ARDS wordt gekenmerkt door wijdverbreide longontsteking en klinisch gezien als acute kortademigheid bij hypoxemie. Deze symptomen komen vaak minder dan 24 uur na een opzwepend gebeurtenis, zoals trauma, sepsis, bloedtransfusie gerelateerde reacties of aspiratie. Het wordt histopathologisch gekenmerkt door neutrofiele alveolitis (dwz wijdverspreide ontstekingen) gelokaliseerd alvéolaire epitheel en vasculaire endotheel leidt tot eiwitlekkage vervolgens hyaline membraanvorming. Aspiratie is gecategoriseerd als chemische longontsteking of aspiratiepneumonie. 1 De zure component van de maag aspiratie bijdraagt aan zowel het pneumonitis voorkeur een secundaire bacteriële longontsteking. Aspiratiepneumonie is een van de belangrijkste risicofactoren voor ALI en de verdere ontwikkeling van ARDS. 2 Maag-aspiratie is een acute gebeurtenis gedefinieerd als het inhaleren van materialen uit thij maag met of zonder orofaryngeale flora in de luchtwegen voorbij de stembanden. De inhoud aspireren kan een lage pH maag vocht, bacteriën, bloed of etensresten bevatten. Maag-aspiratie komt vaak voor bij patiënten op de Intensive Care Unit (ICU), die zijn meestal in een nuchtere toestand en dus geplaatst op een protonpompremmer om aspiratie van verzuurde maaginhoud te beperken. De incidentie van ALI in de ICU bevolking in de VS is 2,5-5 keer hoger in vergelijking met de algemene patiëntenpopulatie. 3 Helaas zijn deze predisponerende aandoeningen vaak leiden tot een toestand van bacteriële overgroei in de maag die kunnen leiden tot ernstige gevolgen in de longen na aspiratie evenement, maag streven is een onafhankelijke risicofactor voor de ontwikkeling van secundaire bacteriële longontsteking (SBP) , ALI en ARDS. Maag aspiratie heeft twee hoofdcomponenten: zoutzuur en maaginhoud, al dan niet bacteriën bevattenof voedsel deeltjes. In het knaagdiermodel de zure component alleen van maag aspiratie levert een aanvankelijke ontstekingsreactie ten gevolge van de directe bijtende schade van lage pH op luchtwegepitheel. Dit wordt gevolgd door een neutrofiele infiltratie en een ontstekingsreactie bij 4-6 uur. 4 Deze twee factoren uiteindelijk leiden tot de vernietiging van pulmonale microvasculaire integriteit hetgeen leidt tot extravasatie van vocht en eiwitten in alveoli en luchtwegen. Dit pathofysiologie begrijpen en verder te onderzoeken mogelijke therapeutische ingrepen, is het belangrijk om te ontwikkelen en te karakteriseren een diermodel dat de onderliggende mechanismen belicht. Een zure aspireren mag alleen omvangrijke of met een voldoende lage pH om de buffercapaciteit van de respiratoire boom omzeilen en bereiken de alveoli zijn. Als dit niet gebeurt, slechts een voorbijgaande bovenste, geleidende luchtwegen schade optreedt die minder waarschijnlijk leiden tot ernstige gevolgen van ARDS. 5 &lt;/ Sup&gt; De aspiratie gebeurtenis nauwkeurig met verwonde alvéolaire epitheel bootsen, is het belangrijk om de natuurlijke afweer van het dier te omzeilen. Een muis model streven een ALI dat het maagzuur letsel bij mensen nabootst produceren, moet men rekening houden met de verschillen in de luchtpijp-bronchiale boom. De open tracheostomie techniek die deze werkwijze gebruikt omzeilt de verschillen tussen muizen en humane respiratoire bomen en modellen de schade op een wijze die ALI reproduceert zowel fysiologisch en histologisch. Historisch gezien werd intratracheale intubatie gebruikt ALI genereren, maar het is moeilijk geacht presteren in muizen zonder larynx letsel. Daarom is deze methode biedt een mogelijk alternatief dat consistente resultaten over meerdere onderzoekers met minimale procedure toegeschreven sterfte opgeleverd.

Pneumonitis zuur is een belangrijke oorzaak van steriele acute longbeschadiging (ALI) bij mensen. Acid longontsteking overspant de klinische spectrum van asymptomatische tot acute respiratory distress syndrome (ARDS), gekenmerkt door neutrofiele alveolitis, en schade voor zowel alveolaire epitheel en vasculaire endotheel. Klinisch, ARDS wordt bepaald door acute hypoxemie, bilaterale fragmentarisch longinfiltraten en non-cardiogene longoedeem. Menselijke studies hebben ons waardevolle informatie over de fysiologische en ontstekingsreactie in de longen veroorzaakt door ARDS, wat heeft geleid tot diverse hypotheses over de ondergeschikte mechanismen. Helaas, moeilijkheden bepalen van de oorzaak van ARDS, evenals een groot aantal pathofysiologie hebben geleid tot een gebrek aan kritische informatie die bij de ontwikkeling van therapeutische strategieën zouden zijn. Bovendien is een reproduceerbare letsel met grote verschillen tussen de verschillende schadelijke en niet-schadelijke beledigingen belangrijk is. Open Tracheostomy Gastric Acid Aspiration Murine Model of Acute Lung Injury Results in Maximal Acute Nonlethal Lung Injury

This protocol induces acute lung injury in a mouse that has close fidelity to the pathogenesis of acid pneumonitis observed in humans.
