Hoewel we hebben beschreven hoe CEU gebruiken in een rat SCI kneuzing model, kan dit protocol worden aangepast om andere experimentele doelstellingen of SCI modellen passen. Wij hebben besloten om SCBF op twee tijdstippen te meten (vóór letsel en 15 min na SCI), maar het aantal tijdstippen en de vertraging tussen SCBF metingen kunnen worden aangepast aan de behoeften van andere studies voldoen. Traumatisch ruggenmergletsel (SCI) is een verwoestende aandoening die leidt tot significante verslechtering in de motorische, sensorische en autonome functies. Tot op heden is er geen therapie zijn efficiëntie patiënten aangetoond. Om die redenen is het van belang om nieuwe technieken die de evaluatie van mogelijke behandelingen zal verbeteren en verder verhelderen schade pathiophysiology 1 te identificeren. SCI is verdeeld in twee opeenvolgende fasen, aangeduid als primaire en secundaire verwondingen. Het primaire letsel komt overeen met de oorspronkelijke mechanische insult. Dat het secondaire verwonding groepen een cascade van verschillende biologische gebeurtenissen (zoals ontsteking, oxidatieve stress en hypoxie) die verder bijdragen tot de geleidelijke uitbreiding van het oorspronkelijke letsel, weefselschade en daarom 2,3 neurologische uitval. Bij de acute fase van SCI, worden neuroprotectieve therapieën die gericht zijn op het verminderen van de secundaire verwondingen pathologie en shOuld dienovereenkomstig verbeteren neurologische uitkomsten. Onder de vele secundaire schade events, ischemie speelt een cruciale rol 4,5. Op het niveau van de SCI epicentrum, de beschadigde parenchymale microvessels belemmert effectieve ruggenmerg bloedstroom (SCBF). Bovendien SCBF is ook significant in de regio rond het letsel epicentrum, een gebied bepaald zogenaamde &quot;ischemische penumbra zone&quot; verminderd. Als SCBF niet snel kan worden hersteld binnen deze regio, kan ischemie leiden tot aanvullende parenchymale necrose en verder zenuwweefsel schade. Als zelfs de geringste weefsel behoud substantiële effecten van de functie kan hebben, is het van groot belang zijn voor geneesmiddelen en therapieën die kunnen verminderen ischemie post-SCI ontwikkelen. Om dit fenomeen te markeren heeft voorgaand werk aangetoond dat het behoud van slechts 10% van gemyeliniseerde axonen was genoeg voor het lopen mogelijk bij katten na SCI 6. Hoewel diverse technieken zijn beschreven SCBF het beoordeleny hebben allemaal aanzienlijke beperkingen. Bijvoorbeeld, het gebruik van radioactieve microsferen 7,8 en C14-iodopyrine autoradiografie 9 vereist daaropvolgende offeren van dieren en kunnen niet worden herhaald op latere tijdstippen. De waterstof wordt gezuiverd 10 afhankelijk van de invoeging van intraspinale elektroden die het ruggenmerg verder beschadigen. Terwijl laser Doppler imaging, plethysmografie 14,15 en in vivo lichtmicroscopie 16 een bijzonder beperkte diepte / meetgebied 11-13. Ons team heeft eerder aangetoond dat contrastversterkte ultrasoon (CEU) beeldvorming kan worden gebruikt voor het real time beoordelen en in vivo de SCBF veranderingen in de rat ruggemerg parenchym 17. Het is belangrijk op te merken dat een soortgelijke techniek Huang et al toegepast. In een varkensmodel van SCI 18. CEU geldt een specifieke vorm van echografie, die het mogelijk maakt om grijswaarden morfologische im associërenleeftijden (verkregen door de conventionele B-mode) met ruimtelijke verdeling van de bloedstroom 19. De SCBF beeldvorming en kwantificatie vertrouwt op intravasculaire injectie van echo-contrastmiddelen. Het contrastmiddel bestaat uit zwavelhexafluoride microbellen (gemiddelde diameter van ongeveer 2,5 pm en 90% een diameter kleiner dan 6 um) gestabiliseerd door fosfolipiden. De microbellen tijdens de ultrasone bundel uitgezonden door de probe zo bloed-echogeniciteit verbeterd en wordt het contrast van de weefsels volgens de bloedstroom. Het is derhalve mogelijk om de bloedstroom te evalueren in een bepaalde regio van belang volgens de intensiteit van het gereflecteerde signaal. De microbellen zijn ook veilig en ze zijn klinisch toegepast bij de mens. Het zwavelhexafluoride snel geklaard (gemiddelde terminale halfwaardetijd is 12 min) en meer dan 80% van de toegediende zwavelhexafluoride wordt teruggevonden in de uitgeademde lucht binnen 2 min na injectie. Dit protocol voorziet in een eenvoudige manier om CEU im gebruikenaging te SCBF veranderingen in de rat te beoordelen.

Verminderde ruggenmerg bloedstroom (SCBF) (dwz ischemie) speelt een belangrijke rol in de traumatische dwarslaesie (SCI) pathofysiologie en is daarom een belangrijk doelwit voor neuroprotectieve therapieën. Hoewel diverse technieken zijn beschreven SCBF beoordelen, hebben allemaal belangrijke beperkingen. Om dat laatste te overwinnen, stellen wij het gebruik van real-time contrast versterkte echografie (CEU). Hier beschrijven we de toepassing van deze techniek in een rat kneuzing model van SCI. Een halsader katheter eerst geïmplanteerd voor herhaalde injectie van contrastmiddel, een natrium- chlorideoplossing van zwavelhexafluoride ingekapselde microbellen. De wervelkolom wordt vervolgens gestabiliseerd met een op maat gemaakte 3D-frame en het ruggenmerg dura mater wordt blootgesteld door een laminectomie bij Thix-ThXII. De ultrasone sonde wordt vervolgens geplaatst op het achterste deel van de dura mater (bedekt met echografie gel). Om basislijn SCBF, een enkele intraveneuze injectie (400 pl) van contra beoordelenst middel wordt toegepast op zijn passage op te nemen door de intacte ruggenmerg microvasculatuur. Een gewicht-drop-apparaat wordt vervolgens gebruikt om een ​​reproduceerbare experimentele kneuzing model van SCI te genereren. Contrastmiddel wordt opnieuw geïnjecteerd 15 min na de verwonding aan post-SCI SCBF veranderingen te beoordelen. CEU zorgt voor real-time en in-vivo evaluatie van SCBF veranderingen na SCI. In de niet gewonde dier, echografie toonde ongelijkmatige doorbloeding langs de intacte ruggenmerg. Bovendien 15 min na SCI, was kritisch ischemie op het niveau van het epicentrum terwijl SCBF bewaard gebleven in de meer afgelegen gebieden intact. In de gebieden naast het epicentrum (zowel rostraal en caudaal) werd SCBF aanzienlijk verminderd. Dit komt overeen met de eerder beschreven &quot;ischemische penumbra zone&quot;. Deze tool is van groot belang voor het beoordelen van de effecten van therapieën die gericht zijn op het beperken van ischemie en de resulterende weefselnecrose na SCI.

Met de hierboven beschreven protocol, is het mogelijk om de SCBF langs een longitudinale ruggengraat sagittale segment kaart. In het intacte ruggenmerg, blijkt er SCBF onregelmatigheden in het parenchym (figuur 12) zijn. Dit kan worden verklaard door de verschillen in verdeling radiculo-medullaire slagaders (RMA) van het ene dier op het andere.
