Vi simulert potensielle kliniske anvendelser av NIRF-guidede BCS gjennom bruk av bryst-formet fantomer med integrerte tumor-simulere slutninger. Intraoperativ tumor lokalisering, NIRF-guidet tumorreseksjon, evaluering av omfanget av operasjonen, og postoperativ vurdering av kirurgiske marginer ble alle funnet mulig ved hjelp av en skredder bygge NIRF kamerasystem. Ikke-invasiv deteksjon av fluorescerende tumor-simulerende inneslutninger var bare mulig for inneslutninger som er plassert i fantom vev i en dybde på 2 cm e...

Brystbevarende kirurgi (BCS) etterfulgt av strålebehandling er standard behandling for brystkreftpasienter med T 1 -T to brystkreft 1,2. Unøyaktigheter i intraoperativ vurdering av omfanget av operasjonen resultat i positive kirurgiske marginer i 20 til 40% av pasientene som gjennomgikk BCS, nødvendiggjør ytterligere kirurgisk inngrep eller strålebehandling 3,4,5. Selv om omfattende reseksjon av tilstøtende sunn brystvev kan redusere hyppigheten av positive kirurgiske marginer, vil dette også hemme kosmetisk resultat og øke komorbiditet 6,7. Nye teknikker er derfor nødvendig at gi intraoperativ tilbakemelding på plasseringen av den primære tumor og omfanget av operasjonen. Optisk avbildning, spesielt nær-infrarøde fluorescens (NIRF) avbildning, kan redusere hyppigheten av positive kirurgiske marginer følgende BCS ved å gi kirurgen med et verktøy for pre-og intraoperativ tumor lokalisering i rEAL-tiden. Nylig rapporterte vår gruppe på den første in-human studie av tumor-målrettet fluorescens bildebehandling i eggstokkene kreftpasienter, viser gjennomførbarheten av denne teknikken til å oppdage primære svulster og intraperitoneal metastaser med høy følsomhet åtte. Før du går videre til kliniske studier hos brystkreftpasienter, men muligheten for ulike tumor målrettet NIRF bildebehandlingsprogrammer i BCS kan allerede bli evaluert preklinisk bruker fantomer. Følgende forskningsprotokoll beskriver bruk av NIRF bildebehandling i vev-simulere bryst fantomer som inneholder fluorescerende tumor-simulere slutninger ni. Fantomene gi en rimelig og allsidig verktøy for å simulere pre-og intraoperativ tumor lokalisering, real-time NIRF-guidet tumorreseksjon, vurdering av kirurgisk margin status, og påvisning av restsykdom. Den geléaktige fantomene har elastiske egenskaper som ligner på menneskelig vev og kan kuttes ved hjelp av konvensjonelle surgical instrumenter. Under simulert kirurgisk prosedyre, blir styrt av kirurgen følbar informasjon (i tilfelle av inneslutninger palpable), og visuell inspeksjon av det operative området. I tillegg er NIRF avbildning anvendt for å gi kirurgen med sanntids tilbakemelding intraoperativ på omfanget av operasjonen. Det bør understrekes at NIRF bildebehandling krever bruk av fluorescerende fargestoffer. Ideelt sett bør fluorescerende fargestoffer anvendes som emittere fotoner i det nær-infrarøde spektralområdet (650-900 nm) for å minimalisere absorpsjon og spredning av fotoner med molekyler fysiologisk rikelig i vev (for eksempel hemoglobin, lipider, elastin, kollagen, og vann) 10,11. Videre autofluorescens (dvs. den indre fluorescens aktivitet i vev på grunn av biokjemiske reaksjoner i levende celler) blir minimalisert i den nær-infrarøde spektralområdet, noe som resulterer i optimale tumor-til-bakgrunnsforhold 11. Ved å konjugere NIRF fargestoffer til tumor-TARGEted moieties (f.eks monoklonale antistoffer), kan fås målrettet levering av fluorescerende fargestoffer for intraoperativ bildebehandlingsprogrammer. Som det menneskelige øyet er ufølsomt å lyse i nær-infrarøde spektrale området, er en svært følsom kamera enhet kreves for NIRF bildebehandling. Flere NIRF bildebehandlingssystemer for intraoperativ bruk har blitt utviklet så langt 12. I denne studien brukte vi en egendefinert bygge NIRF bildesystem som ble utviklet for intraoperativ søknad i samarbeid med det tekniske universitetet i München. Systemet gjør det mulig for samtidig kjøp av fargebilder og fluorescens bilder. Unøyaktigheter i intraoperativ tumor lokalisering og evaluering av kirurgisk margin status resultat i suboptimal utfallet av brystbevarende kirurgi (BCS). Optisk avbildning, spesielt nær-infrarøde fluorescens (NIRF) avbildning, kan redusere hyppigheten av positive kirurgiske marginer følgende BCS ved å gi kirurgen med et verktøy for pre-og intraoperativ tumor lokalisering i sanntid. I denne studien, er potensialet i NIRF-guidet BCS evalueres med vev-simulere bryst fantomer på grunn av standardisering og opplæringsformål. Videre fantomene inneholder hemoglobin og Intralipid for å etterligne absorpsjon og spredning av fotoner henholdsvis oppretting ensartede optiske egenskaper som ligner på humant brystvev. Den største ulempen med NIRF bildebehandling er begrenset inntrengningsdybde av fotoner når forplanter seg gjennom vev, som hindrer (invasiv) avbildning av dyptliggende svulster med epi-belysning strategier.

Resultater fra denne studien har tidligere blitt rapportert andre steder ni. Våre data viser at NIRF bildebehandling kan brukes til å oppdage fluorescerende tumor-simulere slutninger i vev-simulere bryst fantomer, simulere NIRF-guidet brystbevarende kirurgi hos brystkreftpasienter. Near-infrared fluorescence (NIRF) imaging may improve therapeutic outcome of breast cancer surgery by enabling intraoperative tumor localization and evaluation of surgical margin status.
