adsc-sheet transplantation to prevent stricture after extended esophageal endoscopic submucosal dissection

Under tidigare år har cell ark konstruktion sporrade stort intresse för regenerativ medicin, särskilt för rekonstruktiv kirurgi förfaranden. Utvecklingen av diversifierade teknik kombinerar adipos vävnad stromaceller (ADSCs) med olika biomaterial har lett till byggandet av många typer av vävnadstekniska substitut, såsom ben, brosk och fettvävnad från gnagare, svin eller människor ADSCs. Utökad matstrupen endoskopisk submukosala dissektion (ESD) är ansvarig för esofagus strikturbildning. Förträngning förebyggande förblir utmanande, utan effektiva behandlingar som är tillgängliga. Tidigare studier rapporterade effektiviteten av mucosal cell ark transplantation i en hundmodell och i människor. ADSCs skrivs anti-inflammatoriska egenskaper, lokala immunmodulerande effekter, neovaskularisation induktion och differentiering förmågor in i mesenkymala och icke-mesenkymala härstamningar. Denna ursprungliga studien beskriver endoskopisk transplantation av en ADSC vävnadsutvecklad konstruktion för att förhindra matstrupen striktur i en svinmodell. Den ADSC konstruktet var sammansatt av två allogena ADSC ark skiktade på varandra på en pappersstödmembran. De ADSCs märktes med PKH67 fluoroforen att tillåta sondbaserad konfokala laser endomicroscopy (pCLE) övervakning. På dagen för transplantation, var en 5-cm och hemi-omkrets ESD känd för att inducera esofageal striktur utförs. Djuren omedelbart endoskopiskt transplanteras med 4 ADSC konstruktioner. Den fullständiga vidhäftning av ADSC konstruktionerna erhölls efter 10 min av försiktig applicering. Djuren avlivades på dag 28. Alla djuren framgångsrikt transplanteras. Transplantation bekräftades på dag 3 med en positiv pCLE utvärdering. Jämfört med transplanterade djur utvecklade kontrolldjur allvarliga förträngningar, med större fibrotisk vävnadsutveckling, mer frekventa matsmältnings problem och minskad viktökning. I vår modell, transplantation av allogenic ADSCs, organiserade i dubbla cellskikt efter förlängd ESD var framgångsrik och starkt förknippad med en nedre matstrupen striktur hastighet.
