La insuficiencia cardíaca es común y mortal. Es una enfermedad progresiva que da como resultado la contractilidad disminuida del corazón, lo que impide el flujo de sangre a los órganos y las hojas de las demandas metabólicas de la insatisfecha cuerpo. Se estima que 5,7 millones de estadounidenses tienen insuficiencia cardiaca y es la primera causa de hospitalización en los Estados Unidos 9. El costo colectiva de tratamiento de los pacientes en la insuficiencia cardíaca en los Estados Unidos excede $ 300 mil millones de dólares por año 9-10. El único tratamiento definitivo para la insuficiencia cardíaca en fase terminal es el trasplante ortotópico de corazón. Cada año, se estima que se necesitan más de 100.000 corazones de donantes para procedimientos de trasplante cardíaco en los Estados Unidos 1-2. Debido al limitado número de donantes, sólo aproximadamente 2.400 trasplantes se realizan cada año en los EE.UU. 2. Claramente, esta escasez de órganos debe ser abordado ya que se requieren otras estrategias para producir órganos adicionales para Transplantala e, idealmente, estos órganos sería autólogo a fin de evitar las complicaciones asociadas con el rechazo y la inmunosupresión de por vida. Si bien estos estudios, y los nuevos descubrimientos en tecnologías de células madre, están avanzando rápidamente, varios obstáculos aún no se han cumplido. Por ejemplo, las limitaciones en la preservación de la estructura nativa de la matriz, la integración celular en la pared de la matriz, y la capacidad para apoyar la proliferación y el crecimiento de todos limitan el éxito de este enfoque. Mientras que los atributos regenerativos superiores se han atribuido al corazón neonatal, los aspectos prácticos del uso de un pañuelo de papel tales han limitado su exploración. Sobre la base de la capacidad de regeneración demostrado del corazón neonatal, hemos desarrollado nuevas matrices mediante el desarrollo de unatécnica de descelularización para el corazón P3 ratón. El corazón P3 fue elegido para estos estudios, ya que se encuentra dentro de la ventana de la regeneración cardiaca como se determina previamente 6 pero el corazón es lo suficientemente grande para la cosecha, decellularize y recellularize. El objetivo de este estudio es demostrar la viabilidad de crear una matriz de un corazón neonatal de ratón. Nuestros estudios proporcionan evidencia de la viabilidad de decellularizing un minuto, corazón neonatal mientras se mantiene la integridad estructural y proteico de la ECM. neonatal cardiac scaffolds: novel matrices for regenerative studies

The only definitive therapy for end stage heart failure is orthotopic heart transplantation. Each year, it is estimated that more than 100,000 donor hearts are needed for cardiac transplantation procedures in the United States1-2. Due to the limited numbers of donors, only approximately 2,400 transplants are performed each year in the U.S.2. Numerous approaches, from cell therapy studies to implantation of mechanical assist devices, have been undertaken, either alone or in combination, in an attempt to coax the heart to repair itself or to rest the failing heart3. In spite of these efforts, ventricular assist devices are still largely used for the purpose of bridging to transplantation and the utility of cell therapies, while they hold some curative promise, is still limited to clinical trials. Additionally, direct xenotransplantation has been attempted but success has been limited due to immune rejection. Clearly, another strategy is required to produce additional organs for transplantation and, ideally, these organs would be autologous so as to avoid the complications associated with rejection and lifetime immunosuppression. Decellularization is a process of removing resident cells from tissues to expose the native extracellular matrix (ECM) or scaffold. Perfusion decellularization offers complete preservation of the three dimensional structure of the tissue, while leaving the bulk of the mechanical properties of the tissue intact4. These scaffolds can be utilized for repopulation with healthy cells to generate research models and, possibly, much needed organs for transplantation. We have exposed the scaffolds from neonatal mice (P3), known to retain remarkable cardiac regenerative capabilities,5-8 to detergent mediated decellularization and we repopulated these scaffolds with murine cardiac cells. These studies support the feasibility of engineering a neonatal heart construct. They further allow for the investigation as to whether the ECM of early postnatal hearts may harbor cues that will result in improved recellularization strategies.
