Kanker is een klonale ziekte. Sinds dertig jaar geleden Peter C. Nowell voorgesteld kanker klonale evolutie model 1, hebben vele studies geprobeerd klonale populaties ontleden in tumormonsters reconstrueren klonale expansie en evolutie patronen die de tumorigenese proces 2 ten grondslag liggen. Onlangs heeft het hele genoom sequencing onderzoekers nodig om een diepe blik op de klonale heterogeniteit en de evolutie 3,4 nemen. Echter, vanwege het gebrek aan handelbaar markers in vele celtypen is het moeilijk om de precieze klonale architectuur en evolutionaire weg afleiden. Gelukkig is er een natuurlijke klonaliteit marker in rijpe B-cellen van die vele lymfoïde maligniteiten, met inbegrip van DLBCL, afkomstig. In antwoord op antigen stimulatie, kan elke B-cel een productieve IgH VDJ sequentie samen door bij een VH (variabel), D (diversiteit), en J H (verbinden) segment uit een grote pool van deze segmenten. Tijdens deze process kunnen kleine gedeelten van de oorspronkelijke sequentie verwijderd en aanvullende niet-matrijs nucleotiden worden toegevoegd aan een unieke VDJ herrangschikking te creëren. Deze specifieke VDJ omlegging kan worden overgenomen in alle nakomelingen van deze B-cellen, dus etiketteren individuele mature B-cel en zijn nakomelingen 5. Bovendien SHM gebeurt op gerecombineerde VDJ-sequenties in de daaropvolgende kiemcentra (GC) reactie aanvullende mutaties voor de uitbreiding van het antilichaam pool en de versterking van antilichaamaffiniteit 6. Daarom is door het vergelijken en contrasterende VDJ en SHM patronen van lymfoom monsters die deze processen hebben ondergaan, intra-tumor heterogeniteit kon worden afgebakend en klonale evolutie pad van de ziekte kan worden afgeleid. Voorheen kon VDJ herrangschikking en SHM geïdentificeerd met PCR amplificeren van het gerecombineerde gebieden, het kloneren van de PCR producten en vervolgens Sanger sequentiebepaling sequentie informatie te verkrijgen. Deze aanpak is lage doorvoer en lage opbrengst binnenhalen slechts een klein deel van de gehele gerecombineerde VDJ repertoire en belemmeren de karakterisering van de algemene representatie van de klonale populatie binnen een bepaald monster. Een aangepaste aanpak werd gecreëerd door het genereren van NGS geïndexeerd sequencing bibliotheken van VDJ PCR-producten en het uitvoeren van PE 2x150 bp sequencing om meer dan een half miljoen gerecombineerde VDJ sequenties per monster te verkrijgen. Daarnaast werd een aangepaste pipeline ontwikkeld voor kwaliteitscontrole (QC) uitvoeren, uitlijnen, filter VDJ sequentiebepaling leest herschikkingen en SHMS van elke lees- identificeren en uitvoeren fylogenetische analyse van de klonale architectuur van elk monster. Daarnaast is een nieuwe benadering ingesteld om verder te karakteriseren de klonale evolutie patronen voor monsters in verschillende ziektestadia. We hebben deze techniek DLBCL patiënt monsters toegepast. DLBCL is een agressieve vorm van non-Hodgkin lymfoom frequent terugval in maximaal één thIRD van de patiënt 7. DLBCL recidieven normaal optreden vroeg, binnen 2 tot 3 jaar van de initiële diagnose, hoewel sommige doen zich na 5 jaar 8. Prognose voor recidiverende patiënten is slecht, met slechts 10% bereiken van 3 jaar progressievrije overleving als gevolg van beperkte behandelingsmogelijkheden. Dit is de basis voor de dringende behoefte aan nieuwe benaderingen van DLBCL terugval 9,10 behandelen. Echter, moleculaire mechanismen geassocieerd met DLBCL terugval zijn nog grotendeels onbekend. Vooral de rol van klonale heterogeniteit bij diagnose en klonale veranderingen tijdens DLBCL terugval ontwikkeling momenteel gekarakteriseerde, waardoor het moeilijk om een ​​accurate en bruikbare biomarker terugval voorspellen definiëren. Om deze vragen te beantwoorden, passen we onze VDJ-sequencing aanpak op meerdere paren van overeenkomstige primaire diagnose-recidief DLBCL monster paren. Twee verschillende klonale evolutionaire scenario van terugval voortgekomen uit de vergelijking van de klonale architecturen tussen diagnose en recidief samples die meerdere moleculaire mechanismen betrokken zouden kunnen zijn bij DLBCL terugval.

vdj-seq: deep sequencing analysis of rearranged immunoglobulin heavy chain gene to reveal clonal evolution patterns of b cell lymphoma

Begrip tumor klonaliteit is van cruciaal belang voor het begrijpen van de betrokken zijn bij het ontstaan ​​van tumoren en de progressie van de ziekte mechanismen. Verder inzicht in de klonale samenstelling veranderingen die binnen een tumor in reactie op bepaalde micro-omgeving of behandelingen kunnen leiden tot het ontwerp van betere en meer doeltreffende methoden om tumorcellen te roeien. Echter, het bijhouden tumor klonale subpopulatie uitdagend vanwege het gebrek aan onderscheiden markeringen. Om dit probleem aan te pakken, werd een VDJ-seq protocol gemaakt om de klonale evolutie patronen van diffuus grootcellig B-cellymfoom (DLBCL) terugval op te sporen door gebruik VDJ recombinatie en somatische hypermutatie (SHM), twee unieke kenmerken van B-cel lymfomen. In dit protocol werden de next-generation sequencing (NGS) bibliotheken met indexering potentieel opgebouwd uit versterkte herschikte immunoglobuline zware keten (IgH) VDJ regio van paren van primaire diagnose en recidief DLBCL samples. Op werden gemiddeld meer dan een half miljoen VDJ sequenties per monster verkregen na sequencing, die zowel VDJ omlegging en SHM informatie bevatten. Daarnaast werden aangepast bioinformatica pijpleidingen ontwikkeld om sequentie-informatie voor het karakteriseren van IgH-VDJ repertoire in deze monsters volledig te benutten. Bovendien kan deze aanpak worden uitgebreid naar andere klinische aspecten, zoals identificatie minimale ziekte, monitoring terugval voortgang en respons op de behandeling, en het onderzoek van het immuunsysteem repertoirees in non-lymfoom contexten.

VDJ-Seq: Deep Sequencing Analysis of Rearranged Immunoglobulin Heavy Chain Gene to Reveal Clonal Evolution Patterns of B Cell Lymphoma

This protocol describes an approach to interrogate the recombined immunoglobulin heavy chain VDJ regions of lymphomas by deep-sequencing and retrieve VDJ rearrangement and somatic hypermutation status to delineate clonal architecture of individual tumor. Comparing clonal architecture between paired diagnosis and relapse samples reveals lymphoma relapse clonal evolution modes.
