Ødeleggende skjelettskader, kronisk slitasjegikt, og de alvorlige problemene forbundet med rekonstruktiv kirurgi påvirke økonomisk produktivitet, familie trivsel og livskvalitet. Mens små pauser og lesjoner kan helbrede ganske godt, mennesker er ikke i stand til å reparere store defekter og derfor må stole på rekonstruktiv prosedyrer for å gjenopprette struktur og funksjon. Rekonstruksjon kan innebære allogene eller heterogeneic grafts, morcellized bein, implanterte stillas, eller distraksjon Osteogensesis. Dessverre, ikke bare er det vedvarende sykelighet faktorer assosiert med disse behandlingene, men den opprinnelige styrken av reparert benet er sjelden oppnådd. Dermed er nye kliniske tilnærminger nødvendig. En måte å utvikle innovative metoder for å behandle segmentfeil er å studere situasjoner der storskala reparasjon forekommer naturlig. Amfibier famously kan regenerere skjelettelementer, mens pattedyr anses begrenset i ther evne. Men siden begynnelsen av det 20. århundre, noen få rapporter om gjenfødelse i menneske ribbe har blitt publisert som tyder på at mennesker ikke kan være så begrenset 1-4. Foreløpig er dette fenomenet er best kjent av plastiske kirurger som bruker rib materiale for kjeve, ansikt og øre gjenoppbygging, men det er ikke mer bredt verdsatt fem. For å studere dette reparasjon i mer detalj, har vi utviklet en kirurgisk modell ved hjelp av musen. Ved hjelp av denne protokollen, kan forskere identifisere de medfødte faktorer involvert og bruke denne informasjonen til å legge til rette for skjelett healing i andre steder. Det er mange fordeler med å bruke ribbeina som en modell for å studere skjelett reparasjon. Først de omkringliggende ribber gir en naturlig fiksatur (sammenlignet med reseksjon av lårbenet 6,7). Dette reduserer sykelighet risikoen for interne og eksterne fixators og forenkler den kirurgiske prosedyren. Dernest de tynne muskel lag av bryst wall sørge for enkel tilgang og god sikt som gjør analysen sammenlignes med den praktiske calvarial resections 8. For det tredje, i motsetning til den calvariae som dannes ved intramembranøse ossifikasjon, ribbene form ved endochondral ossifikasjon og vokser i lengde via forlengelse på vekstplater plassert på hver ende av en sentral diaphysis. Derfor kan reparasjon av ribbeina være mer sammenlignbare med reparasjon av de lange bein i appendicular skjelett. Videre har vi funnet at i forhold til femur, er periosteum av ribben tykkere og lettere kan manipuleres. Således kan undersøkere som ønsker å analysere benreparasjon i den hensikt å studere periosteum eller testing av celleterapier, farmakologiske midler, og / eller vev stillasene finner kirurgisk modell anvendelige. Oppsummert gir denne rib reseksjon modellen en sammenheng som å studere naturlige storskala benreparasjon i pattedyr som ingen slik modell i alminnelig bruk for øyeblikket eksisterer.

Denne protokollen introduserer forskere til en ny modell for storskala bein reparasjon utnytte musen rib. Fremgangsmåten beskriver følgende: fremstilling av dyret etter kirurgi, åpning av thorax kroppsveggen, utsette den ønskede ribbe fra de omkringliggende interkostalrom muskler, excising den ønskede delen av ribben uten å indusere en pneumothorax, og lukker snittene. Sammenlignet med bein av appendicular skjelett, ribbeina er svært tilgjengelig. I tillegg er det ikke nødvendig med intern eller ekstern fiksaturen siden de tilstøtende ribber gir en naturlig fiksering. Operasjonen bruker kommersielt tilgjengelige forsyninger, er grei å lære, og godt tolerert av dyret. Fremgangsmåten kan utføres med eller uten å fjerne den omgivende periosteum, og således bidrag av periosteum til reparasjon kan vurderes. Resultatene tyder på at hvis periosteum bibeholdes, oppstår robust reparasjon i 1 - 2 måneder. Vi forventer at bruken av denne protokollen vilstimulere forskning på rib reparasjon og at funnene vil lette utviklingen av nye måter å stimulere bein reparasjon i andre steder rundt om i kroppen.

Dyr vanligvis gjenopprette raskt fra denne prosedyren, snittet helbreder godt, og dyrene utviser normal atferd. Fremgangsmåten bruker kommersielt tilgjengelige forsyninger og etter trening, kan være ferdig på under 1 time.
