adsc-sheet transplantation to prevent stricture after extended esophageal endoscopic submucosal dissection

In de afgelopen jaren, heeft de cel-sheet construct brede interesse in de regeneratieve geneeskunde gestimuleerd, in het bijzonder voor reconstructieve chirurgie procedures. De ontwikkeling van gediversifieerde technologieën combineren vetweefsel afgeleide stromacellen (ADSCs) met verschillende biomaterialen heeft geleid tot de bouw van talrijke soorten weefselengineering substituten, zoals bot, kraakbeen en vetweefsel van knaagdieren, varkens of menselijke ADSCs. Extended slokdarm endoscopische submucosale dissectie (ESD) is verantwoordelijk voor de slokdarm strictuurvorming. Vernauwing preventie blijft uitdagend, zonder efficiënte behandelingen beschikbaar. Vorige studies de effectiviteit van mucosale cel vel transplantatie in een hond model en bij mensen. ADSCs toegeschreven anti-inflammatoire eigenschappen, lokale immuunmodulerende effecten, neovascularisatie inductie en differentiatie vaardigheden in mesenchymale en niet-mesenchymale lineages. Deze originele studie beschrijft de endoscopische transplantation van een ADSC-tissue engineered construct oesofageale vernauwing in een varkens model te voorkomen. De ADSC construct bestond uit twee allogene ADSC bladen lagen op elkaar op een papieren drager membraan. De ADSCs werden gelabeld met de PKH67 fluorofoor to-probe gebaseerde confocale laser endomicroscopy (PCLE) controle mogelijk te maken. Op de dag van transplantatie, een 5-cm en hemi-omtrek ESD bekend oesofageale vernauwing te induceren werd uitgevoerd. Dieren werden onmiddellijk endoscopisch getransplanteerd met 4 ADSC constructen. De volledige hechting van de ADSC constructen werd verkregen na 10 min zachte toepassing. Dieren werden opgeofferd op dag 28. Alle dieren werden met succes getransplanteerd. Transplantatie werd bevestigd op dag 3 met een positieve PCLE evalueren. Vergeleken met getransplanteerde dieren, controle dieren ontwikkelde ernstige vernauwingen, met de belangrijkste fibrotisch weefsel ontwikkeling, vaker voedings- problemen, en verminderde gewichtstoename. In ons model, de transplantatie van allogenic ADSCs, georganiseerd in de dubbele cel vellen, na langere ESD was succesvol en sterk geassocieerd met een lagere oesofageale vernauwing tarief.

De cultuur van ADSCs en de procedure voor de ADSC vel te verkrijgen wordt getoond in figuur 1. Figuur 2 toont de constructie van het implantaat, bestaande uit twee bladen ADSC lagen op elkaar uit papier dragermembraan.
