Fecale transplantaties is aangetoond dat een effectieve en veilige behandeling van recidiverende CDI zijn. We hebben een standaard protocol voor FMT geschetst via colonoscopie en inclusief onze resultaten voor de 22 patiënten die de procedure voor recidiverende CDI onderging. Onze resultaten tot nu toe zijn vergelijkbaar met gepubliceerde gegevens voor terugkerende ziekte met slechts één patiënt terugkerende een maand na de FMT. Fecale transplantaties voor de behandeling van pseumembranous colitis werden eerst beschreven in 1958 1. Echter meldingen van inname van faeces voor de behandeling van voedselvergiftiging en ernstige diarree dateren al vierde eeuw China 2. Fecale microbiota transplantatie (FMT) is ook wel in de literatuur &quot;fecale bacteriotherapy&quot;, &quot;human probiotische infusie&quot;, &quot;kruk transplantatie&quot;, &quot;intestinale microbiome herstel&quot; en &quot;fecale transfer&quot;. Het gaat verzamelen van ontlasting van een gezonde, vooraf gescreend donor en met een bereide suspensie in het maagdarmkanaal van de ontvanger via een maagsonde, esophagogastricduodenoscopy, colonoscopie of klysma 3. Veel gevallenreeks hebben gemeld de effectiviteit van toediening via NGT en klysma, maar meer recente studies hebben aangetoond dat toediening van donor ontlasting via een colonoscopie veilig en zeer effectief 4.Terwijl andere methoden, zoals neussonde administratie zijn gemeld, de levering van de donor ontlasting via colonoscopie heeft minder directe bijwerkingen (bijv opgeblazen gevoel en misselijkheid) 4 en we hebben ontdekt dat patiënten meer te accepteren van het concept van FMT wanneer het wordt uitgevoerd door colonoscopie. Een van de therapeutische voordelen van FMT via colonoscopie is de vermindering colon biomassa door de darm lavage die wordt uitgevoerd voorafgaand aan de procedure 4. Meerdere studies hebben de rol van FMT onderzocht voor de behandeling van colitis en diarree veroorzaakt door opportunistische pathogeen C. difficile (CDI). De incidentie van CDI blijft stijgen en is een belangrijke oorzaak van morbiditeit en mortaliteit met een grote economische last wereldwijd. Eerstelijns behandeling voor CDI bestaat uit behandeling met antibiotica, maar recidieven zijn gemeld tussen 15-35% 5. Verschillende case series en rapporten hebben doencumented de veiligheid en werkzaamheid van FMT voor CDI ongevoelig zijn voor de standaard medische behandeling met antibiotica 4,6-16. Een studie naar de lange termijn follow-up van patiënten na FMT via colonoscopie voor CDI meldde een 91% lagere genezingspercentage bij 77 patiënten 17. In ons centrum (Brigham and Women&#39;s Hospital Afdeling Maag-, Leverziekten en endoscopie), we begonnen met een fecale transplantatie programma voor patiënten met terugkerende of refractaire CDI. Geschikte kandidaten worden gedefinieerd als patiënten met recidiverende CDI (een geschiedenis van 3 of meer episodes of 2 episodes die vereist hospitalisatie) of patiënten met refractaire ziekte die reageert op traditionele antibiotica. Wij geloven dat een systematische aanpak van alle fasen van deze procedure maximaliseert werkzaamheid. In dit manuscript en de bijbehorende video, we detail het protocol dat we hebben gebruikt in ons centrum, welke patiënt en donor screening, kruk voorbereiding omvat, en de levering van de ontlasting eent de tijd van colonoscopie. Deze methode heeft positieve resultaten vergelijkbaar met de gepubliceerde literatuur opgeleverd. CASE PRESENTATIE: Deze patiënt is een typische patiënt doorverwezen voor fecale transplantatie. De patiënt is een 69-jarige vrouw met een geschiedenis van chronische lymfatische leukemie die terugval van de ziekte die verdere behandeling met chemotherapie gehad. Ze leed aan drie afleveringen van CDI in het afgelopen jaar, en werd daarom verwezen naar onze kliniek voor de behandeling van FMT voorafgaande aan het opnieuw starten van chemotherapie. Haar eerste aflevering van CDI vond plaats in oktober 2012. Ze geen voorgaande antibiotica had gehad. Ze was erg ziek in de ICU na presenteren met septische shock en onderging het omleiden lus ileostomie met antegrade vancomycine klysma&#39;s voor een 6 weekse cursus in combinatie met orale vancomycine, gezien de ernst van haar ziekte. Ze deed het erg goed met een resolutie van haar diarree en ze was in staat om te komenoff antibiotica. Ze ontwikkelde een hernia rond haar stoma dus het werd teruggedraaid. Kort na de stoma omkering ze weer ontwikkeld diarree en bleek opnieuw om C. te zijn difficile toxine positief. Ze voltooide een 6 weekse cursus van orale vancomycine en was in staat om met succes af afbouwen ervan. Maar enkele maanden later, opnieuw ontwikkelde ze diarree dat C. was difficile toxine positief en werd begonnen aan haar derde kuur met orale vancomycine. Ze was goed op antibiotica toen het werd opgemerkt dat haar witte telling verhoogd tot&gt; 100 en ze heeft gevonden om een ​​herhaling van haar CLL op beenmergpunctie hebben. Haar oncologen waren bezorgd over het initiëren van chemotherapie zonder volledige resolutie van haar CDI en dus werden we gevraagd om FMT voeren.

Intestinale microbiota Transplantation (FMT) is een veilige en zeer effectieve behandeling voor recidiverende en refractaire C. difficile infectie (CDI). Verschillende methoden van FMT administratie zijn gemeld in de literatuur, waaronder neussonde, endoscopie, klysma en colonoscopie. FMT via colonoscopie rendementen uitstekende genezen tarieven en is ook goed verdragen. We hebben ontdekt dat patiënten vinden dit een aanvaardbare en aanvaardbaar wijze van levering. We hebben 24 FMTs op 22 patiënten uitgevoerd op onze Center (tabel 1) met de bovenstaande protocol.
