zoals autisme en epilepsie 8-11. De in dit document beschreven protocol wordt gebruikt in onze Britse National Health Service (NHS) klinisch diagnostisch laboratorium; we gebruiken roman hybridisatie strategieën, batch testen en robotica om de kosten in deze staat gefinancierde dienst te minimaliseren. Vóór het protocol hieronder dient hoogwaardige DNA worden geëxtraheerd uit het geschikte uitgangsmateriaal, gewoonlijk bloed, gekweekte cellen of weefselmonsters. Spectrofotometrie kan worden gebruikt om te worden gemeten (moet&gt; 50 ng / ul) en controleer 260: 280 absorptie verhoudingen (moeten be 1.8-2.0). Gel electroforese kan worden gebruikt om te controleren dat het DNA van hoog molecuulgewicht zonder significante afbraak. Dit protocol is ontworpen voor een hogere doorvoersnelheid laboratoria die zijn labelen 96 monsters per run met behulp van geautomatiseerde liquid handling robotica. Het kan echter worden aangepast voor lagere doorvoer labs zonder automatisering.

Array CGH for the detection of genomic copy number variants has replaced G-banded karyotype analysis. This paper describes the technology and its application in a clinical diagnostic service laboratory. DNA extracted from a patient&#8217;s sample (blood, saliva or other tissue types) is labeled with a fluorochrome (either cyanine 5 or cyanine 3). A reference DNA sample is labeled with the opposite fluorochrome. There follows a cleanup step to remove unincorporated nucleotides before the labeled DNAs are mixed and resuspended in a hybridization buffer and applied to an array comprising ~60,000 oligonucleotide probes from loci across the genome, with high probe density in clinically important areas. Following hybridization, the arrays are washed, then scanned and the resulting images are analyzed to measure the red and green fluorescence for each probe. Software is used to assess the quality of each probe measurement, calculate the ratio of red to green fluorescence and detect potential copy number variants.
