Hospitalisatie van volwassenen voor community-acquired pneumonie (CAP) draagt ​​een gedocumenteerde risico op cardiale complicaties die bijdraagt ​​aan de sterfte 1-4. In een recente studie van Corrales-Medina et al., Cardiale complicaties bleken geassocieerd te worden met en / of verantwoordelijk voor 27% van de longontsteking gepaard sterfgevallen 3. In een recent PLoS Pathogens artikel van Brown et al., S. pneumoniae werd gevonden kunnen translocatie via cardiaal vasculair endotheel, binnenkomst in de myocardium en vorming van discrete niet-purulent microscopische lesies (microverwondingen) gevuld met bacteriën in de ventrikels gedurende invasieve pneumokokkeninfecties (IPD) 7. Bewijs van cardiale microlesion vorming werd waargenomen in cardiale monsters van niet-menselijke primaten en mensen die bezweken aan pneumokokken infectie. Ook experimenteel geïnfecteerde muizen reproduceerbaar ontwikkeld cardiale microverwondingen. In muizen, werd microlesion grootte en aantal positief gecorreleerd met de duur en ernst van bacteremie niveaus van troponine in sera en afwijkende cardiale elektrofysiologie. Bacteriële translocatie naar het hart te treden via dezelfde mechanismen die translocatie van de pneumococcus over de bloed-hersenbarrière en ontwikkeling van pneumococcen meningitis, dwz Choline-bindende eiwit A gemedieerde invasie van vasculaire endotheelcellen in een laminine receptor en bloedplaatjes -activating factor receptor afhankelijke manier 8. Tijdens de vroege stadia van de infectie microverwondingen veroorzaakt door S. pneumoniae verschijnen als gebieden van beschadigde of ontstoken weefsel doet denken aan de pathologische tekenen van cardiomyopathie. Sommige monocyten kunnen worden waargenomen tijdens deze periode, maar hun aanwezigheid is kortstondig en snel lesies necrotisch in verschijning en gevuld met bacteriën tegelijkertijd blijft grow in grootte tot de dood van het dier of antimicrobiële interventie. Belangrijk binnen 3 dagen na antibioticum interventie wordt overvloedig immune cel infiltratie waargenomen bij de vroegere laesie en dit gaat gepaard met sterke collageen afzetting. Soortgelijke remodellering is gemeld optreden na infarct met blijvende gevolgen op de hartfunctie 11-15. Zo microverwondingen zijn een potentiële verklaring voor de ongunstige cardiale gebeurtenissen die zich voordoen tijdens de IPD en eventueel de verhoogde incidentie van cardiale-gerelateerde sterfte in herstellingsoord personen die de ziekte episode hebben overleefd. Hierin wordt instructie die op de experimenteel muismodel van IPD en cardiale laesievorming en visualisatie van cardiale microverwondingen op vroege en late stadia van de infectie. Het protocol voor detectie van collageen in dieren die zijn opgeslagen door antimicrobiële maatregelen blijkt. Het doel van dit artikel is om facilitate het onderzoek van andere onderzoekers over dit belangrijke en nieuwe pneumokokken pathologie.

Tijdens bacteremia Streptococcus pneumoniae kan verplaatsen door het vasculair endotheel in het myocardium en vormen discrete bacteriën gevulde microscopische lesies (microverwondingen) die opmerkelijk zijn vanwege de afwezigheid van infiltrerende immuuncellen. Door hun vrijgave van cardiotoxische producten, S. pneumoniae binnen microverwondingen worden verondersteld bij te dragen tot het hartfalen die vaak waargenomen tijdens fulminaat invasieve pneumococcen ziekte bij volwassenen. Hierin wordt aangetoond een protocol voor experimentele muis-infectie die leidt tot cardiale microlesion vorming reproduceerbaar binnen 30 uur. Instructie wordt voorzien op microlesion identificatie in hematoxyline &amp; eosine gekleurde hart secties en de morfologische verschillen tussen vroege en late microverwondingen worden gemarkeerd. Instructie wordt verstrekt over een protocol voor de controle van S. pneumoniae binnen microverwondingen behulp van antilichamen tegen pneumokokken capsulaire polysaccharideimmunofluorescentie microscopie. Eindelijk is een protocol voor antibiotische interventie die geïnfecteerde muizen en voor de detectie en beoordeling van littekenvorming in de harten van de herstellende muizen redt verstrekt. Samen zullen deze protocollen het onderzoek naar de moleculaire mechanismen die ten grondslag liggen pneumokokken cardiale invasie, cardiomyocyte dood, cardiale remodeling als gevolg van blootstelling aan S. vergemakkelijken pneumoniae en de immuunrespons op het pneumokokken in het hart.
