Abdominalen Adhäsionen sind eine Form von Narbengewebe, das in Reaktion auf eine Entzündung im Abdomen bilden, in der Regel nach einem chirurgischen Eingriff oder intraabdominale Infektion. Adhäsionen sind eine der Hauptursachen für chronische Bauchschmerzen und Unfruchtbarkeit, und sind die häufigste Ursache von Dünndarmobstruktion 1. Das Vorhandensein von Adhäsionen macht die Durchführung einer zweiten Bauchoperation schwieriger und erhöht die Wahrscheinlichkeit von Komplikationen 2. Trotz jahrelanger Forschung, bleiben die Mechanismen, die die Bildung von Adhäsionen zugrunde liegenden schlecht verstanden. Es ist bekannt , dass eine anfängliche Schädigung der peritonealen Oberfläche eine Exsudation von Fibrin-reichen Flüssigkeit verursacht, die dann ein Gerinnsel bildet , das die Oberflächen des Darms und der Bauchwand 3 zusammen bindet. Später wandern Fibroblasten und andere Zellen in die Klebefläche und Bindegewebe 4 absondern. Über Monate bis Jahre reift die Adhäsion von Blutgefäßen entwickeln und Nerven <sup&gt; 5. Mehrere kommerzielle Produkte existieren, die die Bildung von Adhäsionen nach Bauchchirurgie (zB Seprafilm) zu reduzieren sollen. Alle diese Produkte wirken als mechanische Barrieren und Adhäsionsbildung stoppen durch physischen Kontakt zwischen Darmschlingen und der Bauchwand 6,7 verhindern. Trotz des Beweises aus einer kontrollierten Studie , die eine chirurgische Adhäsionsbarriere die Bildung von Adhäsionen verringert 8, anekdotisch viele Chirurgen haben mit der Wirksamkeit der mechanischen Barriere Produkte enttäuscht worden. Momentan gibt es keine Drogen-basierte Anti-Adhäsions-Therapien, was die Tatsache widerspiegelt, dass die genauen in Adhäsionsbildung beteiligten Prozesse kaum verstanden werden. Entwicklung einer Therapie, die bei der Bildung beteiligten zellulären oder molekularen Mittel von Adhäsionen speziell Ziele wird ein verbessertes Verständnis der Ereignisse erfordern, die in der Bildung der Adhäsion beteiligt sind. Mehrere Gruppes haben molekularen Mechanismen identifiziert , die für die Bildung von Adhäsionen 9-11 wichtig sein kann. Tiermodelle bieten eine hervorragende Umgebung für die Bildung von Verwachsungen zu studieren. Abdominal adhesions consist of fibrotic tissue that forms in the peritoneal space in response to an inflammatory insult, typically surgery or intraabdominal infection. The precise mechanisms underlying adhesion formation are poorly understood. Many compounds and physical barriers have been tested for their ability to prevent adhesions after surgery with varying levels of success. The mouse and rat are important models for the study of abdominal adhesions. Several different techniques for the creation of adhesions in the mouse and rat exist in the literature. Here we describe a protocol utilizing abrasion of the cecum with sandpaper and sutures placed in the right abdominal sidewall. The mouse is anesthetized and the abdomen is prepped. A midline laparotomy is created and the cecum is identified. Sandpaper is used to gently abrade the surface of the cecum. Next, several figure-of-eight sutures are placed into the peritoneum of the right abdominal sidewall. The abdominal cavity is irrigated, a small amount of starch is applied, and the incision is closed. We have found that this technique produces the most consistent adhesions with the lowest mortality rate.
