subcutaneous trigeminal nerve field stimulation for refractory facial pain

만성 또는 신경 병증 성 삼차 신경통은 치료가 어려울 수 있습니다. 보수 치료가 실패하면 신경 외과 적 치료가 적용되어야합니다. 만성 안면 통증에 대한 신경 변조 기술은 수행하기가 복잡한 심부 뇌 자극 및 운동 피질 자극을 포함합니다. 피하 신경장 자극은 만성 요통에 대해 인증되었으며 신경 변조의 가장 침습성이 적은 형태입니다. 우리는이 기술을 적용하여 만성 및 신경 병증 성 삼차 신경통을 개별 치료 개념으로 치료했습니다. 첫째, 시험 자극이 수행됩니다. 피하 리드는 국소 마취 하에서 고통스러운 삼차 신경 피부 진피에 놓입니다. 리드는 지속적인 자극을 적용하는 외부 신경 자극기에 연결됩니다. 환자는 자극의 효과를 평가하기 위해 12 일간의 외래 환자 시험을 받는다. 전극은 재판 후에 제거됩니다. 환자가 통증 및 / 또는 공격 빈도가 50 % 이상 감소하면 통증 감소가약물 치료 또는 삶의 질 향상, 영구 이식 예정입니다. 새로운 전극은 전신 마취 하에서 이식되고, 쇄골 내 내부 펄스 발생기에 피하에 터널링된다. 환자는 필요에 따라 자극을 켜고 끌 수 있고 자극의 크기를 늘리거나 줄일 수 있습니다. 이 기술은 운동 피질 자극이나 심부 뇌 자극과 같은 삼차 신경 통증에 대한 신경 변조의 다른보다 침략적 인 수단에 대한 최소 침습성 대안을 나타냅니다.
