Oral hälsovård spelar en viktig roll i förhindrandet av opportunistiska infektioner och aspirationspneumoni i fysiskt sårbara äldre. Oral hälsovård, bestående av borstning efter varje måltid, rengöring av tandproteser, och ta emot professionell munhälsovård, spelar en viktig roll för att minska förekomsten av sådana sjukdomar 1-3. Framför allt är dålig munhygien starkt förknippat med aspirationspneumoni i fysiskt utsatta äldre. Därför bör mer uppmärksamhet ägnas åt den orala hälsovård för äldre patienter, som har begränsad förmåga att rengöra sina proteser själva 4. Även i utvecklade länder, fortfarande användningen av akryl, hartsbaserade avtagbara proteser den vanligaste behandlingsalternativ för tandlösa äldre patienter 5. Med tanke på deras höga vattenupptagningsförmåga, mikroporösa ytan och hydrofoba natur, akryl tandprotes baser tillverkade av polymetylmetakrylat (PMMA) lätt ansamlas plack på sina ytor, som kallas protes plack. 2-metakryloyloxietyl fosforylkolin (MPC) polymerer är välkända biomedicinska polymera material 6, som har anmärkningsvärt antitrombogenicitet och vävnadskompatibilitet på grund av deras höga motståndskraft mot proteinadsorption och cellvidhäftning 7-11. MPC polymerbeläggningar har använts i olika medicinska anordningar för att förhindra infektion 12 och har föreslagits för att ha potential att förhindra protesen plack samlas på PMMA protes ytor 13. Utmaningen i den kliniska tillämpningen av MPC polymerer PMMA tandproteser är att på ett tillförlitligt sätt binda dem till PMMA ytor på ett enkelt och stabilt sätt. Tidigare har MPC polymerer rapporterats vara stabilt bunden till PMMA ytor med en ympning teknik och ha potential att hämma plackansamling. Emellertid är ympningstekniken inte enkel och kräver specialiserad utrustning, which gör sin kliniska tillämpning utmanande. Här beskriver vi ett nytt MPC beläggningsprocedur som utnyttjar fotoreaktiva fosfolipid polymerer-specifikt, en fotoreaktiv metakrylat-derivat, 2-metakryloyloxietyl-4-azidobensoat (MPAz) -synthesized att åstadkomma en variation av ytmodifieringar av olika typer av material. MPAz samin polymeriseras med MPC polymerer och n-butylmetakrylat (BMA) för att ge poly (MPC- co -BMA- co -MPAz) (PMBPAz). PMBPAz kan kovalent binda till ytorna av PMMA proteser via aktivering av aziden grupperna i MPAz under ultraviolett (UV) ljus-bestrålning 14. Förfarandet kräver ingen specialutrustning och kan kompletteras med kliniker inom två minuter. Vi tillämpade även detta förfarande i en klinisk miljö och visat sin kliniska nyttan och effektivitet i att hämma plackavsättning på avtagbara proteser.

2-methacryloyloxyethyl phosphorylcholine polymer treatment of complete dentures to inhibit denture plaque deposition

Removable dentures made of poly (methyl methacrylate) (PMMA) are prone to bacterial adherence and dental plaque formation, which is called denture plaque. Denture plaque-associated infection is a source of serious dental and medical complications in the elderly. 2-Methacryloyloxyethyl phosphorylcholine (MPC) is a well-known biomedical material that exhibits marked antithrombogenicity and tissue compatibility because of its high resistance to protein adsorption and cell adhesion. Therefore, MPC polymer coatings are suggested to have the potential to inhibit plaque deposition on the surface of PMMA dentures. However, coating MPC polymer on the surface of a PMMA denture is a complex procedure that requires specialized equipment, which is regarded as a major barrier to its clinical application. 
Here, we introduce a new MPC polymer treatment procedure that uses poly (MPC-co-BMA-co-MPAz) (PMBPAz) to prevent denture plaque deposition on removable dentures. This procedure enables the MPC coating of PMMA denture surfaces in a simple and stable manner that is resistant to various chemical and mechanical stresses due to the MPC layer of PMBPAz that is covalently bound to the PMMA surface by ultraviolet light irradiation. In addition, the procedure does not require any specialized equipment and can be completed by clinicians within 2 min. We applied this procedure in a clinical setting and demonstrated its clinical utility and efficacy in inhibiting plaque deposition on removable dentures.
